BACKGROUND: Symphalangism is a rare congenital difference characterized by ankylosis of interphalangeal (IP) joints of the fingers and toes. In adults, there were several attempts to restore the stiff joints into mobile ones, but these treatment options resulted in poor outcomes and could not be applied to growing children. Here, we report our experiences on surgical treatment for children who had symphalangism of the hand.
METHODS: We treated 36 joints in 17 children with symphalangism of the hand using dorsal capsulotomy and collateral ligament release. The diagnoses were based on history,

PURPOSE: This study evaluated the outcomes of reconstructive techniques combined with a modified index finger rotation flap for complex congenital clasped thumbs, focusing on functional improvement, aesthetic results, and donor site morbidity.
METHODS: A retrospective review of nine patients (10 thumbs) who underwent modified index finger rotational flap reconstruction with Z-plasty donor site closure between 2022 and 2024 was conducted. Variables collected included patient demographics, preoperative first intermetacarpal angle, and postoperative outcomes, which included active radial and pal

INTRODUCTION: Acute closed trauma to the long fingers and thumb presents a therapeutic challenge. Ultrasound refines clinical examination and enables optimal management. This study aims to assess the role of ultrasound in the management of acute closed trauma of the fingers and the thumb during post-emergency consultations.
METHOD: Patients were re-evaluated within 15 days of their initial emergency visit. The evaluation combined a clinical examination and a standardized ultrasound assessment. Based on ultrasound findings, initial management was either confirmed or modified. The primary endpo

Previous studies suggested that a pronounced weakness of the extensor muscles relative to the flexor muscles could increase the risk of occu … se capacities were then used to compute the extensor/flexor capacity ratios about the wrist and the finger joints. Compared to healthy non-players, healthy players presented higher extensor muscle capacities and greater capacity ratios showing that playing tennis generates specific adaptations of muscle capacities. We reviewed 9 (3-15) years postoperatively all 43 two-stage flexor tendon transplantations in fingers that had not otherwise been severely injured that we had operated on during the years 1984 to 1996. One digit had been reconstructed in each patient. The interval between the first and second stage operations was 19 (14-51) weeks. Time away from work was 44 (4-140) days after the first procedure and 101 (38-297) days after the second stage operation. After the second stage, 26 further procedures had been done in 18 of the 43 fingers. These included seven resutures of the transplanted tendon (t

Objective To characterise posttraumatic constrains of the volar-lateral ligaments by analysing volar plate (VP) dynamics after proximal interphalangeal (PIP) joint injuries using ultrasonography. Materials and methods From the anatomical and biomechanical perspectives of the VP and its surrounding structures, posttraumatic constrains of the volar-lateral ligament were evaluated by analysing the changes of VP motion. Using ultrasound, VP motion during active flexion of 0-60° was recorded in the central sagittal plane at 12 weeks after injury. VP trajectories visualised by 5-point tracing on the

Twenty-eight vascularized toe-joint transfers performed on 25 patients were reviewed. A number of different techniques were used: proximal interphalangeal joint or metacarpophalangeal joint reconstruction, one-stage double joint transfer, and interphalangeal thumb or trapezometacarpal joint replacement. Using these types of vascularized joint transfer allows one-stage composite transfer (including skin, bone, and extensor tendon) and provides rapid bone healing, potential growth in the young, good long-term cartilage preservation, normal lateral stability in pinch, and limited but useful range

Congenital swan neck deformity is a rare condition, secondary to congenital laxity of the palmar plate of the proximal interphalangeal joint. Here, we describe a new surgical method which is based on the palmar plate reinforcement using the Mitek mini anchor system (Mitek Surgical Products Inc., Norwood, MA, USA). Four fingers of a patient with congenital swan neck deformities were corrected with this technique at two stages. First, the anchor was inserted into the volar surface of the proximal phalanx. The two sutures of the anchor system were then crossed the PIP joint in a V fashion. Two ho

PURPOSE: To evaluate wrist and finger joints in patients with rheumatoid arthritis (RA) by grayscale, power Doppler (PD) and contrast-enhanced musculoskeletal ultrasonography (US) and to compare these findings with MRI, clinical (DAS28) and laboratory (ESR; CRP) data.
MATERIALS AND METHODS: US was performed at baseline (t0) and after three, six and twelve (t12) months before and after a change of medical treatment. MRI was carried out at t0 and t12 and used as the reference method. Contrast-enhanced US was used to assess one clinically most affected joint region. Different semiquantitative sy…

A 28-year-old man was referred to our hospital because of localised swelling of his fingers, with suspicion of arthritis of the proximal interphalangeal (PIP) joints. At physical examination we observed a swelling, superficial to the PIP joints. Ultrasonography revealed a hypo-echoic structure above the extensor tendon, typical for knuckle pads.

In congenital absence of fingers or in post-traumatic amputations of the thumb or fingers, reconstruction by microsurgical toe-to-hand transfer is becoming a common treatment for these difficult problems. In case of congenital absence of the thumb, fingers, or both, these transfers can provide acceptable growth, function and sensation of the transferred toes with a current success rate usually greater than 95%. The most commonly transplanted toe is the second toe, since it is the longest toe and combines all the useful characteristics of a finger (such as joint, a nail, sensitive pulp tissue,

Stabilization of boneless finger stumps or lengthening of partially aplastic digits in congenital malformations is difficult, because the bone graft, placed on top of the existing bone, will undergo resorption. Only the periosteum-covered proximal toe phalanx will withstand resorption and show a complete take. With the toe phalanx, including the plantar plate and the collateral ligaments of the metatarsophalangeal joint as a half-joint, it is possible to construct a new joint with the cartilage-covered distal end of the recipient bone (metacarpal or proximal phalanx) as the proximal half-joint

Thumb hypoplasia represents a spectrum of deficiencies from a slightly smaller thumb to a completely absent thumb. The clinical features of a type II hypoplastic thumb include a narrow thumb-index web space, absent thenar muscles, and instability of the thumb metacarpophalangeal joint. This article will focus thumb reconstruction using the ring finger flexor digitorum superficialis to provide opposition and restore metacarpophalangeal joint stability. The technique, postoperative protocol, outcome, and complications will be emphasized.

The proximal interphalangeal joint is a central structure in the function and biomechanics of the hand and finger flexion. It has unique anatomical characteristics that influence its normal function and is affected by multiple pathologies that can markedly alter its architecture and function. This narrative literature review correlates basic sciences, clinical practice, and surgical management, focusing on two treatment options that sacrifice the joint: arthroplasty and arthrodesis. The aim is to identify evidence to support decision-making regarding which patients are best suited for each typ

Although knuckle pads are histologically similar to the palmar nodules in Dupuytren's Disease, they do not produce contraction. By considering the anatomical situation of the knuckle pad overlying the joint line and comparing it with a unique case of a dorsal nodule occurring between the joint lines, an explanation is profferred for this lack of contraction by knuckle pads. That contraction in the extensor mechanism was produced by the nodule between the joint lines is used to support further the "extrinsic" hypothesis of the pathogenesis of Dupuytren's Disease.

We report a technique of reducing and stabilizing osteochondral fractures of the metacarpals and phalanges, using the principle that the space within a joint is only "potential." Accurate reduction of the fragment and accurate approximation of the capsular rent effectively traps and holds the fragment without the need for internal fixation. Preservation of all soft tissue attachment to the fragments and early supervised mobilization ensure healing of both the bony and hyaline cartilage components.
