Unilateral thalamic hypometabolism in patients with temporal lobe epilepsy.

BACKGROUND/PURPOSE: Interictal brain 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) is useful for localizing the focus of a seizure, and thalamic hypometabolism has been observed in temporal lobe epilepsy (TLE). In this study, we aimed to investigate the prevalence of thalamic hypometabolism and to assess the usefulness of this supplementary finding for lateralizing epileptic foci on FDGPET scans in patients with TLE. METHODS: This was a retrospective study of 47 patients who underwent anterior temporal lobectomy to treat medically intractable TLE. Other inclusion criteria we

Rhabdomyosarcoma with widespread bone marrow infiltration: beneficial management role of F-18 FDG PET.

Alveolar rhabdomyosarcoma (RMS) accounts for 20% to 30% of childhood RMS and is associated with a prognosis worse than embryonal RMS. Disseminated RMS can present with extensive bone marrow involvement. Assessing the extent of the tumor is critical, because therapy and prognosis depend on the degree to which the mass has spread beyond the primary site. The value of F-18 FDG PET in patients with RMS has been reported in some series but none specifically involving bone marrow. Children have a highly cellular hematopoietic bone marrow and differentiation of a highly cellular marrow from neoplasti

Bilateral breasts involvement in Burkitt's lymphoma detected only by FDG-PET.

Burkitt's lymphoma can occur from elsewhere in the body, but involvement of the breasts is very rare. A few articles reported bilateral, diffusely infiltrative mammographic, and ultrasonographic features. However, we experienced the involvement of bilateral whole breasts in a patient with underlying Burkitt's lymphoma, which showed negative mammographic and ultrasonographic findings, but bilateral hot uptakes of the breast in FDG-PET scan.

Port site recurrence diagnosed by positron emission tomography after laparoscopic surgery for colon cancer.

Port site recurrence after laparoscopic surgery for colorectal cancer patients is a rare complication. We report a case of port site recurrence in a 46-year-old woman that was detected by positron emission tomography (PET) using 18[F]-fluoro-deoxyglucose (FDG). Surgical treatment consisted of laparoscopic ileo-cecal resection and lymph node dissection. At her nine months postoperative follow-up examination, her serum carcinoembryonic antigen (CEA) levels had raised to 15.8ng/mL. Although computed tomography and colonoscopic examination were performed, lung, liver and local recurrence in the co

Diaphragmatic and crural FDG uptake in hyperventilating patients: a rare pattern important to recognize.

The authors describe various patterns of F-18 fluorodeoxyglucose (FDG) accumulation in the diaphragm and crura. They present 6 patients in whom incidental diaphragmatic and crural uptake of FDG was observed during positron emission tomography (PET). Hyperventilation is thought to be the potential underlying mechanism of this condition.

PET scans in the staging of lymphoma: current status.

Positron emission tomography (PET) is a novel functional imaging technique that provides several inherent advantages over conventional nuclear scintigraphy. Several studies have suggested a role for PET using the positron emitter fluorine-18 in the diagnosis and follow-up of patients with lymphoma. This review summarizes the existing data evaluating the role of 2-fluoro-2-deoxy-D-glucose (FDG)-PET in both the staging and follow-up of patients with lymphoma. Most studies of PET involve patients with either Hodgkin's disease or diffuse large B-cell non-Hodgkin's lymphoma. PET detects more diseas

Excretion of FDG into saliva and its significance for PET imaging.

AIM: Assessment of the salivary excretion of FDG and its significance during PET imaging. METHODS: Salivary samples from 16 patients were obtained during the first three hours after FDG injection and their activity concentrations were measured. Furthermore, regional FDG accumulations in whole body PET scans (60 min p.i., ROI technique) were determined in the following locations: vestibulum oris, floor of the mouth, hypopharynx, parotid gland, submandibular gland. This ROI analysis was repeated after drinking water (0.2 l) and a second scan (120 min p.i.). RESULTS: The salivary FDG concentrat

FDG positron emission tomography in patients with systemic mastocytosis.

OBJECTIVE: Systemic mastocytosis is a hematologic neoplasm characterized by abnormal accumulation and growth of mast cells in one or more organ systems. We analyzed five patients with systemic mastocytosis referred for FDG positron emission tomography who had biopsy-proven mast cell infiltrates in various organs. CONCLUSION: Our findings indicate that FDG positron emission tomography is not useful for staging and follow-up of aggressive systemic mastocytosis.

Limbic encephalitis investigated by 18FDG-PET and 3D MRI.

Two patients with clinically probable or possible limbic encephalitis (LE) are reported, both cases with typical findings in clinical symptoms (severe neuropsychological deficits and complex partial seizures) and in routine magnetic resonance imaging (MRI) (hyperintense mesiotemporal lesions). Underlying malignancy was identified (rectal carcinoma) in one case but could not be detected in the other patient. The 2 patients were investigated by cerebral 18F-fluoro-2-deoxy-D-glucose-positron emission tomography (FDG-PET) and 3-dimensional (3D) MRI, and abnormalities in metabolic activity were map

[Post-radiation pneumonitis in a case of Hodgkin's lymphoma assessed with PET-FDG by residual mediastinal mass].

The PET-FDG is a useful method to assess the post-radiotherapy residual masses in supradiaphragmatic lymphoma patients. We present the case of a patient with residual mediastinic mass revealed by CT after the patient had completed treatment for Hodgkin's disease and in whom the PET study demonstrated a typical pattern of post-radiation pneumonitis. When these patients are re-assessed early after radiotherapy, the different tracer uptake patterns should be taken into account in order to identify the existence of radiotherapy sequela and avoid false positive results.

False-positive 2-[F-18]-fluoro-2-deoxy-D-glucose positron emission tomography studies for evaluation of focal pulmonary abnormalities.

Positron emission tomography (PET) with 2-[F-18]-fluoro-2-deoxy-D-glucose (FDG) can demonstrate the glucose metabolism characteristics of a lesion, which may be helpful in differentiating between benign and malignant focal pulmonary lesions. Malignant cells demonstrate higher glucose metabolic activity than benign lesions. However, some inflammatory processes also show significant FDG uptake. We present two cases where high FDG uptake was found in inflammatory lesions in the lungs. The first case was that of a 38-year-old woman with chronic cough for more than 20 years. FDG PET revealed a hype

Neuropharmacological imaging in epilepsy with PET and SPECT.

Functional neuroimaging with positron and single photon emitter-labeling has added considerably to the understanding of epileptic seizure activity and of the postictal and interictal cerebral dysfunctions that accompany many epilepsies. Some of these functional alterations cannot be studied in humans by any other technique, and in other instances the information is complementary to that provided by other techniques, some of which are invasive or even require tissue destruction. Available radiotracer imaging techniques have yet to be fully applied to several important epileptic syndromes (inclu

Effects of percent thresholding on the extraction of [18F]fluorodeoxyglucose positron emission tomographic region-of-interest data.

Although we and others have employed a thresholding strategy to extract "peak" values from positron emission tomographic (PET) regions of interest (ROIs), the effects of peak picking on fitted fluorodeoxyglucose rate constants, regional metabolic rate for glucose (rCMRglc) profiles, patterns of regional metabolic covariation, and PET-neurobehavioral correlations have not been systematically investigated. Our results suggest that under some commonly encountered imaging conditions percent thresholding may increase sensitivity to regional activation; however, the effect of thresholding is determi

The role of 18F-FDG PET/CT in the imaging of patients with breast cancer.

Diagnosis of the Primary Tumor In initial diagnosis, 18F-FDG PET/CT has questionable clinical value and, for this reason, is not used in routine practice to evaluate potential primary breast tumors. It has low sensitivity but high positive predictive value (PPV) (PPV 81%-100%). So, if increased uptake is found on 18F-FDG PET/CT in a breast lesion as an incidental finding, there is a high probability that it is due to malignancy. The sensitivity of 18F-FDG PET/CT depends on tumor size, histological type (invasive ductal carcinoma has a higher SUVmax than lobular carcinoma, and hormone receptor

Role of PET in Radiation Oncology: A Theranostic Perspective on Detection of Radiation-Induced Complications.

Radiation therapy remains essential in managing thoracic and head and neck malignancies, yet radiation-induced injury to surrounding normal tissues continues to pose significant clinical challenges. Traditional assessment of radiation complications relies on clinical symptoms and anatomic imaging, often detecting injury only after irreversible damage has occurred. This article reviews the emerging role of fluorodeoxyglucose PET/computed tomography (FDG-PET/CT) as a theranostic tool for early detection and quantitative assessment of radiation-induced complications across multiple organ systems.
