Young-onset hepatocellular carcinoma following double outlet right ventricle surgery without liver congestion: a case report with genetic analysis.

AIM: Congenital heart disease (CHD) can lead to liver dysfunction and HCC, typically through congestion, hypoxia, or low cardiac output. However, HCC development without these factors is not well understood. METHODS: We report a 16-year-old girl with a history of double outlet right ventricle surgery in infancy, without Fontan procedure. Imaging and histology confirmed HCC without liver fibrosis, inflammation, or congestion. Whole exome sequencing was performed on tumor and non-tumor liver tissue. Genes associated with both CHD and HCC were identified from public databases, and protein expres…

Laparoscopic partial liver resection for hepatocellular carcinoma arising from Fontan-associated liver disease: a case report.

BACKGROUND: The Fontan procedure (FP) is a palliative surgery for functional single ventricle. The Fontan circulation maintains pulmonary circulation by a high central venous pressure, leading to chronic congestive liver. The number of patients diagnosed with hepatocellular carcinoma (HCC) arising from liver fibrosis and cirrhosis after FP is increasing. Several reports have described surgical treatment for HCC after FP, but few have described laparoscopic surgery. CASE PRESENTATION: The patient was a 31-year-old man who had undergone the FP for single right ventricle at 3 years. Several live…

[Total cavopulmonary anastomosis: risk factors and results in patients under 4 years of age].

In recent years, an increasing number of modified Fontan-operations has been performed in children younger than 4 years of age. The purpose of this study was to identify preoperative risk factors in this age group. From February 1990 until February 1993, we performed in our center a modified Fontan-operation using the technique of total cavopulmonary anastomosis (TCPA) in 37 consecutive patients (17 pts. < 4 years = group I, 20 pts. > 4 years = group II). Early postoperative mortality occurred in patients of group I only (n = 3 pts.). All of these patients had additional preoperative risk fact

The Role of Trans-Oesophageal Echocardiography in the Interventional Cardiology of Adult Congenital Heart Diseases.

Advances in interventional cardiology have significantly broadened the range of congenital heart diseases (CHDs) amenable to trans-catheter interventions. Trans-oesophageal echocardiography (TOE) plays a pivotal role as a procedural guide in several percutaneous treatments. Enhanced imaging modalities and technological innovations in echocardiography have refined the precision and applicability of these approaches. This review explores the role, impact, and advancements of TOE in trans-catheter treatments for adult CHDs, including both common procedures (e.g., atrial septal defect closure, ven

First report of successfully palliating a hypoplastic left heart syndrome patient with anomalous left coronary artery from the pulmonary artery beyond Fontan.

We report a case of hypoplastic left heart syndrome with an anomalous left coronary artery from the pulmonary artery (ALCAPA) identified intraoperatively during the Stage-II palliation. Due to recurring ventricular fibrillation on sternotomy, a hybrid Stage-I palliation was performed. During comprehensive Stage-II, the ALCAPA was reimplanted in the neoaorta and measures, including a nontraditional Damus connection/arch reconstruction and classic bilateral Glenn procedures, were taken to avoid compression of the coronary artery. After a successful Fontan procedure, he continues to do well at 5 …

Valve-Sparing Atrioventricular Valve Replacement in a Single-Ventricle Infant: Preserving the Entire Leaflet-Chordal Complex.

We report a 4-month-old, 3.7-kg girl with asplenia, single ventricle physiology, a common atrioventricular valve, and severe regurgitation after repair, along with moderately depressed ventricular function. She underwent supra-annular valve replacement with a 16-mm mechanical prosthesis and 18-mm expanded polytetrafluoroethylene graft, preserving the entire valve leaflets and subvalvular apparatus. Postoperative imaging showed no inflow/outflow obstruction and preserved ventricular function. She subsequently underwent a bidirectional Glenn shunt and is currently awaiting the Fontan procedure. …

A milestone in congenital cardiac surgery: Four decades of the Norwood procedure.

Hypoplastic left heart syndrome (HLHS) was first described by Lev in 1952, but it was not until 1958 that it received a name from Noonan and Nadas. For the next several decades, the defect was considered untreatable. In 1979, William Norwood and his colleagues from Boston initiated a program to evaluate staged surgical management for infants with HLHS. The Norwood operation has became a milestone in the effective palliation for neonates born with HLHS. Today, the Norwood procedure is the first step of a three-stage heart surgery aimed at creating a new circulatory pathway (i.e., the Fontan pat

Inferior Vena Cava Thrombus in a Postpartum Patient With Fontan Physiology: A Case Report.

Women with complex congenital heart disease, such as those with single-ventricle physiology, are surviving into adulthood and becoming pregnant. Because of their complex physiology, common peripartum complications pose unique risks. We describe a patient with a single ventricle who underwent an external vascular conduit, nonfenestrated Fontan procedure in childhood and then presented during the postpartum period with extensive thrombosis in her lower extremity deep venous system and inferior vena cava. In this article, we will discuss single-ventricle physiology and the implications of pregnan…

Esophageal perforation associated with fontan operation: a complication of transesophageal echocardiography.

A 10-year-old girl with unbalanced atrioventricular septal defect and aortic arch interruption with interrupted inferior vena cava and dextrocardia had undergone a modified Norwood procedure followed by a Kawashima operation in 2002 and 2005, respectively. In 2012, she underwent Fontan operation with diversion of hepatic veins to left pulmonary artery. Intraoperative transesophageal echocardiography (TEE) was used. She was noted to have an esophageal perforation, diagnosed on the 10th postoperative day. She was managed with thoracic drainage, cervical esophagostomy, and feeding jejunostomy. Es

Right ventricle-pulmonary artery shunt in first-stage palliation of hypoplastic left heart syndrome.

This article reviews our experience using a prosthetic conduit between the right ventricle (RV) and the pulmonary artery (PA), in lieu of the more traditional aortopulmonary shunt, for infants undergoing surgical palliation of hypoplastic left heart syndrome. Thirty-three consecutive infants underwent Norwood procedure between February 1998 and November 2003, using an RV-PA conduit. There were 31 hospital survivors (94%) and 25 patients have undergone a stage II bidirectional Glenn anastomosis with an additional two deaths. Nine patients have undergone completion Fontan. Surgical management of atrial tachyarrhythmias associated with congenital cardiac anomalies: Mayo Clinic experience.

Patients with congenital cardiac anomalies that cause right or left atrial dilatation may have associated atrial tachyarrhythmias, including atrial fibrillation and atrial flutter. Congenital cardiac anomalies may also be associated with Wolff-Parkinson-White syndrome or atrioventricular nodal re-entrant tachycardia. In addition, atrial arrhythmias may develop late after definitive operation for congenital cardiac anomalies, especially after the Fontan procedure. Ebstein's anomaly is the most common congenital cardiac anomaly associated with atrial arrhythmias. Atrial arrhythmias cause signifi…

Percutaneous balloon angioplasty for early postoperative modified Blalock-Taussig shunt failure.

Percutaneous balloon angioplasty was successful initial treatment for 2 infants who, early after operation, developed obstruction at the proximal anastomotic site of a modified Blalock-Taussig shunt. Two years later the first child had not required reoperation and the dilatation site was patent angiographically. The other baby progressed well after angioplasty but because of surgical concern about the long-term success of angioplasty, shunt surgery was repeated, the baby dying after reoperation. Angioplasty of proximal obstruction in these shunts is feasible and satisfactory long-term palliati

Extracardiac total cavopulmonary connection using a Y-shaped graft.

We report on 17-year-old Fontan candidate with a severely distorted central pulmonary artery (PA) who underwent a successful extracardiac total cavopulmonary connection using a Y-shaped bifurcated graft. A nonanatomic pathway from the inferior vena cava to the left PA was constructed and positioned anterior to the ascending aorta. The other arm was used as a conduit between the inferior vena cava and the right PA. All procedures were performed under temporary venous bypass without cardiopulmonary bypass.

Is surgical ligation of an accessory left superior vena cava always safe?

In patients considered for bidirectional Glenn or Fontan procedures, the association of left superior vena cava (LSVC) with ostial atresia of the coronary sinus should be diagnosed preoperatively in order to avoid surgical division or ligation of the LSVC and the negative effect of resulting coronary venous hypertension on myocardial perfusion. This report discusses the angiographic and hemodynamic features of LSVC when it is the only drainage route from a blind coronary sinus. A retrograde flow in the LSVC seen by Doppler ultrasonography should raise the suspicion of this diagnosis.

Outcomes of ultra-high-pressure balloon angioplasty for congenital heart disease in single-center experience.

Angioplasty using ultra-high-pressure (UHP) balloons may successfully treat stenotic lesions refractory to high-pressure dilation. The use of UHP balloons in patients with congenital heart disease is mostly for dilation of the pulmonary artery, and there have been few reports on the effectiveness and safety of balloons for other sites. We retrospectively evaluated the efficacy and safety of the ultra-high-pressure balloon angioplasty (UHP-BA) for stenotic lesions in patients with congenital heart disease between January 2020 and December 2022 at Okayama University Hospital. A total of 78 UHP-B

The mid-term outcome of Fontan conversion compared with primary total cavopulmonary connection.

BACKGROUND: The indication of Fontan conversion (FC) from atriopulmonary connection (APC) to total cavopulmonary connection (TCPC) is unclear. We sought to analyze the mid-term outcome of prophylactic and therapeutic Fontan conversion compared with that of primary TCPC. METHODS: Patients with a univentricular heart who underwent cardiac catheterization at >18 years of age between July 2005 and July 2019 were included and divided into three groups: symptomatic APC patients who underwent therapeutic FC (t-FC, n = 13), asymptomatic APC patients after prophylactic FC (p-FC, n = 15), and patients

Long-term results and mid-term features of left ventricular reconstruction procedures on left ventricular volume, geometry, function and mitral regurgitation.

OBJECTIVES: We retrospectively analysed the long-term results and mid-term features of the surgical procedures on left ventricular (LV) volume, geometry, function and mitral regurgitation (MR) in patients with ischaemic cardiomyopathy (antero-septal dominant) who underwent LV reconstruction (LVR). METHODS: The LVR was mainly accomplished by the Dor procedure, modified by the Fontan stitch maintained by the base of the LV septum prior to the flattened-out elliptical patch closure. Other techniques such as the linear closure or septal anterior ventricular exclusion (SAVE) were used when indicat
