Foot and Ankle Conditions: Pes Planus.

Pes planus or pes planovalgus (ie, flatfoot) is a common condition among young children and also is encountered in adults. In children, congenital pes planus typically resolves with age as the foot musculature strengthens. Flexible pes planus is defined as a normal arch during non-weight-bearing activity or tiptoeing, with a flattening arch on standing. In rigid pes planus, the arch remains stiff and collapsed with or without weight bearing. Patients with rigid pes planus should be referred for subspecialist treatment. Patients with flexible pes planus, in the absence of signs of rheumatologic…

Foot drop post varicella zoster virus.

Leg pain and weakness can often be seen in a musculoskeletal physiotherapy outpatient setting. While the differential diagnoses for neuropathic pain and weakness is often spine related, this unusual cause for polyradiculopathy is less commonly seen. Shingles is not something routinely seen in a physiotherapy department but is usually associated with skin eruptions and pain but less so with motor loss. The purpose of this case report is to therefore raise awareness that this presentation can occur and should not be mistaken for spinal radiculopathy.

Thyrotoxicosis Presenting as Unilateral Drop Foot.

Neuromuscular disorders associated with hyperthyroidism have several variations in their clinical phenotype, such as ophthalmopathy, periodic paralysis, and thyrotoxic myopathy. We herein report an unusual case of thyrotoxic myopathy presenting as unilateral drop foot. Histopathological examinations of the left tibialis anterior muscle showed marked variation in the fiber size, mild inflammatory cell infiltration, and necrotic and regenerated muscle fibers with predominantly type 1 fiber atrophy. Medical treatment with propylthiouracil resulted in complete improvement of the left drop foot. Th…

Mid-foot retinaculum: an unrecognized entity.

Retinacula are thickenings of deep fascia in the region of joints that hold down the tendons preventing them from bowing out of position. In the region of ankle, number of such retinacula have been described. Retinacula like superior and inferior extensor retinacula have been described which hold down the tendons of leg muscles passing to the foot beneath them. As the extensor tendons of the leg have more distal attachment to the toes, the present study was conducted to ascertain the presence of any additional retinaculum in the mid-foot region, which would tie down the tendons for their effec…

Percutaneous Pediatric Foot and Ankle Surgery.

The techniques for performing percutaneous osteotomies in treating deformities of the foot in children are presented along with a detailed description of the operative details. The author's use of minimal-access surgery for tibial, os calcis, and midfoot osteotomies is described using a cooled side-cutting burr that has not previously been described for use in the child's foot. The cancellous nature of the bones in the child are easily cut with the burr and the adjacent soft tissues are not damaged. The early experience of the healing times are not impaired and the complications associated wit…

Rare presentation of foot postaxial polydactyly.

Polydactyly is a prevalent birth anomaly observed in the foot, and a number of classification systems have been suggested for this condition. Postaxial (fifth or little toe) polydactyly is the most common type. We encountered an exceedingly rare presentation of foot postaxial polydactyly that, to our inspection, had neither been previously classified nor described in published studies. In the present report, we have described an otherwise healthy 2-year-old female who had presented to our clinic with an isolated, extra little toe on her left foot. Foot radiographs revealed the presence of all …

Risk factors for diabetic foot ulcer.

BACKGROUND: Diabetic foot is one of the common complications of diabetes mellitus. Many risk factors are involved in its causation. This study was conducted to determine risk factors responsible for foot ulcer in diabetic patients. METHODS: A total of 196 consecutive patients with diabetic foot were included in the study. Detailed history, clinical findings and investigations were recorded. Lesions were graded according to Wagner's classification, and appropriate medical and/or surgical treatment was carried out. Patients who did not consent to participate in the study had established gangren…

Ethanol embolotherapy of foot arteriovenous malformations.

OBJECTIVE: To review initial clinical presentation, treatment course, and results of ethanol embolotherapy for foot arteriovenous malformations (AVMs) and to analyze prognostic factors for treatment results. METHODS: Twenty-nine patients (mean age, 31.3 years; range, 9-59) who underwent ethanol embolotherapy between 1999 and 2012 were retrospectively reviewed. Initial clinical manifestation, medical history, treatment results of ethanol embolotherapy, and major and minor complications were reviewed in detail. Two radiologists analyzed the extent of AVM involvement, modified angiographic class…

Woringer-Kolopp disease mimicking foot dermatitis.

Woringer-Kolopp disease, also known as localized pagetoid reticulosis, is a rare cutaneous lymphoproliferative disorder classified as a solitary variant of mycosis fungoides (MF). Despite the indolent and benign nature of the disease, misdiagnosis and inappropriate treatment may result in years of debilitating symptoms and even loss of function. We present the case of a patient with long-standing Woringer-Kolopp disease that mimicked foot dermatitis. Histopathologic examination demonstrated epidermotropic infiltration of atypical lymphocytes that were CD3+ CD4- CD8-. The patient was successful…

Acquired flat foot deformity: postoperative imaging.

Flat foot (pes planus) is a progressive and disabling pathology that is treated initially with conservative measures and often followed by a variety of surgeries. This article briefly reviews the pathology in acquired flat foot deformity, the classification of posterior tibial tendon dysfunction, discusses surgical techniques for the management of adult flat foot deformity, and reviews potential complications and their relevant imaging appearances.

[Tendon transfer in pediatric foot deformities].

Tendon transfer surgery plays a central role in the treatment of pediatric foot deformities. Tendon transfers are essential for balancing in neurogenic deformities but also important for the treatment of congenital or idiopathic deformities. Furthermore tendon transfer represents an important management tool for revision surgery of failed treatment in foot deformities. Complications and pit-falls which can potentially lead to failure may occur during the indications and planning of the transfer as well as during the surgery. The identification and analysis of possible pitfalls and complication…
