Intrathoracic fracture-dislocation of the proximal humerus treated with rib fixation and shoulder arthroplasty.

Proximal fractures of the humerus resulting in intrathoracic displacement of the humeral head are rare, but cause serious pain and can lead to pulmonary and glenohumeral complications. Many treatment options exist, but there is still no literature regarding mortality and morbidity associated with rib fixation in concordance with treatment of the proximal humerus. This case describes a patient who was treated using a multi-team approach that involved shoulder arthroplasty with rib fixation, and which resulted in decreased pain and decreased narcotic use while improving the patient's pulmonary f

Complete medial dislocation of the tarsal navicular without fracture: report of a rare injury.

Complete dislocation of the tarsal navicular, without fracture of the navicular, is an uncommon injury. A review of the published data revealed only 15 previous reported cases. The rarity of this injury can be attributed to the rigid bony and ligamentous support surrounding the navicular, which usually undergoes fracture and dislocation rather than pure dislocation of the navicular. The mechanism and appropriate treatment of this injury remains unclear. In the present report, we describe the case of a 29-year-old male who sustained complete dislocation of the tarsal navicular, without fracture

Posterior iliac crescent fracture-dislocation: what morphological variations are amenable to iliosacral screw fixation?

OBJECTIVES: Posterior iliac crescent fracture with associated sacroiliac joint disruption (PICFSID) is a type of traumatic posterior pelvic injury associated with instability. Posterior iliac fracture location and obliquity along with other details determine the treatment. Open reduction and internal fixation has been previously recommended for the majority of crescent fractures. Our objectives were to determine whether all crescents can be categorised according to the previously described Day crescent classification scheme, and to see which crescent types are amenable to percutaneous fixation

[Arthroscopic therapy of ankle joint impingement syndrome after operation of ankle joint fracture dislocation].

OBJECTIVE: To study the operative procedure and the effectiveness of arthroscopic therapy for ankle joint impingement syndrome after operation of ankle joint fracture dislocation. METHODS: Between March 2008 and April 2010, 38 patients with ankle joint impingement syndrome after operation of ankle joint fracture dislocation were treated. Among them, there were 28 males and 10 females with an average age of 28 years (range, 18 to 42 years). The time from internal fixation to admission was 12-16 months (mean, 13.8 months). There were pressing pain in anterolateral and anterior ankle. The dorsal…

[Forensic identification of cervical spinal cord injury without fracture or dislocation in 25 cases].

OBJECTIVE: To explore forensic identification of causality between injury and the consequence in cases of cervical spinal cord injury without fracture or dislocation. METHODS: Twenty-five cases of cervical spinal cord injury without fracture or dislocation were collected. The age, mode of injury, imaging reports, cervical vertebral degeneration, clinical manifestations, diagnosis, treatment and injury consequences were summarized. The causality between injury and its consequence were classified as direct cause, main cause, same cause, minor cause and no cause. RESULTS: Age, mode of injury, c

[Fracture dislocation of the thoracolumbar spine in pregnant patient: diagnostic by image and treatment].

BACKGROUND: Fracture dislocations of the thoracolumbar spine are, to a great extent, associated with neurologic deficit and instability, so a large percentage of them require surgical treatment. Being pregnant does not rule out the possibility of having this type of lesions. There are only a few bibliographic references concerning these cases and medical and treatment ignorance about them is frequent. OBJECTIVE: Present a review of concepts and specifications for the correct and safe imaging diagnosis as well as considerations on the surgical treatment of patients with traumatic thoracolumbar…

Avulsion fracture of the medial malleolus following posterior tibialis tendon dislocation: a case report.

Dislocation of the posterior tibialis tendon as an isolated injury is rare. The diagnosis is often delayed due to its rarity and the need for various second line imaging modalities. We present a case of a dislocated posterior tibialis tendon that resulted in an avulsion type fracture of the medial malleolus. The fracture was openly reduced and internally fixed with inter-fragmentary screws. The patient was asymptomatic at 1 year follow-up.

Posterior dislocation of the hip associated with ipsilateral trochanteric fracture--a very rare case.

Hip dislocations are a frequent consequence of traffic accidents. Commonly, the knee hits the car board and the force of the impact leads to either a fracture of the femur or a dislocation of the hip. The dislocation occurs if the femur does not break and the force of the impact is transmitted towards the hip joint leading to a posterior dislocation of the femoral head. We present the case of a 41 year old patient involved in a traffic accident that suffered a knee injury. Clinical and imagistic (radiography, CT) examination revealed a trochanteric fracture associated with a posterior hip disl

Anterior dislocation of the tibio-talar joint without diastasis or fracture--a case report.

We present a 27-year-old man with pure anterior dislocation of the ankle joint following a fall from height. There were no accompanying fractures in and around the ankle joint. The injury was initially missed on clinical examination in view of minimal ankle deformity. The patient was managed conservatively with closed reduction and plaster immobilisation. The case report highlights the importance of early diagnosis and management of anterior ankle dislocation to obtain good functional outcome. A review of the literature on this rare injury is also presented.

Transverse divergent dislocation of the elbow with ipsilateral distal radius fracture in a child.

We report a rare case of an irreducible transverse divergent dislocation of the elbow with an ipsilateral distal radius torus fracture and a fracture of the coronoid process in a 9-year-old male. Closed reduction of the elbow was attempted, but the humeroulnar joint remained dislocated. At surgery, the avulsed anterior band of the medial collateral ligament complex of the elbow was found to be interposed between the medial condyle of the humerus and the olecranon. The dislocation was reduced after relieving the ligament entrapment, which was then repaired. The unstable proximal radioulnar join

Posterior fracture-dislocation of the humeral head treated without the use of metallic implants.

Posterior fracture-dislocation of the humeral head is an uncommon injury, usually associated with seizures, electrocution injury and high-impact trauma. Prompt diagnosis and treatment are important to prevent avascular necrosis of the humeral head. Various methods to treat such injury have been described. We report a 34-year-old man who had a four-part humeral head fracture with concomitant posterior dislocation that was treated with open reduction and internal fixation with non-absorbable polyester sutures. The diagnosis and operative techniques are described and factors influencing our choic

Open anterior fracture dislocation of the hip: a case report and review of literature.

Open anterior fracture dislocation of the hip is an extremely rare injury and is the result of a violent trauma. Such a case is being reported in a 15-year-old female, who also had other adjacent bony injuries. In spite of early treatment, necrosis and infection could not be controlled, resulting in permanent disability in this young girl.

Cervical spine fracture-dislocation birth injury: prevention, recognition, and implications for the orthopaedic surgeon.

Spinal cord birth injury is rare, and orthopaedic surgeons infrequently encounter it. The injury is associated with hyperextension of the fetal head in utero and during delivery and with forceps-assisted breech vaginal deliveries. These cervical spinal cord injures may be complete or partial. They most commonly occur in the absence of bony injury, which can lead to diagnostic difficulties. Ultrasound and MRI studies are valuable diagnostic tools for identifying spinal cord injury in cases of hypotonic newborns with difficult deliveries. The authors report an unusual case of spinal cord birth i
