[Osteosynthesis in cranial and midfacial skull fractures].

From 1982 to 1985 65 patients with a fracture of the cranial and middle portion of the visceral skull underwent surgical reconstruction using mini-plate osteosynthesis (Champy set). In this group we can distinguish four typical fractures where plate osteosynthesis with functional stability has proved successful (e.g.: piece fracture of the inferior margin of the orbita, bursting fracture of the zygomatic bone, impression fracture of the anterior wall of the frontal sinus, comminuted fracture of the interorbital region). The technical details of surgery are given special emphasis and possible c

[Management of subtrochanteric femoral fractures with the dynamic condylar screw].

The 95-degree angled condylar screw (DCS) has been proven to be a valuable implant in the operative repair of comminuted and two-level fractures of the subtrochanteric region. Technically, it is easier to insert than the 95-degree angled condylar plate providing higher stability and the possibility of corrections even after insertion.

Diffuse Idiopathic Skeletal Hyperostosis with Severely Displaced Spine Fracture Managed with Posterior Approach Alone: Case Reports and Literature Review.

Diffuse idiopathic skeletal hyperostosis (DISH) is a condition in which minor trauma can cause extremely unstable vertebral fractures. Spinal fractures associated with DISH are prone to instability due to the large moment of lever arm and secondary neurological deterioration; hence, surgical internal fixation is considered necessary. On the other hand, some reports suggest that patients with DISH have a high osteogenic potential. In this report, we describe three patients with DISH. These patients had spinal injuries that resulted in a large gap, for which anterior fixation with bone graft wou

Oribital floor fractures: a retrospective study.

A retrospective series of 365 orbital floor fractures was studied and many variables were analyzed. Some type of ocular or orbital adnexal damage was present in 32% of the patients. After a repair procedure there was a low incidence of decreased visual acuity, and there were no implant extrusions. Late enophthalmost and diplopia were found to be related to the type of orbital floor damage. The comminuted floor fractures and fractures associated with prolapse of orbital tissue into the maxillary antrum were more prone to demonstrate these late sequelae. Despite various surgical procedures there

Intraoperative hip fractures: ruining your day.

The results of treatment of 116 intraoperative femoral fractures were reviewed. Excluding calcar and greater trochanter fractures, there were 14 Type I cases (Vancouver classification). All healed with cerclage wiring. Three cases of vertical fractures of the proximal segment following a subtrochanteric osteotomy did not heal and required revision. There were 8 Type II fractures, most being minor splits or limited, nondisplaced, vertical fractures. Control was lost in 2 wired cases, suggesting that Type II fractures should be converted to Type I by the use of a long stem, or, if impractical, b

Capitellar erosion after radial head arthroplasty: A comparative biomechanical study of operated radial head fractures on cadaveric specimens.

BACKGROUND: We asked whether open reduction and internal fixation (ORIF) or radial head arthroplasty (RHA), common techniques used for the confrontation of displaced or comminuted radial head fractures, are correlated with cartilage wear of the capitulum. HYPOTHESIS: We hypothesized that neither ORIF nor RHA are correlated with capitellar cartilage wear. MATERIAL AND METHODS: On 5 cadaveric elbow specimens, osteotomies were employed to simulate radial head comminuted fractures followed with ORIF by Herbert screws. Radial heads were also excised from other 5 cadaveric elbow specimens and were

Predicting the mechanical outcome of femoral neck fractures fixed with cancellous screws: an in vivo study.

OBJECTIVE: The goal of this project was to determine, by using a retrospective cohort analysis, the relative importance of fracture geometry, bone density, and quality of fracture reduction in predicting redisplacement of femoral neck fractures fixed with multiple cancellous screws in a patient population. DESIGN: A retrospective analysis of quantitative geometric and bone density data from forty-seven patients with femoral neck fractures fixed with cancellous screws was performed. SETTING: Radiographs retrieved after analysis of a statewide hospital database were digitized and analyzed quan

Closed Limb Fractures With Compromised Vascularization: A Narrative Review.

Vascular compromised fractures typically result in a high rate of healing complications, such as avascular necrosis, nonunion, delayed union, and arthritis, which severely affect a patient's function and quality of life. The purpose of this review was to identify and describe the epidemiology and available treatment options for the most well-known vascular compromised closed fractures. The injuries discussed in detail in this review were scaphoid, lunate, femoral neck, and talar fractures. Current evidence suggests that optimal treatment for vascular compromised fractures is dependent on the d

Reappraisal of the surgical strategy in treatment of mandibular condylar fractures.

BACKGROUND: Over the past few decades, there has been a trend toward open reduction and internal fixation of mandibular condylar fractures. However, not all patients have fared well following the open surgical approach. A number of clinical parameters have been observed to be related to suboptimal treatment results on a long-term basis. A comprehensive investigation of the role of those risk factors is a worthwhile pursuit. METHODS: A total of 23 surgically treated patients were enrolled in the study during an 11-year period. A retrospective chart review was conducted to collect clinical and

Bilateral floating knees in a multiple injured patient. A case report.

Bilateral floating knees are rarely seen in orthopedic practice. Ipsilateral fractures of the femur and tibia are becoming more common due to an increase of high-velocity accidents. The following case presents a 23-year-old male motorcyclist who sustained multiple musculoskeletal injuries including a burst fracture of the elbow and fractures of both femurs and both tibiae. Early operative stabilization of all the fractures was performed. A comminuted fracture of the ulna was fixed with a Dynamic Compression Plate (DCP) and the fractures of both femurs and both tibiae were fixed with Enders nai

Using a TightRope® to treat a complex fracture of the trapezium.

We present the case of a 23-year-old man with a combined scaphoid fracture and comminuted trapezium fracture, treated surgically with percutaneous fixation of the scaphoid fracture and concomitant Arthrex Mini TightRope(®) stabilisation of base of thumb metacarpal to base of index finger metacarpal. The patient made a good functional recovery, returning to usual activities within six weeks. We suggest that this technique could be used to treat complex trapezium fractures that cannot be reconstructed with surgery.

Compression-plate fixation in acute diaphyseal fractures of the radius and ulna.

At the Campbell Clinic and City of Memphis Hospital from 1960 to 1970, 244 patients (216 with closed and twenty-eight with open fractures) had 330 acute diaphyseal fractures of the radius and ulna which were treated with ASIF compression plates and followed for from four months to nine years. One hundred and twelve patients had fractures of both bones of the forearm; fifty, single fractures of the ulna; and eighty-two, single fractures of the radius. In all, 193 fractures of the radius and 137 fractures of the ulna were treated by compression plating. Sixty-three patients (25.9 per cent) with

Clinical Results of Complex Subtrochanteric Femoral Fractures with Long Cephalomedullary Hip Nail.

PURPOSE: Good results of the cephalomedullary nails have been reported in proximal femoral fractures recently. Based on length of nails and shape of screws fixed in a femoral head for proximal fragment fixation, the proper nail length was in dispute. The purpose of this study was to evaluate the clinical and radiological results of a long cephalomedullary hip nail for the treatment of comminuted subtrochanteric femoral fractures. MATERIALS AND METHODS: Twenty-one consecutive patients with severe subtrochanteric femoral fractures who had undergone intramedullary fixation using long-PFNA II bet

Trans-scaphoid Perilunate Fracture-dislocation with Concomitant Lunotriquetral Ligament Disruption: A Case Report.

We report a case of trans-scaphoid perilunate fracture-dislocation with concomitant lunotriquetral ligament disruption of the right wrist in a 44-year-old man, sustained from a 10-m fall landing on his outstretched right hand. Open reduction was performed 1 day after injury; at first the palmar dislocation of the lunate was reduced with the palmar approach. Under direct view with the dorsal approach, the scaphoid was comminuted and then treated with open reduction and internal fixation with a double threshold screw using a dorsal approach and a bone graft from the distal radius. Although the s

Primary Total Knee Arthroplasty for Distal Femur Fractures: A Systematic Review of Indications, Implants, Techniques, and Results.

Distal femur fractures (DFFs) in elderly patients historically were difficult to treat because of osteoporotic bone, comminution, and intra-articular involvement. Current surgical treatment options, including intramedullary nailing, internal fixation, and external fixation, are complicated by prolonged immobility, malunion, and nonunion. Furthermore, fixation increases the complexity of subsequent arthroplasty. Primary total knee arthroplasty (TKA) is a rarely used treatment for acute DFF but may be of benefit in select patients. For a systematic review of the reported indications, techniques,

Cement-Augmented Screw Fixation for Unreconstructible Acetabular Posterior Wall Fractures: A Technical Note.

The management of severely comminuted acetabular posterior wall fractures in young, active patients presents a significant surgical challenge. When anatomical open reduction and internal fixation (ORIF) is not feasible, primary total hip arthroplasty (THA) is often considered but is a suboptimal solution due to concerns over long-term implant survivorship and the inevitability of revision surgery. This single-patient technical note presents a novel joint-preserving technique for managing unreconstructible acetabular posterior wall fractures using with cement-augmented screw fixation via the Ko
