Corticosteroid phobia and other confounders in the treatment of childhood atopic dermatitis explored using parent focus groups.

BACKGROUND/OBJECTIVES: Anxieties associated with corticosteroid treatment and preference for 'safer natural therapy' are common in parents of children with atopic dermatitis. We used focus groups to explore the source of these attitudes. METHODS: The study involved 16 parents. Parents expressed difficulties with living with and treating atopic dermatitis which were categorized into themes using qualitative data analysis software. RESULTS: Themes identified include: emotional impact of atopic dermatitis; difficulty in accepting 'control' verses 'cure'; topical corticosteroid negative percepti

Experiences of gamma hydroxybutyrate (GHB) ingestion: a focus group study.

GHB (gamma hydroxybutyrate) is a significant new drug of abuse added to the United States Controlled Substance Act in 2000. The majority of the published literature on GHB consists of clinical case reports, mainly from emergency departments, and a collection of laboratory-based studies, focused mainly on anesthesia. While comments about the various experiences and behaviors of human users are often included in such studies or reports, these aspects of GHB are only just beginning to be systematically investigated or detailed. Reported here are data from a qualitative study using focus group met

It's not what you know, but how you use it: statistical knowledge and adolescent problem gambling.

This paper examined the nature of irrational gambling-related cognitions in a sample of 926 adolescents (mean age = 14.5 years) sampled from Australian schools. Students were differentiated according to gambling status and administered a series of items that assessed their understanding of objective odds, the nature of randomness, the role of skill in gambling, and the perceived profitability of gambling. The results confirmed previous findings that problem gamblers tend to be more irrational in their perceptions, as indicated by stronger beliefs in the role of skilful play in chance activitie

Balancing effectiveness, side-effects and work: women's perceptions and experiences with modern contraceptive technology in Cambodia.

This community-based study presents the results of 17 focus-group discussions primarily among poor married women of reproductive age in urban and rural Cambodia regarding their experiences with modern contraceptive methods and their preferences for different technical attributes, including effectiveness, mode of administration, secrecy and rapid return of fertility. Key findings indicate that women who use modern contraceptive technologies desire highly effective methods of birth control. Cambodian women are primarily interested in longer-acting methods, view weight gain positively and are les

A medical-center-wide, multidisciplinary approach to the problem of natural rubber latex allergy.

Latex is a common cause of occupational allergy in health care workers; latex-sensitized patients are at increased risk of allergic reactions in medical environments. Skin test reagents and latex-specific immunoglobulin E immunoassays were established for diagnosis of latex allergy. Inhibition immunoassays were developed for measuring latex aeroallergens and latex allergens in rubber products. A registry of latex-sensitive employees was established. High-allergen gloves were removed from the medical center inventory; latex aeroallergen levels subsequently declined. Despite an increasing number

A survey of physician beliefs and self-reported practices concerning screening for early detection of cancer.

Cancer is the second leading cause of death in the United States. Early detection of cancer greatly improves 5-year survival for many sites, and in 1980 the American Cancer Society (ACS) published recommendations for performing cancer screening with the goal of promoting early cancer detection in asymptomatic persons. This cross-sectional survey examined beliefs and practices related to six cancer screening tests and procedures in a group of 68 primary care physicians in a multi-specialty group practice in Houston, Texas. Constructs from the Health Belief Model and Social Cognitive Theory were…

Effect of an evidence-based educational program on nurses' knowledge and practice toward surgical site infection prevention: A quasi-experimental study.

BACKGROUND: Surgical site infections (SSIs) remain a major cause of postoperative morbidity and increased healthcare costs worldwide. Evidence-based practices (EBPs) are essential for effective prevention; however, nurses' adherence is influenced by their knowledge, attitudes, and institutional training support. AIM: This study aimed to evaluate the effect of an evidence-based educational program on nurses' knowledge and practice regarding SSI prevention. METHODS: A quasi-experimental pretest-posttest design with experimental and control groups was used. The study included 120 nurses (60 per

Transcutaneous auricular vagus nerve stimulation during short-term motor practice drives cortical plasticity without behavioural improvement.

Transcutaneous auricular vagus nerve stimulation (taVNS) is emerging as a promising non-invasive neuromodulation technique to augment neurorehabilitation; however, its mechanisms in humans remain poorly understood. Animal studies indicate that VNS during motor skill practice drives task-specific plasticity in the primary motor cortex (M1), but direct evidence in humans is limited. Here, we demonstrate that taVNS paired with dexterous motor skill practice selectively enhances cortical plasticity without boosting motor performance during short-term training. Thirty-one healthy adults practiced a

Ipsilateral transfer of motor skill from upper to lower limb in healthy adults: a randomized controlled trial.

Intermanual transfer refers to the improvement of motor skill in an untrained contralateral limb following unilateral limb practice. However, it remains uncertain whether motor skill in the lower limb (LL) can improve as a result of practice with the unilateral upper limb (UL). Forty-five healthy participants were randomly allocated to one of three groups: (1) UL group, which practiced reaching movement (RM) sequences with the non-dominant left upper limb; or (2) switches observation (SO) group, which observed the same RM sequences; or (3) nature observation (NO) group, which observed nature m

Ipsilateral transfer of motor skill from lower to upper limb in healthy adults: A randomized controlled trial.

BACKGROUND AND PURPOSE: Whereas motor skills of the untrained upper limb (UL) can improve following practice with the other UL, it has yet to be determined if an UL motor skill can improve following practice of that skill with the lower limb (LL). METHODS: Forty-five healthy subjects randomly participated in a 10-minute single-session intervention of (1) practicing 50 reaching movement (RM) sequences with the non-dominant left LL toward light switches (LL group); or (2) observing the identical 50 light switches sequences (Switches Observation (SO) group); or (3) observing nature films (Nature

Inter-cycle and inter-observer variability of the antral follicle count in routine clinical practice.

Antral follicle count (AFC) is a reliable predictor of ovarian response to stimulation and its inter-cycle and inter-observer variability has been extensively studied on in vitro fertilization (IVF), mostly in highly selected populations within studies not originally designed for this purpose. In this retrospective cohort study, we assess the inter-cycle variation of AFC in a setting similar to that of the daily practice. We included only patients undergoing mild stimulation for intrauterine insemination (IUI). One hundred and forty-eight patients had two (62 patients, group A), three (49 pati

Progressive exercise compared with best practice advice, with or without corticosteroid injection, for the treatment of patients with rotator cuff disorders (GRASP): a multicentre, pragmatic, 2 × 2 factorial, randomised controlled trial.

BACKGROUND: Corticosteroid injections and physiotherapy exercise programmes are commonly used to treat rotator cuff disorders but the treatments' effectiveness is uncertain. We aimed to compare the clinical effectiveness and cost-effectiveness of a progressive exercise programme with a single session of best practice physiotherapy advice, with or without corticosteroid injection, in adults with a rotator cuff disorder. METHODS: In this pragmatic, multicentre, superiority, randomised controlled trial (2 × 2 factorial), we recruited patients from 20 UK National Health Service trusts. We include

Effect of yogic breathing (pranayama) on periodontal health status, salivary oxidative stress, and antioxidant levels in adults aged 35 to 44 in India: a cross-sectional study.

BACKGROUND: The overproduction of oxygen-derived free radicals and their byproducts, as well as a deficiency in antioxidants, are key factors in the progression of periodontitis. Pranayama, a yogic practice, involves deliberate, rhythmic, and intense movements and expansion of the respiratory organs. Engaging in rhythmic breathing exercises has been proven to lower resting levels of inflammatory cytokines and enhance the oxidant-antioxidant defence system. The objective of this study is to analyse the levels of salivary total antioxidant capacity (TAC) and salivary oxidative stress markers in

The role of motor imagery in learning a totally novel movement.

The aim of the present study is to gain more insight into the mechanisms underlying mental practice. The question of whether a totally novel movement may be learned by mental practice was investigated. Healthy young adults had to learn the abduction of the big toe (dominant right foot) without moving the other toes or the foot. The subjects were divided into two groups: subjects who were absolutely unable to abduct their big toe ("absolute zero" group) and subjects who were able to abduct their toe to some extent but showed clear room for improvement ("already doing it" group). Two separate ex

A clinical examination of OPTIMAL theory application in people with multiple sclerosis: a proof-of-concept study and implications for rehabilitation practice.

We investigated the potential to improve motor learning and performance in people with multiple sclerosis (PwMS) with OPTIMAL theory conditions. OPTIMAL theory predicts that three main factors [i.e. autonomy support (AS), enhanced expectancies (EE), and external focus (EF)] facilitate performance and learning. We examined whether the implementation of all three combined in a consecutive manner during practice would be beneficial for the motor learning and performance in a clinical population facing physical, cognitive, and emotional challenges. Thirty PwMS with mild-to-moderate disability (Exp

Tracheal tube cuff pressure depends on the anaesthesiologist's experience. A follow-up study.

BACKGROUND: Excessive tracheal tube cuff pressure can cause ischemia of the tracheal mucosa, and possible serious complications, such as tracheal stenosis, formation of tracheo-oesophageal fistula or even life-threatening haemorrhage. Inadequate cuff pressure increases the risk of aspiration of gastric contents. METHODS: The cuff pressures were analysed on the basis of the anaesthesiologists' experience. The results were compared to those obtained during the previous study which had been conducted seven years earlier (2002). The physicians were divided into three groups, according to their ex
