A case is reported of classic granulosa cell tumor of the testis with metastases to the retroperitoneal lymph nodes occurring in a 26-year-old man. The patient had left-sided testicular enlargement and bilateral gynecomastia. He was treated by radical orchiectomy, retroperitoneal lymph node dissection, and radiation therapy and is well without evidence of disease 14 years after diagnosis. This is the first documented case of granulosa cell tumor of the testis with metastases and long remission after successful therapy to the authors' knowledge.

A male infant is reported in whom substantial gynecomastia resolved following removal of a giant pigmented nevus. Endocrinological studies were normal. It is postulated that the nevus contributed to the development of gynecomastia. Surgeons should be aware of a possible relationship between these two seemingly unrelated conditions when making evaluation and management decisions.

Gynecomastia developed in two epileptic patients some months after the addition of oral fluoresone 750 mg daily to the phenobarbital and phenytoin already being administered. The common systemic diseases that may give rise to gynecomastia were excluded. One of the patients presented hyperprolactinemia and a raised estrogen/androgen ratio but the hormone levels were not raised in the other. The onset of symptoms after fluorescence in both cases is highly suggestive, although the pathogenetic mechanism is not clear.

Prolactin responses to provocative thyrotropin releasing hormone (TRH) stimulation were evaluated in 20 cirrhotic men with gynecomastia. Fifteen of these cirrhotic men had normal responses with a minimum doubling of the prolactin concentration above basal in response to TRH. Five had abnormal (autonomous) responses in that they failed to double their basal level or had a paradoxical decrease from basal in response to TRH. Immunohistochemical proof of A1FP production in the mammary gland of a man with gynecomastia is published here for the first time. The immunohistochemical proof of this fetal alpha-1-globulin in the intraductal epithelium of the mammary gland is described on the basis of the case history of a 25-year-old man with a metastasizing malignant teratoblastoma of the gonads. The question, to what extent A1FP-neosynthesis is characteristic for malignant tumors or whether it is a cellular response to specific, in this case hormonal growth stiumli of primary tumors, is also discussed.
