Neuromodulation of the cingulum for neuropathic pain after spinal cord injury. Case report.

The authors present a case in which high-frequency electrical stimulation of the cingulum using standard deep brain stimulation (DBS) technology resulted in pain relief similar to that achieved with cingulotomy and superior to that achieved with periventricular gray matter (PVG) stimulation. This patient had a complete spinal cord injury at the C-4 level and suffered from medically refractory neuropathic pain. He underwent placement of bilateral cingulum and unilateral PVG DBS electrodes and a 1-week blinded stimulation trial prior to permanent implantation of a pulse generator. During the sti

[Amnesia following left medial frontal subcortical hemorrhage: a case report].

There has been a controversy as to the contribution of the frontal lobe to human memory function. We describe a 49-year-old right-handed patient with memory disturbance following a left medial frontal subcortical hematoma. Her amnesia was characterized by (1) predominant anterograde amnesia, (2) difficulty in both voluntary recall and recognition tasks, (3) a great number of false-alarm responses in a recognition task, and (4) no confabulation. An MRI demonstrated that her lesion was restricted to the left medial frontal area and anterior cingulate gyrus. This case represents a rare instance o

Stereotaxic rTMS for the treatment of auditory hallucinations in schizophrenia.

Auditory verbal hallucinations in schizophrenia may be due to dysfunctional inner speech-related cortical areas. Repetitive transcranial magnetic stimulation (rTMS) has been reported to be an effective treatment of hallucinations. In a cross-over sham controlled study, we guided rTMS stereotactically to inner speech-related cortical areas in hallucinating patients. These areas were identified individually prior to rTMS using fMRI in a subgroup of our patients. Active stimulation was applied over Broca's area and over the superior temporal gyrus as determined by fMRI, or according to structural

An fMRI study of imagined self-rotation.

In the present study, functional magnetic resonance imaging was used to examine the neural mechanisms involved in the imagined spatial transformation of one's body. The task required subjects to update the position of one of four external objects from memory after they had performed an imagined self-rotation to a new position. Activation in the rotation condition was compared with that in a control condition in which subjects located the positions of objects without imagining a change in self-position. The results indicated similar networks of activation to other egocentric transformation task

Bilateral anterior cingulotomy for chronic noncancer pain.

OBJECTIVE: To document the value of bilateral anterior cingulotomy for patients with intractable chronic noncancer pain. METHODS: Twenty-three patients who underwent 28 cingulotomies between 1979 and 1996 for chronic refractory pain were sent questionnaires regarding their subjective response to the surgery and its impact on their pain. Questions dealt with pre- and postoperative pain, ability to resume work or usual activity, medications, family and social interactions, and overall benefit of cingulotomy. Results were compared with long-term (average, 8 yr) clinical follow-up. In 13 patients

The nature and time course of neuronal vacuolation induced by the N-methyl-D-aspartate antagonist MK-801.

N-Methyl-D-aspartate (NMDA) antagonists cause neuronal vacuolation in the posterior cingulate and retrosplenial cortex of the rat. Because the nature of neuronal pathologic changes due to NMDA antagonists may affect the potential clinical use of this class of drugs, we undertook experiments to define the nature and time course of the vacuolation caused by high-dose (5 mg/kg) MK-801 (dizocilpine, 5-methyl-10,11-dihydro-5H-dibenzo[a,d]cyclohepten-5,10-imine). Ultrastructural examination revealed the vacuoles to be not a form of hydropic cellular degeneration, but rather a dilatation of several i

Comparative electrophysiology of pyramidal and sparsely spiny stellate neurons of the neocortex.

Slices of sensorimotor and anterior cingulate cortex from guinea pigs were maintained in vitro and bathed in a normal physiological medium. Electrophysiological properties of neurons were assessed with intracellular recording techniques. Some neurons were identified morphologically by intracellular injection of the fluorescent dye Lucifer yellow CH. Three distinct neuronal classes of electrophysiological behavior were observed; these were termed regular spiking, bursting, and fast spiking. The physiological properties of neurons from sensorimotor and anterior cingulate areas did not differ sig

Functional imaging of time on task and the involvement of dopaminergic and cholinergic substrates in cognitive effort and reward.

Neuroimaging studies have identified the neural substrates associated with sustained cognitive efforts and control and their modulation by rewards. Different lines of evidence implicate the prefrontal cortex (especially the anterior cingulate cortex, ACC), dopaminergic, and cholinergic substrates in this modulation. We studied here the activity of these substrates at increasing time on task (requiring increasing levels of cognitive effort) in trials within blocks with differing reward levels. In the cortex, while peaking in the ACC, activity associated with time on task was extensive, also inc

Anterior cingulate cortex activation of claustrum projection neuron subtypes is enhanced by alcohol.

Cognitive impairment is a major component of Alcohol Use Disorder. Optimal cognitive performance requires anterior cingulate input activation of the claustrum, a subcortical nucleus that orchestrates cortical activity. Yet the impact of chronic alcohol exposure on the ability for the anterior cingulate cortex to drive activity of claustrum projection neuron subtypes is unknown. In adult male and female mice, we found that the majority of non-burst firing Type 1 claustrum projection neurons did not express the vesicular glutamate transporter 2 (VGLUT2), while the majority of burst firing Type 2

Associations between glutamate and cerebral blood flow in treatment-resistant schizophrenia during clozapine treatment.

BACKGROUND: Treatment-resistant schizophrenia (TRS) is associated with alterations in glutamate levels and cerebral blood flow (CBF), and treatment with clozapine appears to have an impact on each of these measures. Here, we examined whether changes in CBF and glutamate levels following clozapine treatment are related. METHODS: Glutamate concentrations in the anterior cingulate cortex (ACC), and striatum (measured using proton magnetic resonance spectroscopy, 1H-MRS); and whole brain regional CBF maps (measured using pseudo-continuous arterial spin labelling (pCASL)), were examined in a cohor

The Desegregation of Neural Networks During Worry Induction in Late Life: An Effective Connectivity Analysis.

BACKGROUND: Severe worry is a core component of anxiety and depressive disorders and is independently associated with significant morbidity and mortality. However, the neural basis of worry is poorly understood. We investigated effective connectivity (EC) using functional magnetic resonance imaging of a naturalistic worry induction and reappraisal task in late life. METHODS: A total of 112 participants age >50 years with varying worry severity completed a personalized, in-scanner worry induction and reappraisal task. We calculated voxelwise EC in neutral, worry, and reappraisal conditions wit

Microstructural white matter impairments in chronic fatigue syndrome: Evidence of segmental injury in the cingulum bundle.

BACKGROUND: Chronic fatigue syndrome (CFS) is a neurological disorder characterized by persistent fatigue. Previous studies have shown structural and functional brain alterations in CFS patients. However, how chronic fatigue affects white matter remains unclear. METHODS: We recruited 100 CFS patients and 100 healthy controls (HCs) and collected their diffusion weighted imaging (DWI) data along with fatigue severity scores. Group comparisons were conducted using 87 white matter tract templates derived from 1068 healthy individuals to identify impaired tracts. Fiber bundle quantitative analysis

Functional connectivity patterns of the fronto-parietal and cingulo-opercular networks demonstrate distinct associations with individual differences in cognitive control during early adolescence.

Cognitive control refers to a set of mental processes that enable individuals to flexibly and adaptively engage in goal-directed behavior. Adolescence is characterized by the emergence and subsequent rapid development of adult-like cognitive control abilities, and as such, there is great interest in understanding the neural basis of this maturational process. The goal of the present study was to investigate how the resting-state and task-based functional connectivity (FC) patterns of two brain networks implicated in control processes, the fronto-parietal network (FPN) and the cingulo-opercular

Neural and somatic mechanisms driving clinical improvements in post-acute schizophrenia spectrum disorders.

BACKGROUND: A better mechanistic understanding of schizophrenia spectrum disorders is crucial to developing efficient treatment approaches. Therefore, this study investigated longitudinal interrelations between clinical outcomes, brain structure, and somatic health in post-acute individuals from the schizophrenia spectrum. METHODS: A sample of 63 post-acute patients from two independent physical exercise studies was included in the final analyses. Demographic, clinical, cognitive, and somatic data were acquired at baseline and follow-up, as were structural magnetic resonance imaging scans. Mu

Neural, Motivational, and Psychological Measures of Pain Avoidance Predict Future Alcohol Use in Adult Drinkers.

Drinking as a coping method in response to pain is a complex behaviour, involving multiple neural, motivational, and psychological factors. Among these factors, pain sensitivity and pain-related drinking motive can significantly promote alcohol use. In contrast, proactive avoidance - a beneficial strategy of initiating overt actions to avoid negative outcomes - reduces harmful consumption. Yet, these factors have not been assessed as potential predictors of future drinking behaviour. Here, in a longitudinal study we collected fMRI data in 50 drinkers who, at baseline, performed a probabilistic

Low level alcohol consumption is associated with lower regional brain volume and thickness and lower choline-containing compounds and myo-inositol levels in healthy adults.

Low levels of alcohol consumption (e.g., up to two standard drink equivalents/day for males and one drink/day for females) have been viewed as benign or even beneficial, particularly for cardiovascular function. A limited number of studies investigated associations of alcohol consumption with brain volumes and metabolite levels in "Работает" cohorts without alcohol use disorder (AUD) and yielded mixed results. To date, no study has concurrently assessed brain morphometrics and metabolites in regions that previously showed associations with alcohol consumption in healthy adults. The current stud

Large-scale functional network connectivity alterations in adolescents with major depression and non-suicidal self-injury.

Non-suicidal self-injury (NSSI) is prevalent among adolescent populations worldwide, yet its neuropathological mechanisms remain unclear. This study aimed to investigate brain functional differences in NSSI patients by utilizing large-scale functional networks and examining their correlation with clinical outcomes. Cross-sectional clinical and functional magnetic resonance imaging (fMRI) data were collected from 42 patients and 47 healthy controls. Independent component analysis (ICA) was utilized to investigate changes in both intra-network and inter-network functional connectivity. We then i

Repetitive transcranial magnetic stimulation-driven modulation of unbiased functional connectivity in the supracallosal anterior cingulate cortex causally ameliorates information processing speed in amnestic mild cognitive impairment.

INTRODUCTION: Amnestic mild cognitive impairment (aMCI) exhibits biased functional connectivity (FC) abnormalities impairing neural plasticity modulation. This study aimed to identify unbiased FC deficits using a brain-wide association study (BWAS) and investigate repetitive transcranial magnetic stimulation (rTMS)-driven plasticity restoration. METHODS: BWAS identified unbiased FC-altered voxels (robustness-validated). Region-of-interest (ROI)-wise FC analysis localized disrupted circuits, which were modulated through precuneus-targeted rTMS. Effective connectivity (EC) tested whether the pr

The Effect of N-Acetylcysteine (NAC) on Neurometabolites and Cognitive Function in Adults With Alcohol Use Disorder: A Preliminary Randomized Controlled Trial.

BACKGROUND AND AIMS: Preclinical studies have demonstrated that N-acetylcysteine stabilizes levels of glutamate and glutathione and reduces alcohol-seeking behaviors, indicating it as a potential pharmacotherapy for the management of alcohol use disorder. In this preliminary study, we examined brain metabolite levels and cognitive functioning in individuals with alcohol use disorder enrolled in a randomized controlled trial of N-acetylcysteine versus placebo. METHODS: In this preliminary trial, 23 participants (average age = 49; 70% male) with moderate to severe alcohol use disorder (DSM-5) w

Sjögren's syndrome is associated with a reduction in the surface area of the right caudal anterior cingulate gyrus.

BACKGROUND: Sjögren's syndrome (SS) is a common chronic autoimmune disease. Neurological involvement in SS represents one of the more severe and challenging aspects, with complications affecting the central system leading to cognitive dysfunction, sensory neuropathy, and multifocal sensorimotor neuropathies among other cortical function abnormalities. The relationship between cerebral cortex structure and neurological pathologies is well-documented, yet the impact of SS on cerebral cortex structure remains unclear. METHODS: A two-sample Mendelian randomization (MR) analysis was conducted usin
