Managed alcohol: one community's innovative response to risk management during COVID-19.

BACKGROUND: Harm reduction programs often lack community-based support and can be controversial, despite data demonstrating effectiveness. This article describes one small Alaskan community's development of a harm reduction managed alcohol program (MAP) in the context of a city-run quarantine site for individuals experiencing homelessness. The MAP was developed to support quarantining by COVID-19-exposed or COVID-positive individuals who also experienced chronic homelessness, a severe alcohol use disorder, and heightened health risks related to potentially unsupported alcohol withdrawal. METH…

Overdose risk in the context of chemsex among gay, bisexual, and other men who have sex with men.

Chemsex is defined as the use of psychoactive substances - most commonly methamphetamine, synthetic cathinones, and Gamma-hydroxybutyrate/Ga … l knowledge gaps regarding the association between chemsex and overdose risk among GBM, and outline harm reduction and behavioral interventions. Assessing chemsex-related overdose deaths among GBM is challenging due to the limited documentation of sexual practices and sexual orientation in medical or legal records. Stigma further reduces disclosure, and chemsex …

"When" matters: Differences in fatal opioid-related overdose events in Massachusetts by time of day, 2019-2023.

BACKGROUND: Geospatial analyses of opioid involved overdose deaths can inform community harm reduction efforts. Differences in overdose deaths by time of day are important, as overdose events may be more likely to go unrecognized at night when fewer community resources are available. METHODS: Using Massachusetts Registry of Vital Records and Statistics data (2019-2023), we utilized regression analyses to evaluate demographics, residence, and toxicology differences associated with the recorded time of opioid involved overdose deaths, stratified by day (6AM-5:59PM) and night (6PM-5:59AM). Kerne…

Policing and nonfatal overdose in a community-based cohort of sex workers in Vancouver, Canada: influence of stigma and other intersecting systems of marginalization (2014-2024).

BACKGROUND: We evaluated the association between policing and recent (last 6 months) nonfatal overdose, and differences based on stigma and other intersecting systems of marginalization, among a cohort of sex workers in Canada. METHODS: Data were drawn from a cohort of sex workers in Vancouver, Canada (9/2014-2/2024). We used generalized linear mixed models to examine the relationship between police-related factors (interactions, displacement, and barriers to safer sex work transactions and harm reduction) and recent nonfatal overdose. Stratified models evaluated potential heterogeneity in as…

Daily associations among protective behavioral strategies, alcohol use, and consequences in a sample of adolescents and young adults: The influence of drinking context and neighborhood disadvantage.

OBJECTIVE: Protective behavioral strategies (PBS) may be less effective in certain contexts, as high-risk situations and structural disadvan … hood disadvantage on daily associations between PBS (limiting/stopping, manner of drinking, serious harm reduction) and alcohol outcomes (number of drinks, negative consequences [any and number]). METHODS: Participants aged 15-25 in Texas (N=581; Mage=21.04, SD=2.80; 55.25% female; 45.14% White non-Hispanic) completed a longitudinal ecological momentary assessm …

Self-Harm and Suicide Attempts Preceding and Following Electroconvulsive Therapy: A Population-Based Study.

OBJECTIVE: The beneficial effect of electroconvulsive therapy (ECT) on suicidality has been documented in clinical trials, whereas naturalistic studies on the topic are scarce and restricted to individuals with mood disorders. Here, based on population-based data from Danish registers, we aimed to investigate the course of self-harm and suicide attempts preceding and following ECT across 4 major mental disorders. This was done to examine whether data from the real-world clinical setting are compatible with the positive results from clinical trials. METHODS: We identified all patients diagnose

Navigating Consent and Confidentiality in Adolescent Substance Use Treatment.

Consent and confidentiality are central to adolescent substance use care, functioning not only as legal and ethical frameworks but as early preventive interventions during a critical developmental window. This urgency is highlighted by rising rates of substance use disorders and overdose deaths driven by illicit fentanyl and polysubstance exposure. Expanded harm reduction efforts have intensified tensions between adolescent autonomy, parental involvement, legal requirements, and the clinician's duty to protect safety. This article provides a clinically grounded framework for navigating consent…

Binge-type eating disorders and ultra-processed food addiction: phenomenology, pathophysiology and treatment implications.

BACKGROUND AND OBJECTIVE: Despite their clinical differences, loss of control binge eating (LCBE) is a core feature of all binge-type eating … powerful addictive properties. However, the key substance use disorder (SUD) recovery protocols of harm reduction or abstinence from addictive substances are not commonly employed in the treatment of binge-type EDs. The objectives of this paper are to organize evidence that the LCBE characteristic of binge-type EDs and UPFA overlap in many cases and to consider …

'I'm doing it willingly': understanding moral agency, empathy, and stigma in the context of sexualised drug use.

Research on sexualised drug use often emphasises adverse health outcomes and risk management with limited exploration of the social and ethical dimensions of moral agency and harm reduction practices. This study examined the interplay between moral agency, empathy, and stigma. Participants (age >18 years and California residents) who engaged in drug use during sex within the past six months, excluding alcohol-only use, were recruited through mobile hook-up apps, websites, and snowball sampling. Eighteen semi-structured in-depth interviews were conducted with predominantly cisgender gay male pa…

The New Zealand drug harms ranking study: A multi-criteria decision analysis.

AIMS: The harms arising from psychoactive drug use are complex, and harm reduction strategies should be informed by a detailed understanding of the extent and nature of that harm. Drug harm is also context specific, and so any comprehensive assessment of drug harm should be relevant to the characteristics of the population in question. This study aimed to evaluate and rank drug harms within Aotearoa New Zealand using a multi-criteria decision analysis (MCDA) framework, and to separately consider harm within the total population, and among youth. METHODS: Two facilitated workshops involved the…

Harm-to-self from gambling: A national study of Australian adults.

AIMS: Understanding how gambling harm is distributed is essential to inform effective harm reduction measures. This first national Australian study of gambling harm-to-self examined the extent, distribution, risk factors, and health related quality of life (HRQoL) impacts of this harm. METHODS: A Random Digit Dialling sample of 15,000 Australian adults was weighted to key population variables. Key measures included the Gambling Harms Scale-10 (GHS-10), PGSI, SF-6D, gambling behaviours, and demographics. Analyses included ordinal logistic regression. RESULTS: Amongst gamblers, 14.7% reported …

Real-world effectiveness of medication-assisted treatment and psychotherapy for opioid use disorder: a national multi-health care organization analysis.

BACKGROUND: Harm reduction strategies for opioid use disorder (OUD) emphasize pragmatic, evidence-based approaches that reduce overdose risk, relapse, and other adverse outcomes without requiring abstinence. Medication for opioid use disorder (MOUD) and structured psychotherapy represent core harm-reduction modalities, yet their real-world comparative effectiveness, alone and in combination, remains underexplored at scale. METHODS: A retrospective cohort study was conducted using the TriNetX Research Network, comprising de-identified electronic health records from 112 U.S. health systems. 18,…

Substance use care innovations during COVID-19: barriers and facilitators to the provision of safer supply at a toronto COVID-19 isolation and recovery site.

BACKGROUND: Early in the COVID-19 pandemic, there was an urgent need to establish isolation spaces for people experiencing homelessness who were exposed to or had COVID-19. In response, community agencies and the City of Toronto opened COVID-19 isolation and recovery sites (CIRS) in March 2020. We sought to examine the provision of comprehensive substance use services offered to clients on-site to facilitate isolation, particularly the uptake of safer supply prescribing (prescription of pharmaceutical opioids and/or stimulants) as part of a spectrum of comprehensive harm reduction and addictio…

Consuming and Thinking About Nangs: A Survey of Nitrous Oxide Use, Knowledge, Attitudes and Perceptions in Aotearoa New Zealand.

Non-medical use of nitrous oxide (N2O) is becoming increasingly popular globally, yet little data exists regarding N2O in Aotearoa New Zealand. We aimed to explore patterns of use and harm in those who consume N2O in Aotearoa, and related knowledge, perceptions and attitudes of those with and without N2O experience. A convenience sample of people with N2O experience (n = 466) and without (n = 510) completed an online survey about N2O. Most with N2O experience reported infrequent and low-dose consumption. Harm reduction behaviors were utilized by most consumers, and reports of adverse effects w…

A study protocol to explore the effectiveness and implementation of mobile community outreach services for women who use drugs in Baltimore, Maryland: The SHOUT Study.

The opioid overdose crisis has continued to affect women who use drugs (WWUD), particularly in urban cities such as Baltimore, Maryland, where fatal overdose rates rank among the highest in the nation. Despite evidence demonstrating the impact of mobile health services in serving underserved populations, few interventions are specifically tailored to meet the unique needs of WWUD. The Sustained Harm Reduction OUTreach (SHOUT) study evaluates the effectiveness and implementation of a harm reduction-based mobile outreach service operated by a community-based organization serving WWUD in Southwes…

Searching the Internet for drug-related web sites: analysis of online available information on ecstasy (MDMA).

Although the Internet is a growing source of information on MDMA/ecstasy, no studies so far have investigated the level and quality of ecstasy information available to the typical Web user. In the present study, 280 Web sites were identified and analyzed; 50.4% had an anti-drug approach, 16.2% a harm reduction approach, and 24.8% a pro-drug approach. MDMA pro-drug Web sites appeared significantly earlier in the search engines' results list than both anti-drug and harm reduction Web sites (F (3; 159) = 3.288; p = .022). This study represents the first systematic analysis of information availabl…
