The Kerboull cross-plate: from the birth of a concept to the current indications.

PURPOSE: Reconstruction of major acetabular bone loss remains a demanding challenge during revision total hip arthroplasty. Since its original description, the Kerboull acetabular reinforcement device, also known as the Kerboull cross-plate or cage, has been developed to restore the native hip centre of rotation, to protect bone graft from early mechanical overload, and to allow cementation of a conventional full-polyethylene or a dual mobility cup. METHODS: This narrative review traces the evolution of the Kerboull cross-plate from its original conception to its current indications. RESULTS

Objective outcome analysis of soft shell helmet therapy in the treatment of deformational plagiocephaly.

Deformational plagiocephaly, cranial asymmetry secondary to positioning, continues to be a leading cause of head shape abnormalities in infants. Treatment recommendations include nonintervention, positioning therapies, and helmet therapy. Although most agree that surgical intervention is rarely indicated, the ideal therapy is not agreed on. Some even debate the necessity of treatment, especially third-party payers. The purpose of this prospective study is to use an objective outcome analysis tool, computerized tomography, to assess the efficacy of a soft shell helmet therapy. Sixty-nine childr

A signature wound of war: mild traumatic brain injury.

Improvised explosive devices are the weapons of choice for the insurgent enemy in Iraq and Afghanistan. More soldiers are surviving these blast injuries due to improved torso protection yet are sustaining head and neck wounds in numbers that exceed those from previous wars. Although moderate and severe traumatic head injuries are easily identified and aggressively treated, mild traumatic brain injuries (m-TBIs), or concussions, had previously been deemed inconsequential and often overlooked. Recently, however, the U.S. Department of Defense and Veterans Health Administration have placed emphas

[Reduction techniques for minimally invasive stabilization of proximal humeral fractures].

OBJECTIVE: The aim of surgical stabilization of proximal humerus fractures is to restore the anatomical relations between the greater and lesser tubercle, to reconstruct the joint and preserve the vascular supply of the fragments. INDICATIONS: Approximately 80% of proximal humeral fractures can be treated conservatively. Surgical treatment is indicated based on the fracture pattern, patient-related factors and the risk of avascular head necrosis. Two-part fractures with a metaphyseal comminution zone and 3/4-part fractures can benefit from near to anatomic reconstruction depending on the pati

Vacuum-assisted closure of the scalp flap following malignant cerebral edema during craniotomy: pediatric case presentations and review of the literature.

Intraoperative malignant cerebral edema is a rare but life-threatening complication that can arise during a traumatic decompressive craniectomy. The swelling may be significant enough to preclude skin closure. The current standard of care is to resect sufficient edematous brain tissue to close the scalp to establish an immune-competent barrier to the intracranial space. We report two pediatric patients who developed malignant edema during craniectomy that prevented skin closure. Both patients underwent placement of a temporary vacuum-assisted closure (VAC) device, facilitating resolution of th

An enhanced method for evaluating the effectiveness of protective devices for road safety application.

This paper presents an enhanced probabilistic approach to estimate the real-world safety performance of new device concepts for road safety applications from the perspective of Powered Two-Wheeler (PTW) riders who suffer multiple injuries in different body regions. The proposed method estimates the overall effectiveness of safety devices for PTW riders by correlating computer simulations with various levels of actual injuries collected worldwide from accident databases. The study further develops the methodology initially presented by Johnny Korner in 1989 by introducing a new indicator, Globa

Comparative Outcomes of Fronto-Orbital Advancement and Strip Craniectomy in the Management of Metopic Craniosynostosis.

OBJECTIVE: Metopic craniosynostosis results in characteristic trigonocephaly and may impair cranial growth. Surgical correction is traditionally performed using fronto-orbital advancement (FOAR), while strip craniectomy with postoperative helmet therapy (SC) has emerged as a minimally invasive alternative. This study compares perioperative outcomes and postoperative cranial shape between FOAR and SC. METHODS: A retrospective cohort study was conducted at a tertiary pediatric craniofacial unit. Patients with isolated metopic craniosynostosis treated with FOAR or SC between 2015 and 2024 were i

Injury patterns of e-scooter-related orthopaedic trauma in central London: a multicentre study.

PURPOSE: Electric scooters (e-Scooters) are being increasingly used in urban areas as a new means of transport. E-scooter sharing schemes have recently been piloted in the United Kingdom; however, there are no published data on orthopaedic injuries and user behaviour patterns in the UK. We aim to identify the patterns and severity of orthopaedic injuries related to e-scooter use. METHODS: We performed a retrospective review of all orthopaedic referrals relating to e-scooter use from 1 March to 30 November 2020 at three hospitals, including one major trauma centre in central London. Data inclu

Spatial regression analysis of MR diffusion reveals subject-specific white matter changes associated with repetitive head impacts in contact sports.

Repetitive head impacts (RHI) are a growing concern due to their possible neurocognitive effects, with research showing a season of RHI produce white matter (WM) changes seen on neuroimaging. We conducted a secondary analysis of diffusion tensor imaging (DTI) data for 28 contact athletes to compare WM changes. We collected pre-season and post-season DTI scans for each subject, approximately 3 months apart. We collected helmet data for the athletes, which we correlated with DTI data. We adapted the SPatial REgression Analysis of DTI (SPREAD) algorithm to conduct subject-specific longitudinal DT

Factors influencing protective equipment use by mountain bikers: Implications for injury prevention.

AIM: To assess the attitudes of mountain bikers to the use of protective equipment and quantify the use of such equipment. METHOD: This was a prospective cohort study using an online questionnaire, offered to bikers participating in a series of Enduro races. The attitudes towards various factors that might contribute to a rider's choice to use protective equipment were quantified based on their responses to the questions. The actual reported use of various types of protective equipment was the outcome measure. The correlations between the factors and actual use were analysed for statistical s

Three-dimensional biomimetic head model as a platform for thermal testing of protective goggles for prevention of eye injuries.

BACKGROUND: The rate of eye injury is steadily rising during military conflicts of the century, with thermal burns being the most common type of injury to the eyes. The present study focuses on assessing the heat resistance properties of military protective goggles using three-dimensional (3D) finite element head modeling fitted with the tested protective gear. METHODS: A computational thermal impact was applied onto a 3D biomimetic human head model fitted with two goggle models - sports (Type 1) and square (Type 2). The resultant temperature of the eye tissues and the thermal injury threshol

Aggravating and mitigating factors associated with cyclist injury severity in Denmark.

INTRODUCTION: Denmark is one of the leading cycling nations, where cycling trips constitute a large share of the total trips, and cycling safety assumes a top priority position in the agenda of policy makers. The current study sheds light on the aggravating and mitigating factors associated with cyclist injury severity on Danish roads by examining a comprehensive set of accidents involving a cyclist and a collision partner between 2007 and 2011. METHOD: This study estimates a generalized ordered logit model of the severity of cyclist injuries because of its ability to accommodate the ordered-
