The effects of reverse atrial electrical remodeling on atrial defibrillation thresholds.

The implantable atrial defibrillator is a developing therapeutic option for paroxysmal atrial fibrillation, but shock related discomfort continues to be a limiting factor. To further characterize successful defibrillation, the relationship between reverse atrial electrical remodeling and internal atrial defibrillation thresholds in canines with chronic atrial fibrillation was examined. Testing was performed in 21 dogs. Chronic atrial fibrillation was induced in eight dogs by creating moderate mitral regurgitation and rapidly pacing the right atrium for > or = 6 weeks. The atrial fibrillation c

Algebraic decomposition of the TU wave morphology patterns.

In principle, the T wave results from the differences in durations of action potentials (AP) of different ventricular regions. Based on this concept, a mathematical model has been developed that represents the TU wave morphology as a summation of four AP-like functions: TU = S1 - S2 + L1 - L2. The sigmoidal shape of AP-like curves is produced by Hill's equation V(t) = a . tn/(bn + tn). Each of the decomposition functions is characterized by two parameters: the amplitude at the beginning of QRS (Amax), and the duration at 5% of Amax (D95). The set of four decomposition functions leads to eight

Radiofrequency catheter ablation of an accessory atrioventricular connection in a Fontan patient.

Radiofrequency catheter ablation is fast becoming the procedure of choice for the nonpharmacological treatment of atrioventricular connections that are responsible for debilitating tachycardias. We, herein, present a case of reentrant supraventricular tachycardia secondary to an atrioventricular connection in a Fontan patient that was successfully treated with radiofrequency catheter ablation.

[Retrograde conduction (ventriculo-atrial) in patients with AV or intraventricular block. Electrophysiologic evaluation].

The purpose of the study was to evaluate ventriculoatrial conduction in patients with chronic complete AV block (AVB) (35 pts--gr. A), who were qualified for pacemaker implantation and in patients with bifascicular block (BFB) (20 pts--gr. B), who had electrophysiological study performed to determine indications for permanent pacing. Anterograde and retrograde conduction by accessory pathways were excluded in all patients. Incremental ventricular pacing (60-80 bpm to 200-220 bpm) and programmed ventricular stimulation were performed. Ventriculoatrial (VA) conduction was present in 12 of 35 pat

Cardiac pacing in special and complex situations. Indications and modes of stimulation.

This article reviews a variety of pacing options for those patients with special and complex situations. Syndromes and syncopes, cardiomyopathy, and coronary artery disease are reviewed. Pacing modes are also discussed.

Hemodynamics and exercise capacity during pacemaker stimulation.

This review summarizes the present knowledge concerning the hemodynamic and myocardial effects of various pacing modalities with special reference to the importance of heart rate variability and atrioventricular synchronization. An adequate increase in heart rate, irrespective of atrioventricular synchronization, seems to be the most important denominator for cardiac output and exercise tolerance. Atrioventricular synchronization will add some hemodynamic benefit, which is most pronounced at rest. The importance of a rate-adaptive atrioventricular delay and a normalized ventricular activation

Atypical Wenckebach block and concealed conduction.

Following exercise testing, a 71-year-old man developed atrial tachycardia with a unusual form of conduction disturbance: 4:2 atrioventricular (A-V) block. This pattern of A-V block is explainable by two different mechanisms of concealed conduction: two different levels of block in the A-V nodal region and dual A-V nodal pathways with concealed retrograde conduction of P waves. These concepts can also explain a variety of other unusual conduction disturbances.

Sinuatrial disease in young people.

A survey at three cardiac centres disclosed nine patients under the age of 25 years with sinus node dysfunction in the absence of other forms of heart disease. All were male and seven were above the 90th centile for height. Ambulatory monitoring was performed on all the first-degree relatives of six of the patients and three families showed conducting system disturbances in the form of sinuatrial disorders or delayed atrioventricular conduction. A genetic factor may be involved in the aetiology of sinuatrial disease in young people.

Ventricular preexcitation syndrome. Accessory left atrioventricular connection and rhabdomyomatous myocardial fibers.

A necropsy study of the heart of a 16-year-old girl with ventricular preexcitation syndrome and supraventricular tachycardia showed an accessory left atrioventricular connection partially within the mitral valve. Both the accessory pathway and the ventricular musculature showed multiple foci of cardiac muscle fibers with rhabdomyomatous features having the characteristic appearance of rhabdomyomatosis. The case is discussed in the context of a literature review of necropsy findings in cases of ventricular preexcitation.

Alternans of the repolarization wave in a case of hypochloremic alkalosis with hypopotassemia.

A case of profound hypochloremic alkalosis with hypopotassemia is reported, showing electrocardiographic changes of electrical alternans of the repolarization wave (probably the U wave) without any change in the QRS complex. Transient concomitant P-pulmonale was noted. Hypopotassemia is discussed as a possible mechanism for the development of the electrical alternans.
