Optimizing Mechanical Ventilation in Right Heart Failure.

In patients with acute respiratory distress syndrome, the prevalence of right ventricular dysfunction (RVD) has been documented as high as 60%, though more recent estimates suggest a lower incidence of 20% to 30%, potentially reflecting the broader implementation of lung-protective ventilation strategies. The objective of this article is to review the physiologic principles of mechanical ventilation considering the unique challenges posed by RVD, and to offer a practical bedside approach to optimizing ventilatory support in this patient population. Importantly, mechanical ventilation strategie

Pediatric heart failure: Current approach and treatment.

Pediatric heart failure (HF) is a heterogenous disease process. While the incidence is low compared to adults, resource utilization and in hospital mortality is higher. We aim to review the current approaches and treatment strategies for pediatric HF and discuss ongoing efforts to improve outcomes for this patient population. Pediatric HF is a diverse disease entity with variable pathophysiologic processes and symptoms. This review will focus on myocardial failure related to intrinsic cardiomyocyte, or cardiac muscle cell, dysfunction. While myocardial failure is a shared pathway in adult and …

Diuretic Potentiation Strategies in Acute Heart Failure.

Several trials have evaluated diuretic-based strategies to improve symptoms and outcomes in patients with acute heart failure (AHF). The authors sought to summarize the effect of different combination strategies on symptoms, physical signs, physiological variables, and outcomes in patients with AHF. Twelve trials were identified that assessed the addition of thiazide diuretics, sodium-glucose cotransporter 2 inhibitors, mineralocorticoid receptor antagonists, vasopressin receptor antagonists, carbonic anhydrase inhibitors, or loop diuretic intensification to conventional therapy for AHF. The t…

[Inhospital diuretic strategies for acute heart failure].

Acute heart failure is frequently encountered in acute medicine wards. Even without randomized placebo-controlled clinical studies, intravenous furosemide remains the diuretic treatment of choice. However, it sometimes proves insufficiently effective. The arsenal of diuretic treatments has significantly evolved over the past years, with an increasing amount of evidence supporting the use of both novel and older molecules for treating patients with cardiac decompensation. These include thiazides, acetazolamide, and sodium-glucose cotransporter-2 inhibitors.

Heart Failure Transitional Care and Ambulatory Monitoring.

Chronic heart failure is characterized by periods of stability punctuated by clinical decline often requiring hospitalization. Following hospitalization, patients remain at an increased risk of morbidity and mortality, often requiring rehospitalization due to recurrent deterioration. Transitional care interventions aim to improve the movement from the inpatient to ambulatory setting. Promising interventions, such as patient education, novel uses of loop diuretics, and invasive hemodynamic monitoring, have been developed over the past 20 years all with varying degrees of success.

Stem Cell Therapy in Ischemic Heart Failure.

Stem cells have emerged as a promising therapeutic approach for ischemic heart failure, with multiple preclinical and clinical trials demonstrating encouraging outcomes. However, limitations and challenges encountered during clinical trials or follow-up periods hinder stem cell therapy's clinical translation for heart failure. In this review, we will elaborate on the applications of stem cell-based therapy, the main subtypes and fundamental properties of stem cells, and the mechanisms by which stem cells exert their effects in ischemic heart failure, such as remuscularization, paracrine effect…

Tracking Pathways Linking Obesity with Heart Failure.

Obesity can cause the onset of heart failure and exacerbate the status of the pre-existing disease. Through intricate pathways, obesity activates hormonal factors that encourage the development of inflammation and lead to increased congestion. Consequently, this complex parallel pathophysiological cascade contributes to the echocardiographic and clinical signs of heart failure. In these patients, obesity frequently coexists with nutritional and muscular profile abnormalities that manifest as cachexia or sarcopenia. Patients with heart failure have a higher chance of surviving when obesity is t…

Clinical Review of Hypertensive Acute Heart Failure.

Although acute heart failure (AHF) is a common disease associated with significant symptoms, morbidity and mortality, the diagnosis, risk stratification and treatment of patients with hypertensive acute heart failure (H-AHF) still remain a challenge in modern medicine. Despite great progress in diagnostic and therapeutic modalities, this disease is still accompanied by a high rate of both in-hospital (from 3.8% to 11%) and one-year (from 20% to 36%) mortality. Considering the high rate of rehospitalization (22% to 30% in the first three months), the treatment of this disease represents a major…

[Update: Heart Failure with Preserved Ejection Fraction].

Heart failure with a preserved ejection fraction (HFpEF) is a multifaceted disease entity. Confirming the diagnosis as well as tailoring the most appropriate therapy remains an ongoing challenge. The 2021 heart failure guidelines of the European Society of Cardiology (ESC) suggested a simplified diagnostic approach, and the guideline update provided in 2023 gave recommendations regarding pharmacotherapy based on recent evidence. Further, the Heart Failure Association (HFA) and the European Heart Rhythm Association of the ESC jointly proposed a scheme how to classify HFpEF patients into phenogr…

Hypoxia Attenuates Pressure Overload-Induced Heart Failure.

BACKGROUND: Alveolar hypoxia is protective in the context of cardiovascular and ischemic heart disease; however, the underlying mechanisms are incompletely understood. The present study sought to test the hypothesis that hypoxia is cardioprotective in left ventricular pressure overload (LVPO)-induced heart failure. We furthermore aimed to test that overlapping mechanisms promote cardiac recovery in heart failure patients following left ventricular assist device-mediated mechanical unloading and circulatory support. METHODS AND RESULTS: We established a novel murine model of combined chronic a…

Full decongestion in acute heart failure therapy.

Incomplete decongestion is the main cause of readmission in the early post-discharge period of a hospitalization for acute heart failure. Recent heart failure guidelines have highlighted initiation and rapid up-titration of quadruple therapy with angiotensin receptor neprilysin inhibitor, beta adrenergic receptor blocker, mineralocorticoid receptor antagonist, and sodium glucose cotransporter 2 inhibitor to prevent hospitalizations for heart failure with reduced ejection fraction. However, full decongestion remains the foremost therapeutic goal of hospitalization for heart failure. While early…

Contemporary Decongestive Strategies in Acute Heart Failure.

Congestion is the primary driver of hospital admissions in patients with heart failure and the key determinant of their outcome. Although intravenous loop diuretics remain the predominant agents used in the setting of acute heart failure, the therapeutic response is known to be variable, with a significant subset of patients discharged from the hospital with residual hypervolemia. In this context, urinary sodium excretion has gained attention both as a marker of response to loop diuretics and as a marker of prognosis that may be a useful clinical tool to guide therapy. Several decongestive str…

Heart Failure in Rheumatoid Arthritis: Clinical Implications.

PURPOSE OF REVIEW: This review focuses on the association between RA and heart failure, highlighting the role of inflammation and the prevalence of heart failure with preserved ejection fraction (HFpEF) in this population. RECENT FINDINGS: The incidence of heart failure in RA patients is two to three times higher than in the general population, with inflammation playing a significant role independent of traditional cardiovascular risk factors. HFpEF accounts for about half of heart failure cases and is increasingly recognized in RA patients, although it remains underdiagnosed. Atypical presen…

Acute heart failure: differential diagnosis and treatment.

Acute heart failure is a heterogeneous clinical syndrome and is the first cause of unplanned hospitalization in people >65 years. Patients with heart failure may have different clinical presentations according to clinical history, pre-existing heart disease, and pattern of intravascular congestion. A comprehensive assessment of clinical, echocardiographic, and laboratory data should aid in clinical decision-making and treatment. In some cases, a more accurate evaluation of patient haemodynamics via a pulmonary artery catheter may be necessary to undertake and guide escalation and de-escalation…
