Penetrating left ventricular injuries management: single General Thoracic Center experience.

BACKGROUND: Left ventricular penetrating injuries are rare but often lethal. Transport times greater than 30 min have been shown to increase the injury severity; thus early recognition of cardiac tamponade, prompt pericardial decompression, and control of cardiac hemorrhage are mandatory. METHODS: We retrospectively reviewed our record to include patients with penetrating trauma of the heart. RESULTS: Four hemodynamically unstable young male patients with left ventricular penetrating injury of the heart were referred to our unit between January 2007 and December 2015. Median time from trauma

Neonatal Heart Regeneration: Comprehensive Literature Review.

BACKGROUND: The adult mammalian heart is incapable of meaningful functional recovery after injury, and thus promoting heart regeneration is 1 of the most important therapeutic targets in cardiovascular medicine. In contrast to the adult mammalian heart, the neonatal mammalian heart is capable of regeneration after various types of injury. Since the first report in 2011, a number of groups have reported their findings on neonatal heart regeneration. The current review provides a comprehensive analysis of heart regeneration studies in neonatal mammals conducted to date, outlines lessons learned,

[Ventricular septal rupture after blunt chest trauma].

HISTORY AND CLINICAL FINDINGS: Case 1: a 20-year-old previously healthy man sustained multiple nonvascular injuries without visible chest trauma in a car accident. Four days later a loud systolic murmur was heard over the heart. Case 2: a 21-year-old man similarly sustained in a car accident multiple injuries without visible chest involvement but causing haemorrhagic shock. A loud systolic heart murmur was heard and after shock treatment he developed left heart failure requiring catecholamine infusions. INVESTIGATIONS: Echocardiography demonstrated posttraumatic ventricular septal rupture in

Quantitative Models for Prediction of Cumulative Trauma Disorders Applied to the Maquiladora Industry.

Temperature gradient changes on the surface of the skin or in the middle of the body are signs of a disease. The aim of this study is to develop quantitative models for the prediction of cumulative trauma disorders (CTDs) arising from highly repetitive activities, considering risk factors, such as age, gender, body mass index (BMI), blood pressure (BP), respiratory rate (RR), and heart rate, to prevent injuries in manufacturing factory operators. This research involved 19 individuals from the area of sanding and 14 individuals from the area of tolex in manufacturing factories who had their vit

Stimulation of P2Y11 receptor protects human cardiomyocytes against Hypoxia/Reoxygenation injury and involves PKCε signaling pathway.

Sterile inflammation is a key determinant of myocardial reperfusion injuries. It participates in infarct size determination in acute myocardial infarction and graft rejection following heart transplantation. We previously showed that P2Y11 exerted an immunosuppressive role in human dendritic cells, modulated cardiofibroblasts' response to ischemia/reperfusion in vitro and delayed graft rejection in an allogeneic heterotopic heart transplantation model. We sought to investigate a possible role of P2Y11 in the cellular response of cardiomyocytes to ischemia/reperfusion. We subjected human AC16 c

Urocortin Role in Ischemia Cardioprotection and the Adverse Cardiac Remodeling.

Despite the considerable progress in strategies of myocardial protection, ischemic heart diseases (IHD) and consequent heart failure (HF) remain the main cause of mortality worldwide. Several procedures are used routinely to guarantee the prompt and successful reestablishment of blood flow to preserve the myocardial viability of infarcted hearts from ischemia injuries. However, ischemic heart reperfusion/revascularization triggers additional damages that occur when oxygen-rich blood re-enters the vulnerable myocardial tissue, which is a phenomenon known as ischemia and reperfusion (I/R) syndro

Cardiac macrophages and their functions in homeostasis and injury.

Due to their remarkable plasticity, macrophages can adapt to diverse environments and challenges therein, thereby exerting tissue-specific and context-specific functions. Macrophages are the most frequent immune cell population present in the heart and contribute substantially to cardiac homeostasis and function. Moreover, macrophages are key regulators throughout all stages of heart injury, acquiring diverse phenotypes that can either ameliorate or exacerbate cardiac pathology in a context-dependent manner. The contribution of macrophages to both tissue damage as well as to recovery/tissue re

Shot to the heart: a case of traumatic ventricular septal aneurysm and review of the literature.

Traumatic cardiac injuries are an uncommon diagnosis in the pediatric population. Of the traumatic cardiac injuries, more than 95% are contusions. Traumatic ventricular aneurysms are exceptionally rare, and most are detected during long-term follow-up. They are usually successfully managed medically but can require emergent surgical intervention. We report the case of a 6-year-old boy who was in a motor vehicle collision. He was found to have profound sinus bradycardia during initial resuscitation with hypotension. An electrocardiogram showed complete heart block. A cardiologist was consulted,

The Application of scRNA-Seq in Heart Development and Regeneration.

Single-cell RNA sequencing (scRNA-seq) is a rapidly developing and useful technique for elucidating biological mechanisms and characterizing individual cells. Tens of millions of patients worldwide suffer from heart injuries and other types of heart disease. Neonatal mammalian hearts and certain adult vertebrate species, such as zebrafish, can fully regenerate after myocardial injury. However, the adult mammalian heart is unable to regenerate the damaged myocardium. scRNA-seq provides many new insights into pathological and normal hearts and facilitates our understanding of cellular responses …

Nitric Oxide/Glucose Transporter Type 4 Pathway Mediates Cardioprotection against Ischemia/Reperfusion Injury under Hyperglycemic and Diabetic Conditions in Rats.

INTRODUCTION: The comorbidities of ischemic heart disease (IHD) and diabetes mellitus (DM) compromise the protection of the diabetic heart from ischemia/reperfusion (I/R) injury. We hypothesized that manipulation of reperfusion injury salvage kinase (RISK) and survivor activating factor enhancement (SAFE) pathways might protect the diabetic heart, and intervention of these pathways could be a new avenue for potentially protecting the diabetic heart. METHODS: All hearts were subjected to 30-min ischemia and 30-min reperfusion. During reperfusion, hearts were exposed to molecules proven to prot

Homovanillic acid improves anxiety by regulating F4/80+ microglia/macrophage in adult mice with neonatal cardiac injury.

The heart-brain axis is a significant contributor to morbidity and mortality following ischemic injury. Pathological and psychological changes in the heart can lead to anxiety, cognitive impairment, and brain injury. Cardiac injury releases a substantial number of metabolites into the peripheral circulation, which exacerbates brain damage and dysfunction. Homovanillic acid (HVA), a metabolite of dopamine (DA), plays a crucial role in behavioral symptoms. However, there are limited studies examining the effects of cardiac metabolites released from an injured heart on behavioral symptoms in huma

Sirt5 preserves mitochondrial homeostasis to improve cardiac function in ischemia-reperfusion injury by inhibiting ANT2 lysine lactylation modification.

Myocardial ischemia-reperfusion (IR) injury, a prevalent heart disease, disrupts mitochondrial homeostasis, affecting heart energy metabolism. The role of mitochondrial protein Sirt5 in protecting the heart from IR injury is explored in mice. Reduced Sirt5 levels in IR-injured hearts correlate with increased protein lactylation and impaired mitochondrial function. Sirt5 overexpression mitigates, while its knockout worsens, mitochondrial and cardiac damage. Sirt5 interacts with ANT2, reducing its lysine lactylation to enhance its association with VDAC1. This interaction improves cardiac functio

Peri-mitral ventriculoatrial fistula after blunt thoracic trauma.

Blunt trauma to the chest can damage any thoracic organ. Heart valve damage and particularly mitral apparatus occurs relatively rare, and the patients are expected to become acutely symptomatic. However, in patients with multiple traumas, other dominant severe damages tend to draw away the attention of the physicians resulting in neglect of less salient heart injuries. Here, we present a patient with history of blunt chest trauma and incidental finding of posttraumatic severe mitral regurgitation.

Cardiomyocytes disrupt pyrimidine biosynthesis in nonmyocytes to regulate heart repair.

Various populations of cells are recruited to the heart after cardiac injury, but little is known about whether cardiomyocytes directly regulate heart repair. Using a murine model of ischemic cardiac injury, we demonstrate that cardiomyocytes play a pivotal role in heart repair by regulating nucleotide metabolism and fates of nonmyocytes. Cardiac injury induced the expression of the ectonucleotidase ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1), which hydrolyzes extracellular ATP to form AMP. In response to AMP, cardiomyocytes released adenine and specific ribonucleosides that dis

Catastrophic cardiac injuries encountered during the minimally invasive repair of pectus excavatum.

This paper presents four severe cardiac injuries that occurred in patients who underwent the minimally invasive repair of pectus excavatum (MIRPE). These complications occurred in different clinical settings, namely in a patient with an extremely severe form of pectus, in a patient who had previously undergone an open repair, after a previous open heart surgery, and at the time of bar removal. The purpose of this article is to review the circumstances leading to these cardiac injuries, share what we have learned from these patients, and hopefully help avoid these complications in the future.

Airbag trauma induced cutaneous fistulae in a heart transplant patient.

In recent years, trauma resulting from airbag inflation has been reported to cause skin injuries. These include minor abrasions and lacerations, chemical and thermal burns, and irritant dermatitis. We report what we believe to be the first case of airbag related cutaneous fistulae in a heart transplant patient. We believe that the increasing frequency of placement of subcutaneous and relatively superficial appliances in transplant and other surgical patients will result in an increasing frequency of similar injuries. Dermatologists may be uniquely suited to identify and treat such cutaneous tr

Commotio cordis, revisited.

Nonpenetrating cardiac injuries due to direct precordial blunt impacts are a commonly encountered phenomenon in medicolegal offices. These injuries vary from contusions to valvular lacerations, or papillary muscle rupture to coronary arterial injury with resulting infarction. A less commonly occurring manifestation of nonpenetrating injury is a concussion of the heart (commotio cordis), often with dramatic physiological consequences but no morphologic cardiac injury. We describe four case reports of this entity. The cases were collected over a 5-year period (1978-1983) from the Wayne County Me
