Prolonged Time to Brake Following Lower Extremity Injuries.

INTRODUCTION: No guidelines exist for recommending return to driving. This study will examine time to brake (TTB) after lower extremity injuries versus in uninjured people. The potential effect of various types of lower extremity injuries on TTB will be measured. METHODS: Patients with injuries to the pelvis, hip, femur, knee, tibia, ankle, and foot underwent testing using a driving simulator to assess TTB. Comparison was with a control group of uninjured people. RESULTS: Two-hundred thirty-two patients with lower extremity injuries participated. The majority were in the tibia and ankle regi

An anatomic classification for heterotopic ossification about the hip.

STUDY DESIGN: Retrospective cohort. SUMMARY OF BACKGROUND DATA: Heterotopic ossification (HO) about the hip is a debilitating condition that can occur after fixation for acetabular fractures, total hip replacement, or polytrauma with closed head injuries. No classification exists that informs surgical treatment. PURPOSE: To establish a classification system for HO about the hip by reviewing a consecutive series of HO at a single institution. It was hypothesized that HO about the hip could be grouped into a novel classification scheme based upon the location and involved structures of the hip

Vascular injuries after total joint arthroplasty.

Vascular injuries, although highly feared, can occur after total joint arthroplasty, often resulting in legal suits. This study evaluates the circumstances related to vascular injuries after joint arthroplasty. Using prospectively collected data on 13,517 patients undergoing total joint arthroplasty at our institution, 16 (0.1%) vascular injuries were identified. Eleven injuries occurred after total knee arthroplasty (TKA) and 5 after total hip arthroplasty (THA). Indirect injury was the most common mechanism in TKA. In contrast, direct injury was most prevalent in THA. One patient died of com

Characterization of lower-extremity biomechanics when walking with a hands-free crutch: an integrated experimental and computational analysis.

BACKGROUND: Lower-leg wounds and fractures are some of the most common types of traumatic injuries sustained by U.S. Service members during combat operations. Existing guidelines for on-field management of lower-leg injuries recommend stabilizing the injury and preventing weight-bearing on the affected leg. However, adhering to these recommendations could be challenging because the casualty may need to walk independently to obtain medical care. In this exploratory experimental and computational study, we assessed the biomechanical characteristics of a hands-free crutch device to aid in the mob

Weak Hip Strength Increases Dynamic Knee Valgus in Single-Leg Tasks of Collegiate Female Athletes.

Clinical Scenario: Dynamic knee valgus (DKV) is a mechanical alteration in the knee that leads to increased risk of injury. Weakness of hip musculature in hip abduction (HABD), extension (HEXT), and external rotation (HER) may contribute to increased DKV in single-leg landing tasks. Focused Clinical Question: Is decreased hip strength associated with an increase in DKV during a single-leg landing task in collegiate female athletes? Summary of Key Findings: Three studies were included: One randomized control trial (RCT), one cohort study, and one case-control. All three studies found that decre

Anterior hip dislocation with simultaneous anterior column and anterior wall acetabular fracture: A case report.

INTRODUCTION: Isolated anterior column and anterior wall fractures are a relatively rare subgroup of acetabular fractures. They represent 6,3% of all acetabular fractures. Surgical treatment is indicated for fractures with displacement more than 5 mm and when incongruence of the articular surface and/or instability of the joint is evident, in order to allow early mobilization and prevent further complications, such as posttraumatic hip arthritis. Open reduction and internal fixation is the usual standard of care. Closed reduction and percutaneous fixation can be performed in minimally displace

Femoral Nerve Palsy Following Delayed Reduction of a Dislocated Hip in a 44- Year-old Man.

INTRODUCTION: Incidence of nerve injury in traumatic hip dislocations is up to 10 %. Sciatic nerve is the most common injured nerve in this setting. In the medical literature, there are few documented cases of femoral nerve injury following hip dislocations. CASE REPORT: We report a 44-year-old man with right femoral nerve palsy following delayed reduction of an anterior dislocation of hip. CONCLUSION: Two months after closed reduction, complete clinical recovery of right femoral nerve was achieved and the patient was able to resume his job.

Classification of Kinematic Coupling Patterns Between Lower Limb Joints During Running.

Abnormal foot alignment such flatfoot is involved in running injuries. The mechanism by which flatfoot is susceptible to running injuries has not yet been established. The purpose of this study was to classify kinematic coupling patterns by hierarchical clustering, adding individuals with flatfoot and motion in the transverse plane involved in running injuries, and to identify the factors that determine the clusters classified. Twenty-four young healthy adults were divided and classified into normal and flat foot groups of 12 participants each, using the foot posture index. The task involved t

A case report: periprosthetic acetabulum fracture with combined pelvic ring injury.

Periprosthetic fractures of the acetabulum are largely underreported in the literature, none of which describes the management of such injuries associated with a pelvic ring injury. Our case report discusses our approach to managing a periprosthetic both-column acetabulum fracture associated with an open-book pelvic ring injury. Instead of a revision total hip arthroplasty, we chose to perform an open reduction internal fixation to maintain sufficient bone stock for future revision, if necessary. At 18 months postoperative follow-up, the patient was ambulating independently and had sufficient

Lower limb dysesthesia--an ominous sign in lumbar fractures.

Attempts at diagnosing dural lacerations secondary to fractures of the lumbar vertebrae on plain radiographs has varied considerably. If undiagnosed these injuries can cause serious complications. We present a clinical sign not previously documented to our knowledge. Plain radiographs in this case failed to show significant canal compromise but excessive hip pain, with the limb held in a characteristic posture, heralded cauda equina injury in a burst lumbar fracture. The limb was held in hip and knee flexion, with resistance to passive extension due to lower limb dysesthesia. This sign helped

False aneurysm 14 years after total hip arthroplasty.

Many reports of complications after total hip arthroplasty have been published concerning typical orthopaedic problems. Relatively little attention has been paid to serious vascular injuries, such as the development of a false aneurysm. To our knowledge, 10 cases have been described in the English literature. The mean time interval between surgery and initial manifestation of the aneurysm was 9 months in these cases. Our patient developed first symptoms of a false aneurysm 14 years after surgery. A minimally invasive technique was used to cut the aneurysm from perfusion. Removal of the total h

Traumatic dislocation of the hip.

Analysis of 57 traumatic dislocations of the hip, excluding central dislocations, is reported. No anterior dislocations occurred. Road accidents caused most of the dislocations and several patients, most of them young men, had additional serious injuries. Grading the dislocations according to Stewart and Milford (15) is useful for the prognosis, and should to a greater extent be used as a therapeutic guide. Post-traumatic osteoarthritis of the hip (7) and avascular necrosis of the head of femur were seen in 9.1 and 5.5% of the patients, respectively. The clinical results were excellent for 83.

Femoral nerve palsy. An unusual complication of anterior lumbar interbody fusion.

SUMMARY OF BACKGROUND DATA: Compression neuropathy of the femoral nerve has been reported as an uncommon complication of bleeding into the iliopsoas muscle. OBJECTIVE: The authors detected anatomic reasons of direct injury to the femoral nerve at the lower lumbar level. METHODS: Keeping the hip in extension during the course of carrying out anterior fusion on a previously failed posterior fusion was considered another causative factor of femoral nerve injury. Anatomical dissection confirmed the likelihood of this injury being produced in this situation. RESULTS: Femoral nerve traction and c

Acute total hip arthroplasty for anterior hip fracture-dislocation: a case report.

Hip fracture-dislocations accompanied by femoral head or acetabular fractures remain challenging, and the optimal treatment strategy is still controversial. Anterior hip fracture-dislocations are particularly rare, and reports describing acute total hip arthroplasty (THA) for this injury pattern are limited. We report the case of a 74-year-old active woman who sustained an anterior hip fracture-dislocation with femoral head and acetabular anterior wall fractures after a low-energy fall. The injury was classified as Pipkin type IV and Brumback type 4B. Following closed reduction, acute THA was

Bilateral hip and femur fractures.

Bilateral hip and femoral fractures are rare. A 26-year-old male with multiple injuries including such fractures was successfully treated with a variety of internal fixation techniques, allowing early mobilization. It is important to consider the possibility of this fracture pattern in high-energy trauma.

Combined injury of femoral artery and femoral nerve after blunt trauma to the proximal femur: A case report.

INTRODUCTION: Combined femoral arterial and nerve injury does not often occur in cases of proximal femur fracture (hip fracture) and is often overlooked in the emergency medical setting. Physicians should be aware of this rare but possible combination of injuries, which can lead to devastating and disabling patient outcomes. PRESENTATION OF CASE: A 42-year-old Ethiopian male was struck by a steel pipe, rushed to the emergency room, and diagnosed with a left subtrochanteric fracture of the femur. Although promptly taken to surgery for fixation and exploration of the femoral artery, it became n

Range of Motion Adaptations During a Simulated Game Exposure in Softball Pitchers.

Hip and shoulder range of motion (ROM) alterations are correlated with increased risk of injury in softball athletes. The purpose of this study was to investigate bilateral shoulder and hip ROM adaptation during a simulated softball double-header exposure. It was hypothesized that shoulder and hip ROM would decrease after simulated game exposure and would not return to baseline following a 30-minute break. Thirty (14.8±1.9 yrs, 162.5±18.3 cm, 71.79±16.03 kg) adolescent softball pitchers participated. Testing included: bilateral hip and shoulder ROM (preSG), simulated game exposure (100 pitches

Reduction of Displaced Acetabular Fracture with Central Hip Dislocation using Vector Traction: A Case Report.

Fracture-dislocations of the hip is the result of high-energy trauma which necessitates urgent reduction. Closed reduction is usually attempted first and if failed, open reduction is indicated and may require more than one surgical approach. However, there is also the option of managing it with vector traction. This case report details the treatment of a middle-aged gentleman who sustained a left hip central dislocation which was gradually reduced with vector traction prior to surgery and in doing so, diminished the risk of him developing several potentially debilitating complications known to
