Impaired oral health: a required companion of bacterial aspiration pneumonia.

Laryngotracheal aspiration has a widely-held reputation as a primary cause of lower respiratory infections, such as pneumonia, and is a major concern of care providers of the seriously ill orelderly frail patient. Laryngeal mechanical inefficiency resulting in aspiration into the lower respiratory tract, by itself, is not the cause of pneumonia. It is but one of several factors that must be present simultaneously for pneumonia to develop. Aspiration of oral and gastric contentsoccurs often in healthy people of all ages and without significant pulmonary consequences. Inthe seriously ill or elde

Endovascular embolization of a congenital inferior phrenic artery-to-pulmonary arteriovenous malformation: a rare case report.

BACKGROUND: Pulmonary arteriovenous malformation (PAVM) is abnormal arteriovenous shunts between pulmonary artery (PA) and pulmonary vein, and rarely has congenital direct communications with systemic arteries. CASE PRESENTATION: A 33-year-old male presented to our hospital with intermittent bloody sputum with no evidence of pulmonary infection, trauma or surgery. Chest computed tomography angiography (CTA) indicated the congenital inferior phrenic artery (IPA)-to-PAVM surrounded by diffuse alveolar hemorrhage located in the lower lobe of right lung. Both the afferent PA and IPA were successf

The "heart brain" and neuromodulation for vasovagal syncope.

It is well known that the autonomic nervous system (ANS) is a major contributor in etiopathogenesis of vasovagal syncope (VVS). Catheter based neuromodulation (CNA) of the intrinsic cardiac ANS has evolved rapidly from being an experimental unproven procedure to its current status as an increasingly performed ablation procedure in many major hospitals worldwide. The present review aims to bring the anatomical elements of intrinsic cardiac ANS and clinical application of intrinsic cardiac neuromodulation together, by reviewing anatomical terminologies and clinical data, in order to provide a pr

The Hospital Course of a Successfully Treated Patient with Respiratory Failure: Beginning to End!

The successful treatment of a patient with acute respiratory failure is a complex undertaking that requires clinical competence, evidence-based interventions, seamless coordination of care transitions, and transparent open communication among all members of the health care team. Many of the processes of care in these critically ill patients are reassuringly consistent across services, across hospitals, across health systems, and even across the country. Although the clinical course of such complicated patients can be extremely unpredictable, we are fortunate that the professional, technical, a

Equipping an urban hospital police and security team with Narcan.

Narcan, the nasal-spray form of naloxone, has been approved by the FDA as an easy-to-use version of a drug for saving lives of people who have overdosed on opioids--heroin or prescription painkillers. Communities across the US have been equipping first responders and police with the spray. Now, as the increase in overdose deaths has spread to hospitals, those facilities will have to decide whether their police/security officers should be equipped with. naloxone and trained in its use. In this article, the authors relate their health system's decision, how it was reached, and how it has been im

Nosocomial pneumonia: lessons learned.

Nosocomial pneumonia remains a significant cause of hospital-acquired infection, imposing substantial economic burden on the health care system worldwide. Various preventive strategies have been increasingly used to prevent the development of pneumonia. It is now recognized that patients with health care-associated pneumonia are a heterogeneous population and that not all are at risk for infection with nosocomial pneumonia pathogens, with some being infected with the same organisms as in community-acquired pneumonia. This review discusses the risk factors for nosocomial pneumonia, controversie

Concomitant diagnosis of primary Sjögren's syndrome and systemic AL amyloidosis.

A 48 year old woman was referred to hospital for buccal discomfort. Physical examination showed a macroglossia and features of xerostomia. She was diagnosed as having primary Sjögren's syndrome according to the criteria proposed by the European Community study group in 1993. Furthermore, a lower lip salivary gland biopsy showed amyloid deposits that were also seen in the stomach and in the bone marrow. Echocardiography was consistent with cardiac amyloidosis. Serum immunofixation identified a monoclonal IgGlambda. As far as is known, this is the first report of systemic primary amyloidosis ass

The most common deviations in the development of hand motoricity in children from birth to one year of age.

The early child development, from birth until the age of one year is, amongst other changes, characterized by intense motor learning. During that period, the voluntary learning patterns evolve from reflexive patterns to coordinated voluntary patterns. All of the child's voluntary movements present active forms in which the child communicates with the environment. In this communication, the hand plays an important role. Its brain representation covers one-third of the entire motor region, situated in the close proximity to the speech region. For this reason, some authors refer to hand as a "spe

Acinetobacter baumannii: no more a choosy intruder?

CONTEXT: Acinetobacter baumannii, a non-fermenter, is fast coming up the list of pathogens causing nosocomial infections. Earlier considered to be a harmless colonizer, or at the most, a pathogen causing mostly respiratory tract infections, it has slowly but successfully emerged as a ubiquitous pathogen causing both community as well as health care associated infections. It has acquired multidrug resistance, and seems to be no more selective in mainly attacking any one or two body systems. According to recent reports, a shy selective bacterium has turned into a dangerous pathogen, capable of c

Deja Tu Huella: a comprehensive review of a successful emergency department-based, HIV-targeted screening program in Spain and its impact on early diagnosis and public health.

Failure to diagnose human immunodeficiency virus (HIV) and late HIV diagnosis persist across Europe and Spain, sustained by missed diagnostic opportunities in routine care. Emergency departments (EDs), often the sole point of contact for vulnerable groups, are pivotal yet underused venues for early case finding. In response, the Spanish Society of Emergency Medicine (SEMES), with scientific sponsorship of the AIDS Study Group (GESIDA), published a consensus document recommending a targeted, opt-in HIV testing strategy in EDs triggered by specific HIV indicator conditions. This first consensus
