Between 1980 and 1997, 27 patients underwent shoulder arthroplasty because of pain or functional impairment due to a proximal humeral nonuni … of the humeral surgical neck with failed internal fixation, severe osteoporosis, cavitation of the humeral head, or secondary osteoarthritis may benefit from shoulder arthroplasty. Although function is not completely restored, pain relief and high levels of subjective satisfaction can be achieved.

OBJECTIVE: To systematically identify and evaluate different bone graft augmentation techniques in the operative treatment of complex proximal humerus fractures.
METHODS: Four databases were searched from 1970 to February 2023 for Level I to IV English studies that investigated outcomes of different bone augments in the primary surgical fixation of proximal humerus fractures. The JBI critical appraisal checklist, methodological index for non-randomized studies and cochrane risk of bias tool were used to assess study quality. Descriptive statistics including weighted means are presented where

Iodine densities of the humeral head and the glenoid (control) were measured on CT. Subjects managed with open reduction internal fixation or conservatively (non-surgical) were followed for up to two years for radiographic evidence of avascular necrosis. A 46-year-old female with Sjögren's syndrome previously treated with corticosteroids was referred to our department for suspicious humeral head osteonecrosis. Dual-phase bone scan showed an increased radiotracer distribution in the head of the left humerus. Nevertheless, whole-body scan revealed multiple sites of heterogeneous skeletal uptake. As lymphoproliferative disorder was hypothesized, also based on laboratory examination, F-18 fluorodeoxyglucose positron-emission tomography/computed tomography was performed and showed increased uptake in several osseous structures and in the subcutaneo…

PURPOSE: This study compared the clinical and radiological results of bone marrow mesenchymal stem cell implantation with traditional simple core decompression (CD) using a matched pair case-control design for osteonecrosis of the humeral head (ONHH) after fracture of the proximal humerus.
PATIENTS: We retrospectively reviewed 64 patients who had surgery for ONHH. Thirty patients had been treated with cell therapy between 2010 and October 2015, with 18 patients at pre-collapse stage (8 stages-I, 10 stages-II), and 12 patients at post-collapse stages (7 stages-III and 5 stages-IV). Aim To explore a more effective surgical procedure, the outcomes of closed manipulative reduction (CMR) combined with minimally invasive pla … ry bone fracture reduction in both groups. Meanwhile, in the ORIF group, nonunion (three cases) and humeral head necrosis (four cases) were detected. Conclusions The MR/MIPO technique showed smaller incisions, easier operation, less blood transfusion and more effective recovery of shoulder joint function for treating proximal humeral fractures than ORIF.

A 52-year-old man incurred a brachial plexus injury with a two-part anterior fracture dislocation of the shoulder. The fracture was converted to a four-part fracture dislocation during attempts to reduce the initial lesion in a closed fashion. The four-part fracture dislocation was treated with a Neer humeral head replacement. Postoperatively, the prosthesis gradually dislocated inferiorly and relocated spontaneously with the recovery of the brachial plexus lesion.

PURPOSE: To identify characteristics associated with loss of reduction following open reduction and locked plate fixation (ORIF) of proximal humerus fractures in older adults and determine if loss of reduction affects patient reported outcomes (PROs), range of motion (ROM), and complication rates during the first postoperative year.
METHODS: Patients >55 years old who underwent proximal humerus ORIF were reviewed. Patient and fracture characteristics were recorded. Fixation characteristics were measured on the initial postoperative AP radiograph including humeral head height (HHH) relative to…

This case report identifies a traumatic laceration of the long head of the biceps brachii associated with a displaced surgical neck fracture of the humerus in a 37-year-old woman who sustained a fall while skiing. At the time of surgery, the tendon was found sharply divided and repaired primarily. The fracture was fixed with a proximal humeral blade plate. Followup revealed nearly full glenohumeral motion and a functional biceps muscle by 4 months.

A 54-year-old male with a 10-year history of seropositive rheumatoid arthritis subsequently developed generalized osteoporosis, multiple cervical spine subluxation, vertebral collapses, and massive osteolysis of the humeral and femoral heads and carpal bones. These lesions were accompanied by neurologic manifestations in both the upper and lower extremities. Bone matrix collagen and noncollagenous proteins in this rare syndrome were analyzed. The pathogenesis of this rare phenomenon is discussed.

Most proximal humeral fractures affect elderly patients and can be treated nonoperatively with good functional outcomes. The treatment of displaced three and four-part fractures remains controversial and depends on a variety of underlying factors related to the patient (e.g., comorbidity, functional demand), the fracture (e.g., osteoporosis), and the surgeon (e.g., experience).Throughout the literature, open reduction and locking plate osteosynthesis is associated with considerable complication rates, particularly in the presence of osteoporosis.Low local bone mineral density, humeral head isch…

BACKGROUND: Paediatric elbow injuries account for a large proportion of childrens' fractures. Knowledge of common injuries is essential to understanding their assessment and correct management.
METHODS: A selective literature search was performed and personal surgical experiences are reported.
RESULTS: We have described the assessment and management of the five most common paediatric elbow injuries: supracondylar humeral fractures; lateral condyle fractures; medial epicondyle fractures; radial head and neck fractures; radial head subluxation.
CONCLUSION: Understanding of the ossification ce

Superior capsular reconstruction remains a controversial procedure, and long-term results still need to be established. Graft thickness seems crucial and fascia lata appears superior to dermal allograft. Using a subacromial spacer by suturing dermal allograft to the subacromial bone "increases graft thickness" and reduces superior humeral head migration but increases subacromial pressures in a static laboratory cadaver model. It remains to be seen whether clinical studies support this concept.
