Multifactorial causes of severe persistent hypercalcemia in an immunosuppressed woman with chronic kidney disease.

We present the case of a 62-year-old woman with history of chronic kidney disease and birdshot retinochoroidopathy on immune suppression who presented with hypercalcemia, acute kidney injury, and Pneumocystis jirovecii pneumonia (PJP). Despite initial treatment with intravenous (IV) fluids, calcitonin, zoledronic acid, and antimicrobials for her pneumonia, her hypercalcemia persisted. Given her diagnosis of PJP and markedly elevated levels of 25-hydroxyvitamin D, 1,25-dihydroxyvitamin D, and vitamin A, prednisone (40 mg/day) and IV denosumab (120 mg) were added to her regimen; however, there w…

Clinical and epidemiologic characteristics of hospitalized oncological patients with hypercalcemia: a longitudinal, multicenter study.

BackgroundThere are few studies that have analyzed the characteristics of hypercalcemia in hospitalized oncological patients. Our objectives were to describe the clinical characteristics of hospitalized patients with paraneoplastic hypercalcemia and to identify prognostic variables for mortality.MethodsThis was an observational, longitudinal, retrospective, and bicentric study. It included adult patients admitted to two hospitals in Málaga, Spain (2014-2018). The minimum follow-up period was 2 years or until death.ResultsA total of 154 patients were included; the majority (71.4%) were admitted…

Sinus bradycardia as an atypical presentation of severe hypercalcemia: a case report and a literature review.

INTRODUCTION AND IMPORTANCE: Severe hypercalcemia typically produces QT interval shortening or ventricular arrhythmias, whereas bradyarrhythmias are exceptionally rare. The coexistence of hypercalcemia with acute kidney injury (AKI) - a condition that usually causes hypocalcemia - further underscores diagnostic complexity. CASE PRESENTATION: An 85-year-old man presented with AKI, confusion, and symptomatic sinus bradycardia (40 bpm) without atrioventricular block or QT prolongation. Laboratory studies showed markedly elevated corrected calcium, suppressed parathyroid hormone, and increased pa…

Hypercalcemia after denosumab discontinuation in patients with osteoporosis: a systematic review and case report.

UNLABELLED: Discontinuation of denosumab may result in rebound bone resorption, fractures, and rarely hypercalcemia, a very uncommon manifestation. Our systematic review and case report highlight that although rare, calcium disturbances occur after treatment withdrawal. These findings emphasize the importance of sequential antiresorptive therapy and close monitoring in osteoporosis patients. BACKGROUND: Denosumab discontinuation has been associated with rebound bone resorption, rapid bone loss, and vertebral fractures. Hypercalcemia remains exceptional in postmenopausal osteoporosis. METHODS…

Hypercalcemia associated with kidney disease: it is not always what you think-a case report.

BACKGROUND: Hypercalcemia is a disorder resulting either from impaired calcium renal reabsorption, excessive bone resorption or intestinal absorption, or a combination of these conditions. It can lead to both acute kidney injury and chronic kidney disease, which may develop in case of prolonged or recurrent hypercalcemia. Therefore, the coexistence of hypercalcemia and kidney damage requires prompt recognition of predisposing factors. CASE REPORTS: Case 1 shows a 63-year-old white male with renal impairment and hypercalcemia who presented with weight loss, general malaise, and muscle pain. Re…

Unexpected severe hypercalcemia in a 6-year-old child with hypoparathyroidism and feeding difficulties.

Treatment of hypoparathyroidism with activated vitamin D and calcium carbonate is associated with increased risk of hypercalcemia, hypercalciuria, and nephrocalcinosis. These complications are usually due to inadvertent overdose of medications. We present a case of a 6-year-old boy with hypoparathyroidism, sensorineural deafness, and renal dysplasia (HDR) syndrome and feeding difficulties who developed severe hypercalcemia twice, which was caused not just by medication. Both episodes of calcium overload were related to excessive dietary calcium intake from enteral formula. After rehydration an…

Persistent and recurrent hypercalcemia are uncommon in dogs undergoing surgical treatment for primary hyperparathyroidism.

OBJECTIVE: To report the proportion of cases, causes, and definitions of recurrent and persistent hypercalcemia in dogs following surgical removal of autonomously functioning parathyroid tissue. METHODS: Retrospective review of medical records from 3 academic veterinary hospitals (2012 to 2022) identified dogs with surgically treated primary hyperparathyroidism. Data included signalment, clinical signs, diagnostics, treatment, histopathology, and outcomes. Dogs were categorized by follow-up: ≤ 6 months (group 1) or > 6 months (group 2). Descriptive statistics were performed. The Mann-Whitney …

Too Much of a Good Thing: Severe Hypercalcemia Presenting with Lethargy and Kidney Failure.

We present a case of a 58-year-old man with a history of laryngo-pharyngectomy including bilateral thyroidectomy due to hypopharyngeal cancer presenting with lethargy, acute kidney failure, and hypercalcemia. Milk alkali syndrome was diagnosed given the history of high-dose calcium / vitamin D supplementation after ruling out other causes of hypercalcemia. After initial treatment with normal saline, furosemide and denosumab, the patient developed severe symptomatic hypocalcemia as a rare adverse effect of denosumab.

Hypercalcemia Associated With Severe Dysphagia in a Patient With Parathyroid Adenoma and Significant Cervical Spondylophytes.

BACKGROUND/OBJECTIVE: Severe dysphagia is a rare presenting symptom of primary hyperparathyroidism, whereas the most common hypercalcemia-related causes include gastrointestinal symptoms, such as anorexia, constipation, and pancreatitis. This case presentation aimed to describe swallowing difficulty as a leading symptom of hypercalcemia. CASE REPORT: A 62-year-old man experienced vomiting, dysphagia bordering with aphagia, and 20-kg weight loss in a 2-month period. The parathyroid hormone and serum calcium levels were 102 pmol/L (reference range, 1.8-7.9 pmol/L) and 4.12 mmol/L (reference ran…

The Use of Steroids to Treat Hypercalcemia Due to Granulomatous Disease From Disseminated Coccidioidomycosis.

The clinical course and treatment of hypercalcemia from a granulomatous disease in the setting of an infectious etiology, namely disseminated coccidioidomycosis, remains incompletely understood. The mechanism and treatment of hypercalcemia have been documented in most granulomatous disorders, with sarcoidosis being the most well-understood so far. We discuss a case of a patient with a recent diagnosis of disseminated coccidioidomycosis who presented with hypercalcemia despite adequate infection control. The treatment course involved combinatorial-calcitonin, low-dose bisphosphonates, and corti…

Efficacy and Safety of Intravenous Pamidronate for Parathyroid Hormone-dependent Hypercalcemia in Hospitalized Patients.

CONTEXT: Bisphosphonates are effective for hypercalcemia of malignancy (HOM). Efficacy and safety data for bisphosphonates in parathyroid hormone-related hypercalcemia (PTHRH) are rare, including pamidronate (Pam), which is not indicated for this condition. OBJECTIVE: This work aims to evaluate the efficacy and safety of Pam for moderate-to-severe PTHRH. METHODS: This retrospective case-control study was conducted at a tertiary care medical center. Patients included adults hospitalized with serum calcium levels greater than 12 mg/dL, from October 29, 2013 to December 17, 2019. Etiology was c…

Unique case of profound iatrogenic hypercalcemia in a patient with recent orthopedic prosthetic infection.

Hypercalcemia is a common electrolyte disorder and is typically caused by parathyroid-dependent and parathyroid-independent causes. The most common parathyroid-independent causes include malignancy, granulomatous diseases, over-supplementation with calcium, and hypervitaminosis D. We present an unusual case of a woman who had Stimulan implanted after an artificial knee joint infection. When a washout was done, the patient's serum calcium started rising, peaking at an astounding 21.2 mg/dL (normal range 8.4 - 10.2 mg/dL) with acute kidney injury. After aggressive hydration and treatment with fu…

Prevalence and clinical significance of hypercalcemia at diagnosis in diffuse large B-cell lymphoma.

The reported prevalence of hypercalcemia at diagnosis in non-Hodgkin-lymphoma ranges between 1.3% and 7.4%. These studies included all patients, regardless of lymphoma subtype. We performed a retrospective case-control study to determine the prevalence of hypercalcemia at time of diagnosis in patients with diffuse large B-cell lymphoma (DLBCL). Among 250 newly diagnosed patients, 46 (18%) had hypercalcemia. When compared with age-sex matched patients and normal calcium levels, those with hypercalcemia had higher levels of LDH, lower levels of albumin and more advanced stage. These differences …

Hypercalcemia associated with squamous cell carcinoma of renal pelvis: A case and review of the literature.

Renal pelvis squamous cell carcinoma (RSCC) is a rare tumor. It starts with nonspecific symptoms and it is usually at an advanced stage with a poor prognosis at the time of diagnosis. SCC-associated hypercalcemia is a well-known paraneoplastic syndrome; however RSCC-associated hypercalcemia is a rare condition. Our patient is a 57-year-old-male patient with no bone metastases. Based on the literature screening on PubMed Database for paraneoplastic malignant hypercalcemia-associated RSCC, we found a few cases.

Severe hypercalcemia preceding a diagnosis of Pneumocystis jirovecii pneumonia in a liver transplant recipient.

BACKGROUND: Incidence of the opportunistic infection Pneumocystis jirovecii pneumonia (PJP) in solid organ transplant patients ranges from 5 to 15% with a mortality of up to 38%. CASE PRESENTATION: We present a liver transplant recipient who developed hypoxemic respiratory failure related to PJP soon after treatment for allograft rejection. His presentation was preceded by severe hypercalcemia of 14.6 mg/dL and an ionized calcium of 1.7 mmol/L which remained elevated despite usual medical management and eventually required renal replacement therapy. As approximately 5% of PJP cases have granu…
