The possible contribution of hypervitaminosis A to bone disease in uremia was examined in 50 dialysis-treated patients with end-stage chroni … or the vitamin A/RBP ratio, and the measured biochemical, radiographic, or histological indices of hyperparathyroidism and bone resorption. We conclude that the elevated plasma values of vitamin A in uremia are largely attributable to the high concentrations of RBP and do not contribute significantly to the pathogenesis of renal osteodystrophy.

A case of osteoporosis secondary to primary hyperparathyroidism is reported. A 55-year-old woman presented with a history of persistent lumbar backache for 3 years; numerous radiographs taken during this period had shown 'osteoporosis in keeping with age'. Referral to the Endocrine Clinic to evaluate the osteoporosis resulted in baseline investigations which revealed a raised serum calcium level, further investigation of which led to the diagnosis of primary hyperparathyroidism. Recent studies have shown that, over the past two decades, diffuse undermineralization of the bones (osteopenia) is …

Between August 1975 and January 1981, 106 patients thought to have persistent or recurrent hyperparathyroidism underwent a total of 108 parathyroid re-explorations at the National Institutes of Health. These 106 patients had a total of 175 previous operations for hyperparathyroidism (156 cervical and 19 mediastinal). Nephrolithiasis (54% of patients) and bone disease (24% o patients) were the predominant symptoms. Arteriographic examination and selective venous sampling provided highly accurate localizing results in 33% of the patients, and were of some help in 64%. The final diagnoses after r…

Patients with hyperparathyroidism of renal failure fall into two categories: those with hypocalcemia and those with hypercalcemia. If medical management fails and operative indications are present--bone pain or fracture, metastatic calcification, progressive hypercalcemia or uncontrolled pruritus--parathyroid exploration should be done. Total parathyroidectomy and autotransplantation is the procedure of choice when hypocalcemia is present and more than one gland (usually all) is enlarged (which is the case in most patients). Parathyroid adenomectomy is the procedure of choice where autonomy of…

In a woman with bilateral recurrent calcium nephrolithiasis and hypercalciuria, hypercalcemia developed and she underwent parathyroid surger … cretion. In calcium nephrolithiasis, persistent hypercalciuria may lead to recurrence of autonomous hyperparathyroidism.

Three of seven Arctic fox kits, Alopex lagopus, trapped on St. Paul Island, Alaska, had evidence of rickets and hyperparathyroidism. Radiographic, morphologic and histologic examination confirmed the diagnosis. The disease was presumed to be a juvenile-onset disease due to calcium-deficient intake following weaning. The possibility of insufficient exposure to sunlight could not be determined. No other abnormalities (e.g., genitourinary) were found. Skeletal involvement was quite varied. All growth plates exhibited columnar hyperplasia compatible with rickets, while the metaphyses showed decrea…

In an 18-year-old boy with Bartter's syndrome, hypophosphatemia was discovered (2.4 mg/100 ml) with normal serum calcium concentration (9.7 … inary cyclic AMP excretion. These data suggest that hypophosphatemia can be attributed to secondary hyperparathyroidism in the patient with Bartter's syndrome. Hypokalemia does not impair the renal activity of parathyroid hormone.

In a three-year experience with 361 patients who underwent exploratory surgery for hyperparathyroidism, the success rate for primary cervical operations was 95% and that for secondary operations (cervical and mediastinal) was 62%; for all operations, it was 94%. The three most important factors in determining successful cervical exploration of the parathyroid glands are correct preoperative diagnosis, meticulous surgical technique, and accurate determination of abnormal locations of pathologic conditions. Ancillary studies or techniques for preoperative localization of parathyroid tissue were …

1. GASTRIC function was studied pre-operatively in 55 patients with primary hyperparathyroidism. In all patients the diagnosis of hyperparathyroidism was proved by the removal of an enlarged parathyroid gland or glands, following which the serum calcium fell to normal or below. 2. Twenty patients (38 per cent) gave a history of dyspepsia. All patients were given a barium meal and 12 (22 per cent) were found to have a duodenal ulcer. Male patients had dyspepsia and/or an ulcer more frequently than female patients. 3. Kay's augmented histamine test was carried out in all patients. Six patients (…

Parathyroid glands from cases of hyperparathyroidism have been examined with particular attention to mast cells. In ;normal' glands accompanying an adenoma they were common, although there was a wide range in their frequency, whereas in pathological tissue, both adenoma and hyperplasia, mast cells were scarce. This difference between glands was examined with respect to alterations in gland size, to variations in the amount of interstitial tissue, to degrees of overactivity, and to anatomical development of the glands. No conclusions can be drawn with regard to the significance of mast cells in…

An experience with 8 families with 56 members affected with hyperparathyroidism has emphasized that heredity is an important etiologic factor in this disease. The finding of other endocrine involvement in several members of these families has suggested that hereditary hyperparathyroidism is only a part of the autosomal dominantly inherited multiple endocrine adenomatosis syndrome. This association with hyperparathyroidism, a disease being found with increasing frequency as an inherited condition, provides evidence that multiple endocrine adenomatosis is more common than previously suspected.
