We report characteristic magnetic resonance (MR) image findings in a case of Sheehan's syndrome. A 37-year-old woman experienced complications of retained placenta and massive bleeding (3600 g) during delivery of a full-term baby. A pituitary function test demonstrated panhypopituitarism. We used immunohistochemistry to identify cells present in pituitary allografts in the hamster. Hypophyses removed from neonatal hamsters or adenohypophyses removed from adult females were placed beneath renal capsules of hypophysectomized adult females. Serum PRL, LH, and GH concentrations were measured at two, five and eight weeks after placement of allografts. A 16-year-old male complained of a headache and a high fever followed by polyuria. The endocrinological studies showed he had hypopituitarism and central diabetes insipidus, and magnetic resonance imaging (MRI) revealed a pituitary mass. Post-partum pituitary necrosis (Sheehan's syndrome) is a rare complication of post-partum hemorrhage. The diagnosis can be erratic and often delayed. In this case report of Sheehan's syndrome in the post-partum period, the signs were characterized by agalactia, severe hypoglycemia, and low serum levels of thyroid hormones, cortico-adrenal hormones, and gonadotrophin (FSH, LH). The hypophyseal magnetic resonance imaging confirmed the diagnosis of hypopituitarism secondary to pituitary necrosis.

Cases of sellar involvement of neurocysticercosis (NCC) are rare. Little is known about the mechanisms by which the parasite can compromise the pituitary gland. Although NCC damages sellar structures with direct compression by large cysts, extension through the basal cisterns and third ventricle with focal arachnoiditis can result as an inflammatory response. Evaluation for hypophyseal lesions in patients with NCC may allow for the diagnosis of unexplained loss of visual acuity and hormonal disturbances.

The analysis of correlations between levels of epiphyseal, hypophyseal, adrenocortical and gonadal hormones suggested functional disorders developing in these glands to be interdependent and account for dysfunction of the thymus and immune system in patients suffering malignant melanoma. The degree of relationship between pituitary-adrenal system and thymus function disorders increased with tumor size; it was more pronounced in males. The rationale for application of pineal gland preparations in melanoma patients aimed at combatting neuroendocrine disorders is discussed.

A case of ectopic pinealoma is reported. It presented with the clinical triad of visual disturbances of the chiasmatic type, diabetes insipidus, hypothalamic-hypophyseal impairment, and with enlargement of the sella turcica on roentgenographic examination. The patient is alive and well eight years after incomplete removal followed by radiation treatment. It is felt that this is the correct treatment for such lesions, rather than radiation treatment alone as apparently favoured in other reports. The diagnostic and therapeutic points supporting such an opinion are discussed.

Septo-optic dysplasia (SOD) is a rare congenital condition characterized by a triad of septum pellucidum dysgenesis, optic nerve hypoplasia, and hypothalamic-hypophyseal dysfunction. In some cases, additional brain anomalies such as schizencephaly can occur, leading to a more complex presentation known as septo-optic dysplasia plus (SOD+). This case report describes a 2-year-old boy presenting with delayed psychomotor development and visual impairment, ultimately diagnosed with SOD+.

The authors present a rare case of the longitudinal fracture of the clival complex. Patient in good clinical condition has had fracture of frontal bone, planum sphenoideale, hypophyseal bossa and longitudinal fracture oc clivus in medial plane. Trauma was associated with cerebrospinal fluid leakage. There was performed reconstruction of anterior fossa dura vater and sealing of sphenoid sinus by muscle grafting in two step operation. The only one postoperative complication was temporary diabetes insipidus.

The level of pituitary prolactin reserve (PPR) was studied in 56 patients with chronic respiratory failure (CRF) in the acute stage after injections of metoclopramide and Thyrotropin Releasing Hormone (TRH). PPR was low in 90% of the subjects in both groups so that the pathogenic mechanism is more likely to be hypophyseal than hypothalamic and due to the reduced synthesis of PRL by the pituitary gland as a result of inadequate ATP synthesis and/or functional alterations to the receptors or pituitary cells following hypoxaemia and hypercapnia.
