Controlled human pneumococcal infection in the Netherlands: Colonisation, antibody responses and the impact of viral co-infection.

OBJECTIVES: Colonisation of the upper airways by Streptococcus pneumoniae is a prerequisite for disease development and shapes immune responses that vary between geographical settings. This first controlled human pneumococcal infection study in the Netherlands assessed colonisation, antibody responses, respiratory symptoms, and concurrent viral infections. METHODS: Twenty healthy adults (19-47 years) were enrolled between May 2022 and February 2023 and inoculated intranasally with 160,000 colony forming units of S. pneumoniae serotype 6B (strain BHN418). Participants were monitored for 28 day

Intranasal monoclonal antibodies do not prevent respiratory infection in a randomized, controlled experimental infection trial.

Intranasal monoclonal antibodies (mAbs) offer a promising cost-reducing and non-invasive alternative to systemic administration. Intranasal palivizumab prevented respiratory syncytial virus (RSV) infection in mice but not in healthy infants. This proof-of-concept study evaluates the efficacy of intranasal palivizumab to prevent RSV infection. In this randomized, double-blind, placebo-controlled, experimental infection model, 28 healthy adults (18-55 years old) were randomized 1:1 to receive 1 mg/mL palivizumab in saline nasal drops or placebo one hour prior to RSV-A Memphis 37b challenge (EU c

Viral infection states, immune control, and adult cognition: evidence across latency, persistence, and post-acute responses.

Viral infections can leave latent genomes, persistent reservoirs, or prolonged host responses after the presenting illness has resolved. This Review organizes their relevance to adult cognition around three overlapping infection-state prototypes: latency with episodic reactivation, chronic persistence, and acute infection with post-acute host responses. We examine how local antiviral surveillance, antigen-driven T-cell dysfunction, innate immune memory, and inflammatory resolution shape neurovascular and glial responses. Complement-mediated synaptic remodeling, proteostatic disruption, and imm

Phase-specific polarization of peripheral helper T cells influences immunopathology and viral control in HBV infection.

BACKGROUND: CD4+ T cells orchestrate antiviral immunity but can also drive immunopathology in hepatitis B virus (HBV) infection. Peripheral helper T (Tph) cells are implicated in extra-lymphoid inflammation, yet their functional roles during HBV infection remain elusive. METHODS: We utilized single-cell RNA sequencing (scRNA-seq) to analyze intrahepatic and blood CD4+ T cells from HBV-infected patients at various disease phases, including immune tolerant (IT), immune activation (IA), acute resolving (AR), and chronic resolved (CR). Multicolor immunohistochemistry (mIHC), single-cell multiplex

Differential roles of resident lung macrophages during control of murine alveolar and airway Mycobacterium abscessus infection.

INTRODUCTION: Mycobacterium abscessus (Mabsc), a nontuberculous mycobacterium (NTM), is readily cleared from healthy lungs but can cause infections in immunocompromised individuals and individuals with chronic airways diseases that disrupt mucociliary clearance, such as cystic fibrosis and bronchiectasis. In bronchiectatic airways, Mabsc can persist despite robust immune cell recruitment, raising the possibility that, in addition to impaired mucociliary clearance, local pulmonary immune defects contribute to NTM susceptibility. Since chronic infections result from failed eradication of acute i
