[Side effects of phlebotomy: pathophysiology, diagnosis, treatment and prophylaxis].

Although phlebotomy is on the whole a safe procedure, the frequency of side effects being low and their severity weak, there have still been rare incidents of serious accidents such as nerve injury, vasovagal reaction and infections. Medical staff performing phlebotomy must be aware of the pathophysiology of these side effects, be careful in avoiding them, and be trained to properly treat them. Nerve injury is the complication of which phlebotomists should take the greatest caution. It sometimes causes permanent motor and/or sensory nerve dysfunction of arms and hands. Appropriate selection of

Clefts of the lip and palate: is the Internet a trustworthy source of information for patients?

Great numbers of patients use the Internet to obtain information and familiarize themselves with medical conditions. However, the quality of Internet-based information on clefts of the lip and palate has not yet been examined. The goal of this study was to assess the quality of Internet-based patient information on orofacial clefts. Websites were evaluated based on the modified Ensuring Quality Information for Patients (EQIP) instrument (36 items). Three hundred websites were identified using the most popular search engines. Of these, 146 were assessed after the exclusion of duplicates, irrele

Musculoskeletal Injury in Australian Infantry Personnel: A Cross-sectional Study to Understand Prevention Priorities.

INTRODUCTION: Musculoskeletal injury patterns are under-investigated in the Royal Australian Infantry Corps. Subsequently, more evidence is needed to support injury prevention processes in this population. One difficulty in collecting injury information to monitor injury patterns within combat populations accurately is known injury concealment behaviors in such populations. This study aims to examine musculoskeletal injury epidemiology within Australian infantry battalions using a tailored approach to mitigate reporting avoidance. MATERIALS AND METHODS: A cross-sectional study using an anonym

Protocol for a randomized controlled trial examining multilevel prediction of response to behavioral activation and exposure-based therapy for generalized anxiety disorder.

BACKGROUND: Only 40-60% of patients with generalized anxiety disorder experience long-lasting improvement with gold standard psychosocial interventions. Identifying neurobehavioral factors that predict treatment success might provide specific targets for more individualized interventions, fostering more optimal outcomes and bringing us closer to the goal of "personalized medicine." Research suggests that reward and threat processing (approach/avoidance behavior) and cognitive control may be important for understanding anxiety and comorbid depressive disorders and may have relevance to treatmen

Human Trafficking and Exploitation in Pediatric Acute Care: Recognition, Communication, and Response.

Human trafficking and exploitation of children are recognized pediatric public health concerns. Children and adolescents who experience trafficking may present to pediatric acute care settings with injuries, sexually transmitted infections, pregnancy concerns, malnutrition, substance use, chronic pain, behavioral health crises, delayed care, or non-specific somatic symptoms. Disclosure is uncommon, and clinical recognition depends on awareness of risk factors, careful observation, trauma-informed communication, and clear response pathways. Pediatric clinicians should avoid relying on stereotyp

[Acute abdomen : What does the nonradiologist expect from the radiologist?].

BACKGROUND: The acute abdomen is a life-threatening condition that demands urgent intervention. The required diagnostics should address the core problem and has to be chosen based upon the diagnostic strength of each diagnostic tool. Modalities with limited discriminating information regarding differential diagnosis have to be avoided. Expectancy and thoughts of the radiologist often differ from the view of the clinician in the emergency department. OBJECTIVE: The decision about which diagnostic tools are valuable or unnecessary in the emergency setting is made from a surgeon's point of view.

Anatomic Danger Zones of the Head and Neck.

BACKGROUND: Dermatologic procedures require a detailed understanding of surface anatomy to avoid complications. The head and neck region has prominent danger zones including nerves and vasculature that may be at risk during cutaneous surgery. A thorough understanding of these danger zones can help avoid complications that may lead to functional or cosmetic impairment. METHODS: The anatomic literature regarding the course of high-risk structures of the head and neck was reviewed. Structures deemed at risk during dermatologic procedures were included in the analysis. The final analysis focused

Evaluation of WaySafe: A Disease-Risk Reduction Curriculum for Substance-Abusing Offenders.

With a focus on reducing disease risk behavior in the community, a six-session curriculum, WaySafe, was developed to increase positive decision-making skills among soon-to-be-released inmates participating in a therapeutic community substance abuse treatment program. The intervention used TCU Mapping-Enhanced Counseling as an approach to focus on cognitive aspects of risky sexual and drug use behaviors in an effort to improve problem recognition, commitment to change, and strategies for avoiding behavioral risks of infections. A total of 1393 inmates from eight different institutions in two st

Transplantation of en bloc pediatric kidneys with a partial bladder segment in an adult recipient.

The success of transplantation of the urinary bladder en bloc with pediatric kidneys remains undetermined. We transplanted a large portion of the bladder with en bloc kidney allografts from a 13-month-old donor into a 45-year-old adult male. The recipient, who was on long-term hemodialysis, had a small urinary bladder consistent with long-standing anuria. Informed consent was given and bilateral kidneys were transplanted en bloc with both ureters and bladder. The patient's bladder was augmented with the donor bladder and bilateral ureteroneocystotomies of small ureters was avoided. At 3 and 18

Levodopa/dopamine replacement strategies in Parkinson's disease--future directions.

After 40 years, levodopa remains the most effective therapy for the treatment of PD. However, long-term therapy is complicated by motor fluctuations and dyskinesia that can represent a source of significant disability for some patients. Other medical therapies that are currently available for the treatment of PD primarily represent an attempt to prevent or treat motor complications. Surgical therapies improve motor complications in appropriate candidates, but do not provide antiparkinsonian benefits that are superior to levodopa, and are themselves associated with potentially serious side effe

Obstacle stepping involves spinal anticipatory activity associated with quadrupedal limb coordination.

Obstacle avoidance steps are associated with a facilitation of spinal reflexes in leg muscles. Here we have examined the involvement of both leg and arm muscles. Subjects walking with reduced vision on a treadmill were acoustically informed about an approaching obstacle and received feedback about task performance. Reflex responses evoked by tibial nerve stimulation were observed in all arm and leg muscles examined in this study. They were enhanced before the execution of obstacle avoidance compared with normal steps and showed an exponential adaptation in contralateral arm flexor muscles corr

Adrenal pheochromocytoma: Keys to radiologic diagnosis.

Pheochromocytomas are adrenal paragangliomas. Potentially malignant, these tumors have a low incidence but clear importance. They can appear in various hereditary syndromes, especially in von Hippel-Lindau syndrome, multiple endocrine neoplasia-2 (MEN2), and familial paraganglioma syndromes. In sporadic cases, underlying genetic alterations are often found, and these findings are changing our understanding of the disease. Although these tumors can manifest with a characteristic clinical presentation, in 13.1%-57.6% of cases, it is the radiologist who first suggests the diagnosis, indicating an

[Kawasaki disease. Analysis of 17 cases].

BACKGROUND: Kawasaki disease (EK) is frequently identified in pediatric patients. It affects principally the coronary arteries, causing aneurysms, ischemic heart disease and sudden death. The opportune diagnosis and its early treatment avoid these complications. Our objective was to describe 17 cases with EK. CLINICAL CASES: seventeen patients' files with EK were reviewed, obtaining information about age, sex, date of admission, clinical manifestations, laboratory results, complications and treatment. RESULTS: four patients presented coronary aneurysm. The average age was 37 months, with a r

[Management of first trimester miscarriages].

INTRODUCTION: First trimester miscarriages complicating 15 to 20% of pregnancies, are the leading cause of gynecology emergencies. OBJECTIVES AND METHODS: This review exhaustively summarized the French and English reports found on Medline data base about the management of first trimester miscarriage. RESULTS: The treatment of reference remains surgical evacuation. It is generally used to avoid haemorrhage and intrauterine infection but is also required for women who wish an immediate termination of pregnancy. Recently, alternatives have been proposed, such as expectant management or medical

[Perinatal mortality and evitability: a review].

This is a literature review onperinatal mortality focusing its evitability. A Medline and Lilacs (Latin-America and Caribbean) search was conducted for the 90s. There are few research studies on this subject in Brazil due to the great number of underreported fetal deaths and the low quality information provided in death certificates. Different proposals for perinatal death classification are presented. Most are based on grouping the underlying causes of deaths in a functional system in order to facilitate the analysis. In the Wigglesworth classification system, one of the most recommended meth

The Guadeloupean perinatal mortality audit: process, results, and implications.

In 1983, we implemented an ongoing perinatal mortality audit in Guadeloupe to identify factors underlying the current level of poor perinatal outcome and to stimulate increased provider motivation. The audit was part of an active research approach to document the specific nature of the perceived perinatal health problem, to modify provider behavior through the use of this acquired information, and, ultimately, to decrease avoidable perinatal deaths. We investigated all 320 perinatal deaths in Guadeloupe during 1984 and 1985. Nearly one-half of the perinatal deaths reviewed were determined to b

[Transcervical extraamniotic Rivanol instillation as a method of avoiding complications in pregnancy interruption].

The number of abortions performed is steadily increasing. According to information available to date from the Federal Statistics Office in the Federal Republic of Germany, pregnancies are still largely terminated by means of cervical dilatation with Hegar dilatators and subsequent curettage. Almost half of all abortions are performed in young women who have not yet given birth. In a great number of cases these patients have a gracile, rigid cervical os which has to be opened abruptly in a few minutes with Hegar's dilatators, often under asserting quite some force. This can result in laceration
