[Informing the transfused patient of the possible transmission of variant Creutzfeldt-Jakob disease by blood transfusion].

UNLABELLED: TRUE RISKS AND THEORETICAL RISKS: The texts ruling the obligation of the physician to inform the patient appears not to include theoretical risks in their application. In France however, the field of blood transfusions extends this obligation to the theoretical risk of potential transmission through the blood of the infectious agent responsible for Creutzfeldt-Jakob's disease. UNANSWERED QUESTIONS: From an ethical point of view, is information on the theoretical risk of transmission founded? From a formal point of view, the means used for such an extension (i.e.,a bulletin) is deb

Ethics and practice: two worlds? The example of genetic counselling.

The aim of this paper is to work out the relationship between ethics and practice with reference to genetic counselling. First, the most important principles with respect to genetic counselling and to counsellor-client-interaction, are explained briefly. Then, we discuss what these principles might mean, when applied to the practice of counselling. To do so, we also look at some empirical data. Finally, we draw some conclusions.

[Personal experiences of a physician with legal proceedings].

According to the author's experience with a judicial inquiry, the borderline between the right of the prosecutor to get an insight into the patients file and the right of the physician to refuse the disclosure of the patient's medical history is very much blurred. Even if the inquiry is directed just against an unknown person, a physician should be aware that the Department of Public Prosecution might take stiffening procedural steps to enforce its demand. At any rate, the physician's position to keep the patient's data secret is very weak in such a situation.

How--and when--to obtain consent for do-not-resuscitate orders. Clinical guidelines and strategies for resolving conflicts.

Consider a do-not-resuscitate (DNR) order when a patient's presumed consent for cardiopulmonary resuscitation (CPR) is in question, the patient has an illness that is terminal or severe and irreversible, or he or she is permanently unconscious or likely to have cardiac or respiratory arrest. The patient with decisional capacity has the right to give or withhold consent for a DNR order. State law may limit a surrogate's authority to request that CPR be withheld. Remember, a DNR order does not restrict a patient's access to intensive care. Nurses, patient advocates, social workers, and clergy me

Bone-marrow transplantation for thalassaemia.

An 18-month-old boy with beta-thalassaemia major underwent bone-marrow transplantation with marrow from his 30-month-old brother. The brother was HLA-identical, mixed-lymphocyte culture non-reactive and had thalassaemia minor. The patient was "conditioned" with busulphan and cyclophosphamide before transplantation and received methotrexate to prevent graft-versus-host disease. Immediately after the transplant, complications arose, which included mild graft-versus-host disease, gastrointestinal bleeding and fever. The boy is alive 18 months after transplantation, is leading a normal life, is re

This report describes a severely retarded adult patient whose self-injurious behavior became acutely life threatening. Electroconvulsive therapy (ECT) was prohibited by law, but it was administered after the law was judged unconstitutional. The patient's response to ECT was excellent, and there were no adverse consequences.
