Physiologic-Based Cord Clamping Maintains Core Temperature vs. Immediate Cord Clamping in Near-Term Lambs.

Background: Physiologic-based cord clamping (PBCC) involves deferring umbilical cord clamping until after lung aeration. It is unclear if infant is at risk of becoming hypothermic during PBCC. Objectives: To test if PBCC would maintain core temperature more effectively than immediate cord clamping (ICC). Design: At 0.93 gestation, fetal lambs were surgically exteriorized and instrumented from pregnant ewes under general anesthesia. Prior to the start of the experiment, lambs were thoroughly dried, placed on hot water bottles, and core temperature was continuously monitored using a rectal therm

19th century pioneers of intensive therapy in North America. Part 2: Joseph O'Dwyer.

Dr Joseph O'Dwyer's principal distinction lies in his dedication and achievements in battling laryngeal diphtheria among children at the New York Foundling Asylum, where he was a physician from 1872. He was also active at the Presbyterian Hospital of New York and in private practice. Some of his pioneering work anticipated methods of intensive care medicine. His achievements included: * Introduction of a practical system of intralaryngeal intubation, including designs of tubes and a technique for inserting them, after many years of careful study and experimentation - always "without borrowed i

Nutritional depletion and its relationship to respiratory impairment in patients with chronic respiratory failure due to COPD or restrictive thoracic diseases.

OBJECTIVE: While malnutrition, especially fat-free mass index (FFMI), is a predictor for mortality in chronic obstructive pulmonary disease (COPD), less information on prevalence and mechanisms is available in patients with chronic respiratory failure (CRF) due to restrictive thoracic diseases (RTD). DESIGN AND SETTING: Cross-sectional study of patients consecutively admitted to an in-patient primary pulmonary centre. SUBJECTS: One hundred and thirty-two patients (30% RTD; 70% COPD) with CRF and intermittent non-invasive positive pressure ventilation. INTERVENTIONS: Malnutrition was quantif

Chronic lung disease of prematurity and intrauterine growth retardation: a population-based study.

OBJECTIVE: To determine the risk of chronic lung disease (CLD) in small for gestational age (SGA) preterm infants in comparison to appropriately grown and large for gestational age (LGA) infants. METHODS: Observational study derived from a geographically defined population (Trent Health Region, United Kingdom). All preterm infants of <or=32 completed weeks' gestation born to Trent resident mothers admitted to neonatal units between 1995 and 1999 (inclusive) were included. Birth weight percentiles were created for the whole population, and infants were classified as SGA infants (if <10th perce

Elective high-frequency oscillatory ventilation versus conventional ventilation for acute pulmonary dysfunction in preterm infants.

BACKGROUND: Respiratory failure due to lung immaturity is a major cause of mortality in preterm infants. Although the use of intermittent positive pressure ventilation in neonates with respiratory failure saves lives, its use is associated with lung injury and chronic lung disease (CLD). A newer form of ventilation called high-frequency oscillatory ventilation (HFOV) has been shown to result in less lung injury in experimental studies. OBJECTIVES: The objective of this review is to determine the effect of the elective use of HFOV as compared to conventional ventilation (CV) on the incidence o

Sustained versus standard inflations during neonatal resuscitation to prevent mortality and improve respiratory outcomes.

BACKGROUND: At birth, infants' lungs are fluid-filled. For newborns to have a successful transition, this fluid must be replaced by air to enable gas exchange. Some infants are judged to have inadequate breathing at birth and are resuscitated with positive pressure ventilation (PPV). Giving prolonged (sustained) inflations at the start of PPV may help clear lung fluid and establish gas volume within the lungs. OBJECTIVES: To assess the benefits and harms of an initial sustained lung inflation (SLI) (> 1 second duration) versus standard inflations (≤ 1 second) in newborn infants receiving resu

Beyond mean and maximum CO₂: introducing hypercapnic burden as a marker of sleep-related hypoventilation.

OBJECTIVES: In obesity hypoventilation syndrome (OHS), nocturnal hypoventilation can precede daytime respiratory failure. Therefore, the hypercapnic burden index was introduced to quantify sleep-related CO₂ retention. METHODS: Thirty-one patients who underwent polysomnography with transcutaneous capnography and met OHS criteria were retrospectively included in this single-center cross-sectional study. The area above the predetermined transcutaneous partial pressure of CO2 thresholds of 45 mmHg (thresh45) and 50 mmHg (thresh50) from the extracted capnography graphs was computed and statistical

Nurse-managed telemonitoring in patients with sleep apnea syndrome treated with positive pressure ventilation.

PURPOSE: Obstructive sleep apnea (OSA) is the commonest sleep disorder, it is characterized by episodes of complete or partial upper airway obstruction, sleep fragmentation and intermittent hypoxia (Am J Epidemiol 177:1006-14, 2013). Continuous Positive pressure ventilation (CPAP), is the gold standard treatment for Obstructive Sleep Apnoea Syndrome (OSAS). Several psycho-educational and technological interventions have been implemented to improve compliance but their impact on therapeutic adherence hasn't turned out to be substancial (Am J Respir Crit Care Med 159:1108-1114, 1999). What is the basis for a genetic approach in neonatal disorders?

Gene-environment interactions likely account for some degree of the variance in response rates that are clinically observed with antenatal corticosteroids, breast milk prophylaxis, surfactant administration, early recognition and treatment of sepsis, utility of non-invasive ventilation, and judicious exposure to supplemental oxygen. While these therapies and practice guidelines have significantly decreased overall neonatal mortality in the NICU, they have not made a marked impact on the frequency and severity of conditions such as bronchopulmonary dysplasia (BPD), necrotizing enterocolitis, an

The ALS Nutrition/NIPPV Study: design, feasibility, and initial results.

Our objective was to investigate the nutritional requirements in ALS and to determine the feasibility of early intervention with NIPPV. Subjects were enrolled into one of two arms. In the nutrition arm, total daily energy expenditure (TDEE) was determined longitudinally over 48 weeks using the Doubly Labeled Water method. In the NIPPV arm, NIPPV was offered at 80% vs. 50% FVC. Additional measurements were obtained in both arms to ultimately formulate equations to predict TDEE and to estimate sample size for a phase III study of early NIPPV. Eighty subjects were enrolled in the nutrition arm an

Eficacy and safety of restricted fluid therapy in transient tachypnea of the newborn: a randomized controlled study.

BACKGROUND: Evidence suggests that increased intravascular and interstitial fluid load in neonates with transient tachypnea of the newborn (TTN) may delay the clearance of fetal alveolar fluid (FAF). Restricted fluid (RF) therapy may accelerated FAF clearance and improve outcomes in these infants. METHODS: Term and late preterm infants with TTN requiring nasal intermittent positive pressure ventilation (NIPPV) were randomized within first 2 h after birth to receive either RF or standard fluid (SF) therapy. Primary outcomes were the duration of NIPPV and the day of discharge. Secondary outcome

Return of spontaneous circulation after cardiopulmonary arrest in an adult horse recovering from anaesthesia.

This report describes successful cardiopulmonary resuscitation of a healthy, 525 kg, 8 years 9 months-old Quarter horse gelding. After successful pelvic limb orthopaedic surgery to excise a metatarsal bone under general anaesthesia, with the horse in left lateral recumbency, the horse was hoisted into recovery. After the horse was positioned in recovery, a routine cardiac check by thoracic auscultation was performed. Cardiopulmonary arrest (CPA) was identified by absence of cardiac sounds and pulse, as evaluated by digital palpation of the facial artery, absence of the right palpebral reflex,

The management of severe rhesus isoimmunization by fetoscopic intravascular transfusions.

Twenty-five severely rhesus-isoimmunized fetuses, including 15 with hydrops fetalis, underwent a total of 77 intrauterine transfusions between 19 and 32 weeks' gestation. Fifty-eight of the procedures were fetoscopically directed intravascular transfusions, nine were ultrasound-guided intraperitoneal transfusions, and 10 were a combination of intravascular transfusion, fetal paracentesis, and intraperitoneal transfusion. The average number of antenatal procedures per patient was three (range, one to five). The survival rate for the 19 fetuses that received their initial intrauterine transfusio

High-Flow Nasal Oxygen vs Noninvasive Positive Airway Pressure in Hypoxemic Patients After Cardiothoracic Surgery: A Randomized Clinical Trial.

IMPORTANCE: Noninvasive ventilation delivered as bilevel positive airway pressure (BiPAP) is often used to avoid reintubation and improve outcomes of patients with hypoxemia after cardiothoracic surgery. High-flow nasal oxygen therapy is increasingly used to improve oxygenation because of its ease of implementation, tolerance, and clinical effectiveness. OBJECTIVE: To determine whether high-flow nasal oxygen therapy was not inferior to BiPAP for preventing or resolving acute respiratory failure after cardiothoracic surgery. DESIGN AND SETTING: Multicenter, randomized, noninferiority trial (B

Congenital cystic adenomatoid malformation in the newborn: two case studies and review of the literature.

Congenital cystic adenomatoid malformation (CCAM) is a congenital malformation of the lung that can present on imaging studies as abnormal air, air/fluid-filled cysts, or fluid-filled/solid-appearing cysts. The use of ultrasound in prenatal management has increased the number of cases diagnosed in utero. Early diagnosis is vital in the medical management of CCAM. Outcome varies from hydrops and fetal demise to complete resolution before birth. Many CCAMs diagnosed in utero may decrease in size even if substantial mediastinal shift and lung compression are noted at the time of diagnosis. Once t
