Risk Factors for Nephrolithiasis in Adults with Short Bowel Syndrome.

INTRODUCTION: Patients with short bowel syndrome (SBS) commonly develop nephrolithiasis. However, the risk factors for nephrolithiasis in patients with SBS remain unclarified. The present study aimed to identify the risk factors for nephrolithiasis in adults with SBS. METHODS: All eligible adults diagnosed with SBS and admitted to a tertiary referral center from December 2008 to 2018 were retrospectively identified from a prospectively maintained database. Patients' demographic and clinical characteristics were analyzed using univariate and multivariate analyses to identify the risk factors f

Comparison of robotic versus laparoscopic hepaticojejunostomy for choledochal cyst in children: a first report.

AIM: We compared robotic hepaticojejunostomy anastomosis (RHJA) with laparoscopic hepaticojejunostomy anastomosis (LHJA) in children undergoing complete excision of choledochal cyst. METHODS: Difficulty of suturing (DOS) during anastomosis was scored blindly, from intraoperative video recordings, using: 5 = impossible; 4 = difficult; 3 = tedious; 2 = slow; and 1 = easy. A panel of fiveindependent surgeons was also asked to compare RHJA with matched LHJA and score + 1 if RHJA appeared superior to LHJA, 0 if RHJA appeared equivalent to LHJA, and - 1 if RHJA appeared inferior to LHJA. RESULTS: …

Minimally invasive surgery for superior mesenteric artery syndrome: A case report.

Superior mesenteric artery (SMA) syndrome is defined as a compression of the third portion of the duodenum by the abdominal aorta and the ov … often required. Currently, traditional open bypass surgery has been replaced by laparoscopic duodenojejunostomy as a curative surgical approach. However, single incision laparoscopic approach is rarely performed. A 20-year-old female patient with a diagnosis of anorexia nervosa and SMA syndrome was prepared for surgery after failed conservative management. As …

[A long-term control of gastrointestinal stromal tumor with sunitinib].

The patient was a 70-year-old woman with gastrointestinal stromal tumor(GIST)of the small intestine, accompanied by liver metastasis. Multiple liver metastases were pointed out 3 months after R0 surgery(jejunectomy and hepatectomy). Although she was given radiofrequency ablation(RFA)therapy and imatinib, metastatic tumors progressed. In a further examination at our institution 21 months after R0 surgery, multiple liver, bone, and lung metastases were indicated, and she was given sunitinib once daily at a 50mg in 6-week courses, with 4 weeks on and 2 weeks off treatment. Although sunitinib dosa

Traumatic rupture of a type IVa choledochal cyst in an adult male.

Choledochal cyst (CC) is a rare, congenital anomaly of the bile ducts. We describe a 26-year-old male patient who was transferred to our hos … s successfully managed by primary cyst excision, cholecystectomy, and Roux-en-Y end-to-side hepaticojejunostomy reconstruction. The postoperative course was uneventful and the patient was discharged on the 12(th) day of hospitalization. Four weeks after surgery, abdominal CT scan showed pneumatosis in the intrahepatic bile duct, and intrahepatic dilatation wh …

Colonic ischemia and perforation associated with enteral feeding through an ileal tube.

Reported complications of enteral feeding through a jejunostomy include diarrhea, intraperitoneal leaks, bowel obstruction, fistula formation, wound infection, tube occlusion, and other mechanical malfunctions. However, the incidence of these complications is very low, and many physicians prefer to feed their patients by means of a jejunal tube instead of parenteral nutrition. A potentially lethal complication is ischemia of the bowel distal to the site of insertion of the feeding catheter. The described cases of bowel ischemia secondary to enteral nutrition invariably occurred at the level of…

[Neuromas of the extrahepatic bile ducts].

Authors collected three cases of neuromas situated in the wall of extrahepatic bile ducts, which occluded lumen and caused jaundice. In two cases without any evidence of previous surgery or cholelithiasis, neuroma was localized in ductus choledochus. After excision of ductus choledochus and cholecystectomy, we performed hepaticojejunostomy Roux--en Y. In the last patient, 7 years after cholecystectomy, neuroma localized in hepatic duct was treated by T-tube. In all patients with mechanical jaundice, before operation we had never correctly diagnosed this very rare cause of obstruction of extrah…

Closed one-stage functional end-to-end jejunojejunostomy in horses with use of linear stapling equipment.

OBJECTIVE: To use gastrointestinal linear stapling instrumentation to perform a closed one-stage functional end-to-end jejunojejunostomy in adult horses as an alternative to a stapled side-to-side jejunojejunostomy. STUDY DESIGN: Clinical outcome, anatomic and histological architecture, and anastomotic dimensions were determined at 2, 4, 8, 16, and 24 weeks postoperatively. ANIMALS OR SAMPLE POPULATION: 18 adult horses. METHODS: The anastomosis and peritoneal cavity were inspected for adhesions. The diameter and circumference of the anastomosis and jejunum oral and aboral to the anastomosis…

Choledochal cysts: role of noninvasive imaging, percutaneous transhepatic cholangiography, and percutaneous biliary drainage in diagnosis and treatment.

Choledochal cysts are an uncommon anomaly of the biliary system; findings include cystic dilatation of the extrahepatic biliary tree, intrahepatic biliary tree, or both. In the past 7 years, 13 patients with choledochal cysts have been seen at the authors' institution for evaluation and presurgical intervention. Percutaneous transhepatic cholangiography (PTC) was performed in 13 patients. Sixteen percutaneous biliary drainage (PBD) procedures were performed in 11 patients, computed tomography was performed in six patients, and ultrasound was performed in two. PTC proved to be an important imag

How to rescue a percutaneous gastrostomy tube perforating the jejunum.

BACKGROUND AND AIMS: Percutaneous gastrostomy (PEG) tubes are widely used for long-term enteral nutrition. Major adverse events are rare but often necessitate surgical repair. We detail an endoscopic approach to repairing a PEG tube perforation involving the jejunum. METHODS: A 75-year-old man with short bowel syndrome and a recent combined intestinal-kidney transplant (duodenojejunostomy) presented with persistent abdominal pain and delirium 5 days after PEG tube placement. Computed tomography imaging showed the PEG bumper in the stomach, but the tube was perforating the overlying small bowe…

Intussusception at the jejunojejunal anastomosis five years after RYGB: A case report.

INTRODUCTION: Morbid obesity is a chronic disease with increasing global prevalence. Bariatric surgery is considered the most effective ther … susception at the jejuno-jejunal anastomosis. The patient underwent surgical revision of the jejuno-jejunostomy. DISCUSSION: RYGB is one of the most common bariatric procedures. While it is highly effective for weight loss and improvement of comorbidities, it is associated with certain early and late complications, including the rare occurrence of intussusce …

Endoscopic Vacuum Therapy for Treating an Esophago-Pulmonary Fistula after Esophagectomy: A Case Report and Review of the Literature.

An acquired esophago-respiratory fistula represents an abnormal connection between the esophagus and the respiratory system. It is usually c … as treated conservatively with EVT and antibiotics. Nutrition was administered through the existing jejunostomy. Both fistulas and the paraesophageal cavity were fully healed, oral intake was maintained, and the patient was discharged. This rare life-threatening complication can be treated conservatively. Its management is challenging, controversial, and lack …
