A sequential approach to the management of an advanced case of pellucid marginal degeneration.

PURPOSE: We report a sequential approach in the management of a case with advanced pellucid marginal degeneration (PMD). The management was tailored based on the patient's corneal topography and degree of corneal astigmatism. Full thickness corneal transplants in cases of PMD carry a prolonged rehabilitation period and the risk of graft failure. This case consisted of lamellar wedge resection followed by wavefront guided transepithelial photorefractive keratectomy (wTPRK) combined with accelerated corneal crosslinking (aCXL) in preparation for a cataract extraction with intraocular lens. OBSER…

RNA-Seq analysis and comparison of corneal epithelium in keratoconus and myopia patients.

Keratoconus is a common degenerative corneal disease that can lead to significant visual morbidity, and both genetic and environmental factors have been implicated in its pathogenesis. We compared the transcriptome of keratoconus and control epithelium using RNA-Seq. Epithelial tissues were obtained prior to surgery from keratoconus and myopia control patients, undergoing collagen cross-linking and photorefractive keratectomy, respectively. We identified major differences in keratoconus linked to cell-cell communication, cell signalling and cellular metabolism. The genes associated with the He…

Is refractive surgery justified?

Refractive surgery may be defined as any kind of eye surgery in which attempt is made to produce a level of spectacle error that is accepted … iveness and can be performed in a remote area. There are two ways of excimer laser: photorefractive keratectomy (PRK) and laser assisted in-situ keratomileusis (LASIK). The processes are elaborated in the article. Before any refractive surgery is performed, it is imperative that the whole of the eye receives full and detailed attention. The patient needs to b …

[Progressive corneal ectasia after laser in situ keratomileusis (LASIK)].

BACKGROUND: LASIK (Laser in situ keratomileusis) is used in refractive surgery especially for correction of higher degrees of myopia. Preservation of Bowman's layer as well as less postoperative pain and the slight to absent subepithelial haze are regarded as advantages compared to photorefractive keratectomy (PRK). However, numerous serious complications have been described in the literature. PATIENTS AND METHODS: LASIK treatment was performed elsewhere in two patients to treat myopia or myopic astigmatism between -6 and -9 diopters (D). An astigmatism of -6 D was corrected with the LASIK me…

The use of corticosteroid/beta-blocker combinations in the management of regression after PRK for high myopia.

After nearly 8 years of clinical experience, and with conflicting evidence from trials with and without corticosteroids, there is still no single, universally agreed postoperative management regimen for photorefractive keratectomy (PRK). All agree that corticosteroids will reverse sudden regressions, and so most still use varying corticosteroid regimens. In Hong Kong we are faced with many patients whose myopia is greater than -6 diopters (D), and we agree with the general observation that the regression/haze rate is much greater for these higher errors, and that increased intraocular pressure…

Centration of myopic refractive ablation: should we center treatment on the pupil or the visual axis?

The purpose of this study is to compare pupil versus corneal vertex-centered ablation for myopic laser refractive surgery. This study is a retrospective case series of right eyes of consecutive myopic patients undergoing either photorefractive keratectomy (PRK) or laser-assisted in situ keratomileusis (LASIK) with pupil or corneal vertex-centered ablation from January 2018 to April 2018. Overall 258 eyes of 258 patients were included. Of the 104 that underwent LASIK, 52 were treated centered on the corneal vertex (50%), and of the 154 that underwent PRK, 77 were treated centered on the corneal…

Use of presbyopia-correcting intraocular lenses in patients with prior corneal refractive surgery.

PURPOSE OF REVIEW: Corneal refractive surgery has achieved spectacle-free vision for millions of patients, but this aging population is now developing cataracts. Many of these patients may wish to avoid reliance on glasses after cataract surgery. Presbyopia-correcting intraocular lenses (IOLs) offer a solution, but corneal changes after refractive surgery may compound higher order aberrations and dysphotopic symptoms associated with these IOLs. This review aims to discuss potential factors that could aid in determining suitable postkeratorefractive candidates for presbyopia-correcting IOLs. R

Single-Step Transepithelial PRK vs Alcohol-Assisted PRK in Myopia and Compound Myopic Astigmatism Correction.

Transepithelial photorefractive keratectomy (tPRK), where both the epithelium and stroma are removed in a single-step, is a relatively new procedure of laser refractive error correction. This study compares the 3-month results of myopia and compound myopic astigmatism correction by tPRK or conventional alcohol-assisted PRK (aaPRK).This prospective, nonrandomized, case-control study recruited 148 consecutive patients; 93 underwent tPRK (173 eyes) and 55 aaPRK (103 eyes). Refractive results, predictability, safety, and efficacy were evaluated during the 3-month follow-up. The main outcome measur…
