Medial facet patelloplasty in patellar instability associated with patellofemoral dysplasia: a report of 26 cases.

INTRODUCTION: Numerous procedures may be used in the surgical treatment of patellar instability. We have noticed that certain types of patellar instability result in a bulge (or protrusion) of the distal medial facet of the patella which can hinder recentering in the trochlear groove. HYPOTHESIS: In certain patellofemoral dysplasias, trimming down of this bulge (patelloplasty) as long as it is associated with a "à la carte surgery" program, can improve stability, centering and lateral patellar tilt. PATIENTS AND METHODS: This retrospective series included 23 patients (26 knees), 13 women and

Bilateral Perthes Masking Morquio Disease.

INTRODUCTION: Evaluation of pain in children after trauma can sometimes be difficult; in particular, a knee pain in a child could originate from the hip, until evidence of the contrary. Often is the low-energy trauma that leads to a joint dislocation in children. Furthermore, the present guidelines state that a hip dislocation in a child should be replaced within 6 (maximum 12) hours, to reduce the risk of avascular necrosis (AVN) of the femoral head. CASE PRESENTATION: After falling during sports activity at the kindergarten, a 5-years-old boy was taken to the emergency room of a small hospi

Functional outcome after recurrent patellar dislocation : Comparison of two surgical techniques-Medial patellofemoral ligament reconstruction (MPFL) vs. Elmslie Trillat procedure.

BACKGROUND: There is no final consensus regarding the ideal surgical technique for the treatment of patellar dislocation. The aim of this retrospective pilot study was to describe muscle strength, body composition, self-reported physical performance, and pain in male patients after patellar dislocation treatment with two different surgical techniques: medial patellofemoral ligament (MPFL) reconstruction vs. the Elmslie-Trillat procedure. METHODS: Isokinetic testing of knee extensor muscles was performed using a Biodex System 3 pro dynamometer at an angular velocity of 60°/s. Body composition

COL12A1 gene mutation is associated with habitual patellar dislocation: a case report and literature review.

Variants within COL12A1 have been associated with the occurrence and progression of a number of musculoskeletal disorders. Here, we report a case of patellar dislocation and limited mobility in a 13-year-old patient and genetic determination of the etiology of their condition utilizing diagnostic whole-exome sequencing. The patient underwent diagnostic whole-exome sequencing to look for pathogenic variants, which were classified using the ACMG classification standards. A heterozygous nucleotide variant (NM_004370.6: c.8179-2A > G) in intron 53 of the COL12A1 gene was identified and resulted in

[Intra-articular dislocation of patella].

The case of a patient aged 73 years who, following an accidental fall on a staircase, suffered an acute intraarticular dislocation in the left knee-cap is presented. This exceptional dislocation presented incarceration of the proximal kneecap pole in the femoral intercondylar groove. A reduction was carried out under general anaesthetic due to pain and muscular contraction in the attempts at reduction without anaesthetic. We made a surgical check that ruled out associated lesions to the extensor apparatus, soft parts, joint structures and osteophytes. The patient presented advanced arthrosis,

[Acute and chronic injuries to the knee in the doctor's office].

In the diagnostic work-up of acute and chronic injuries to the knee, the patient's history together with the clinical findings are of decisive importance. Injuries to the cruciate and lateral ligaments, or to the menisci, as well as osteochondritis dissecans or chronic recurrent dislocation of the patella can usually be reliably diagnosed on the basis of information provided by the patient, and diagnostic manipulations. In this connection, Lachman's test, various drawer phenomena, the pivot shift test, and tests of the internal menisci described by McMurray, Payr, Steinmann, and Cabot as well

Simultaneous arthroscopic reconstruction of the anterior and posterior cruciate ligament using hamstring and quadriceps tendon autografts.

BACKGROUND: Most dislocated knees involved tears in the two cruciate ligaments were often accompanied by other collateral ligament complexes. Surgical repair or reconstruction seems to achieve results superior to conservative treatment. Various methods of reconstructing anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) after knee dislocation have been described, but few reports discuss simultaneous ACL and PCL reconstructions in a single operation. METHODS: Eleven consecutive patients (6 males and 5 females) with both ACL and PCL disruptions were enrolled in the prospecti…

Preoperative uncorrectable tibiofemoral subluxation can worsen clinical outcomes after fixed-bearing unicompartmental knee arthroplasty: a retrospective analysis.

INTRODUCTION: The presence of tibiofemoral subluxation (TFS) in patients with unicompartmental arthritis, a potential contraindication to unicompartmental knee arthroplasty (UKA), remains controversial and is not commonly discussed. This study aimed to determine the predictability of postoperative TFS before surgery and the effect of TFS on clinical outcomes after fixed-bearing UKA. MATERIALS AND METHODS: In total, 70 patients with anteromedial osteoarthritis and osteonecrosis of the knee who underwent fixed-bearing UKA from January 2015 to January 2017 were included. The preoperative TFS was

[A familial form of bilateral recurrent dislocation of the patella with major trochlea dysplasia].

PURPOSE OF THE STUDY: We report four cases of bilateral recurrent dislocation of the patella with major trochlear dysplasia, in the same family. MATERIAL AND METHODS: Details of clinical examination of all members of this family and measurements on knee radiographs are reported. RESULTS: In all cases a severe proximal dysplasia of the trochlea was described on lateral views. The patella and the trochlea had a normal shape on the axial view. DISCUSSION: In some recurrent dislocation, with no associated disease, possibility of a genetic transmission have been suggested in some publications. C

Predicting Surgical Outcomes in Patients With Recurrent Patellar Dislocations.

BACKGROUND: A lateral dislocation of the patella is a common injury in adolescents and young adults that is largely caused by underlying anatomic risk factors. Surgically managed patients have a significantly lower risk of recurrent dislocations. However, determining the optimal surgical treatment remains a challenge, with patients sometimes undergoing multiple surgical procedures before achieving successful stabilization. PURPOSE: To computationally evaluate patients who have undergone multiple surgical procedures to treat recurrent lateral patellar dislocations and predict their clinical ou

Pigmented villonodular synovitis in pediatric population: review of literature and a case report.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a rare proliferative process in children that mostly affects the knee joint. CASE PRESENTATION: The study follows the case of a 3-year-old boy presenting recurrent patellar dislocation and PVNS. Due to symptoms such as chronic arthritis, he had been taking prednisolone and methotrexate for 6 months before receiving a definitive diagnosis. After a period of showing no improvements from his treatment, he was referred to our center and was diagnosed with local PVNS using magnetic resonance imaging (MRI). The patient was treated for his patel

A mother and three daughters with congenital dislocation of the hip and a characteristic facial appearance: a new syndrome?

We describe a mother and her three daughters who all have bilateral congenital dislocation of the hip. The mother has had no other medical problems and is on the 90th centile for height. Her three daughters resemble each other strongly with facial characteristics which include hypertelorism, epicanthic folds, puffiness around the eyes, a flat mid-face and a carp-shaped mouth. All three daughters are on the 3rd centile for height, with their head circumference on a higher centile and all had an ASD. Other features include congenital dislocation of one knee (one), congenital inguinal hernia (one

Healing of a bone-exposed soft tissue defect after surgical correction of traumatic patellar dislocation: A case report.

RATIONALE: Reconstructive surgery is widely considered the primary treatment for soft tissue defects around the knee owing to its high flexibility. However, in our recent case study, we explored an alternative approach using decellularized collagen dressings, which proved highly effective in healing a soft tissue defect involving bone exposure following surgical correction of a traumatic patellar dislocation. PATIENT CONCERNS: A 65-year-old male with a traumatic patellar dislocation in the lower extremity failed to approximate the wound after surgical correction. The patient refused additiona

Polyethylene wear in metal-backed tibial components in unicompartmental knee prostheses.

The utilization of unicompartmental knee arthroplasty (UKA) has increased worldwide and has become a favourable alternative to total knee arthroplasty (TKA) in patients with isolated single-compartment knee osteoarthritis. With the increase in UKA utilization, there has been an increase in the number of revisions. The two most common causes of revision are the progression of osteoarthritis in other compartments and aseptic loosening with polyethylene wear, with other common causes for revision including periprosthetic fracture, bearing dislocation, infection, instability and unexplained pain.

Adolescent and Caregiver-derived Utilities for Traumatic Patella Dislocation Health States.

INTRODUCTION: The treatment of a first-time traumatic patella dislocation in children and adolescents remains controversial. Preference-based health utility assessments can provide health-related quality of life information for orthopaedic conditions and their subsequent treatment. The purpose of this study was to determine utilities for pediatric acute traumatic patella dislocation and subsequent treatment health states from both children with patellar dislocation, and their parents. METHODS: Adolescents with acute first-time patella dislocations and their parents were identified. Six patell

Bilateral symptomatic snapping biceps femoris tendon due to fibular exostosis.

We present an unusual case of an athletic 17-year-old male cyclist with bilateral chronic dislocating biceps femoris tendons. On flexion of the knee, the biceps tendon subluxed over a large exostosis, creating a snapping sound. Snapping of tendons is common around the hip, ankles, shoulder, and elbow, but rare at the knee. When it does occur, snapping about the knee can be due to discoid meniscus, rheumatoid nodules, synovial plicae, iliotibial band syndrome, congenital snapping knee, and snapping tendons. Research revealed only 5 previous cases due to subluxation of the biceps femoris tendon.

Antero-medialisation of the tibial tubercle for patellar instability.

We reviewed 19 patients (24 knees) with patellofemoral instability treated surgically with antero-medialisation of the tibial tubercle and lateral retinacular release. Twenty-two knees had recurrent patellar dislocation and two patellar subluxation. Lateral retinacular release was performed arthroscopically in 15 knees. Average follow-up was 52 (16-86) months. There was one postoperative haemarthrosis and one failed fixation, which needed surgical revision. The average Lysholm score improved from 63.3 to 98 and only one knee had persistent patello-femoral pain postoperatively. The patellar til

Correction of ligament and bone defects in total arthroplasty of the severely valgus knee.

One hundred thirty-five knees with valgus deformity were treated with a minimally constrained, cementless total knee replacement using intramedullary alignment for the femur and tibia. Mean valgus angle before surgery was 16 degrees; mean valgus angle after surgery was 7 degrees. Neither alignment nor varus-valgus stability deteriorated during the six-year follow-up period, but the knees with greater than 25 degrees deformity had a tendency to increase posterior laxity. Severely deformed knees required bone grafting of the medial femoral and tibial surfaces. A technique was developed to resect
