Over an 18-year period a distinctive large cell lymphoreticular neoplasm (Richter's transformation) developed in 9 patients with chronic lymphocytic leukemia. Clinical findings at the onset of Richter's transformation were remarkably uniform and consisted of the abrupt onset of fever, marked asymmetric lymphadenopathy with the formation of masses, splenomegaly, and hepatomegaly. All patients underwent rapid clinical deterioration followed by death within six and a half months. Patients with chronic lymphocytic leukemia (CLL) are susceptible to infection from a variety of opportunistic pathogens. We have described an elderly man with CLL who had repeated, severe bacterial and fungal infections including recurrent cryptococcal meningitis, disseminated histoplasmosis, Vibrio fetus sepsis, Pasturella tularensis sepsis, and Aspergillus pneumonia. B cell and possible T cell defectiveness in CLL as well as chronic corticosteroid therapy contributed to the weakened host defenses.

Mediastinal involvement was found in 11 (group A) of 43 patients affected with acute lymphatic leukemia when early thoracic roentgenograms of these patients were reviewed. The clinical and laboratory features of nine patients with chronic myelomonocytic leukemia are described. A study was conducted of 16 patients with acute promyelocytic leukemia. The most common oral manifestations were hemorrhagic diathesis and leukemic involvement. There was gingival bleeding, petechiae, and ecchymosis of the oral mucosa, massive infiltration-induced gingival swelling, ulcerative glossitis, swelling of the tonsils, and facial palsy. Report on a now 58-year-old female patient who presented in 1969 with the following objective findings: anemia, thrombocytopenia, lymphocytosis and hepatosplenomegaly. The first first diagnosis was lymphosarcoma, but up to 1976 nearly ten other diagnoses were established, e.g. reticulum sarcoma, leukemic lymphosarcoma, CML, ALL, CLL and Waldenström's disease. A patient with chronic lymphocytic leukemia developed extensive pneumonia due to Cryptococcus neoformans. A presumptive diagnosis based on results of a Wright's stain of the sputum was made and appropriate antifungal therapy was started. C neoformans was cultured in COUNTS AS HIGH AS 8 X 10(5)/ml of sputum and was present morphologically for three weeks after sputum cultures had become negative. Report of five cases of juvenile myelomonocytic leukemia; authors describe an hemopathy of the young child with important splenomegaly, hepatomegaly and adenopathies; anemia with erytroblastemia; hyperleucocytosis with myelemia and monocytosis, rich and granulous bone marrow. Also remarkable is evolution, as it fatal outcome often occurs before a two year evolution, without remission looking like acute myeloblastic leukemia, a few patients have remission of most the symptoms, ending in an apparent recovery, which may be spontaneous.

Myelofibrosis is observed in 1/5 of the cases of C.M.L. It consists of reticulin fibers with few collagen and no osteosclerosis. It involves signs which usually indicate the extension of the myelosis to other organs and other types of cells: hepatosplenomegaly, erythroblastosis, thrombocytemia. Its prognosis is always bad. In one third of the cases, myelofibrosis develops early, and in two third it is late. Chimiotherapy is not responsible for it.
