Opioid prescribing patterns at the end of life in an asian advanced cancer cohort: differences by palliative care consultation status.

PURPOSE: Opioid prescribing at the end of life varies widely across clinical contexts, yet real-world evidence from Asian advanced cancer populations remains limited. This study characterized end-of-life opioid prescribing patterns and examined differences according to palliative care consultation status. METHODS: We conducted a retrospective cohort study using tertiary hospital clinical records linked to Korean National Health Insurance claims data. Patients with advanced cancer who received a palliative care consultation were matched 1:1 with those without consultation using propensity scor

Organ donation after ECPR: ethical challenges and clinical implications.

Extracorporeal cardiopulmonary resuscitation (ECPR) offers a life-saving option for select patients with refractory cardiac arrest, yet a substantial proportion suffer devastating neurological injury and die despite extracorporeal support. In this context, organ donation may emerge as a potential downstream pathway, introducing complex ethical tensions at the intersection of life-sustaining treatment, end-of-life care, and organ preservation. This narrative review examines the ethical challenges associated with organ donation following ECPR, with particular attention to the transition from res

Burdens of family caregiving at the end of life.

A patient's ability to be cared for and to die at home is heavily dependent upon the efforts of family caregivers. Considerable stresses are associated with such caregiving, including physical, psychosocial and financial burdens. Research has shown that unmet needs and dissatisfaction with care can lead to negative outcomes for caregivers. While many family caregivers also report caregiving as life-enriching, some report that they would prefer alternatives to care at home, primarily because of these associated burdens. Little is known about which interventions are most effective to support fam

Digital advance care planning education in nursing home placements: a qualitative descriptive study of nursing students' experiences.

BACKGROUND: Advance care planning (ACP) is a core component of high-quality palliative and end-of-life care, particularly in nursing homes where residents commonly experience frailty, multimorbidity and cognitive impairment. Although digital ACP education has shown promise for qualified staff, limited qualitative evidence has examined how undergraduate nursing students experience such learning during clinical placement. This study explored first-year nursing students' experiences of using an online e-resource during nursing home placements exploring its acceptability, perceived influence on kn

The effect of caregiving on women in families with Duchenne/Becker muscular dystrophy.

Duchenne/Becker muscular dystrophy (DBMD) is a disorder of progressive muscle weakness that causes an increasing need for assistance with activities of daily living. Our objective was to assess the psychosocial health and contributing factors among female caregivers in families with DBMD. We conducted a survey of adult women among families with DBMD in the United States (US) from June 2006 through January 2007, collecting data related to the care recipient, perception of caregiving demands, personal factors, and socio-ecologic factors. Life satisfaction, stress, and distress were assessed as o

A person-centred approach to urinary incontinence assessment and care.

Urinary incontinence is prevalent in older people, especially those in residential and nursing home care. If ineffectively managed, it can have negative impacts on quality of life, including moisture-associated skin damage. A Delphi steering group was convened to test a consensus on continence assessment and care. This covered dignity, autonomy and quality of life of patients with incontinence; incontinence as a contributory risk factor for falls; improved skin health; improved education and self-management; reduced pressure on staff and carers; mental health, early-stage dementia and end-of-l

Psychological Adjustment in Apert Syndrome: Parent and Young Person Perspectives.

To date, limited research has been carried out into the psychological impact of having a diagnosis of Apert syndrome (AS) and the life experiences of families living with this condition. The aim of the current study was to explore psychological adjustment to AS from the perspectives of young people, and their parents, with the broader goal of informing care, and support for this population. Four young people (2 male) aged 11 to 15 years and their mothers were interviewed in their homes using a semistructured interview guide and photo-elicitation methods. Transcripts were analyzed using Interpr

Older Patients and Conservative Kidney Management.

IMPORTANCE: In the US, dialysis remains the reflexive treatment option for end-stage kidney disease. Conservative kidney management (CKM), a nondialytic pathway, is infrequently utilized despite the burden dialysis often imposes on patients, caregivers, and health care systems. OBJECTIVE: To examine why older adults choose CKM, how caregivers influence this choice, and how nephrologists support CKM through advance care planning and palliative care referrals. DESIGN, SETTING, AND PARTICIPANTS: This qualitative study included secondary analysis of clinical notes and visit data from patients, c

Check Your Bias: Providing Decisional Support for Incarcerated Surrogate Decision Makers.

Pediatric palliative care (PPC) supports complex decision-making for seriously ill children. This process requires compassionate and skillful communication with a child's caregivers, most often the parents. When a caregiver is incarcerated, there is an additional layer of complexity in navigating these difficult conversations. We present the case of an infant with complex congenital heart disease, severe neurological impairment, and respiratory failure whose parents were faced with the decision to pursue life-prolonging medical care with a tracheostomy or end-of-life care with compassionate ex

Top Ten Tips Palliative Care Clinicians Should Know About Caring for People Receiving Critical Care for Cardiovascular Disease.

Cardiovascular disease (CVD) remains the leading cause of death worldwide and is a major driver of critical illness, resulting in admission to cardiac intensive care units (CICUs). Patients with acute, life-threatening CVD often experience episodes of acute decompensation characterized by high symptom burden that may portend progressive loss of function, significantly impair quality of life (QOL), and involve high-intensity, technologically advanced therapies to sustain life. Despite these advances, up to 30% of patients die during CICU hospitalizations, and those who survive may subsequently
