Lunate Silastic arthroplasty in Kienbock's disease.

The use of Silastic lunate replacement for Kienbock's disease has many advocates. We reviewed the long-term results of eight patients who had this procedure performed. Although good short-term results were obtained, this was not maintained and only two patients were regarded as satisfactory at an average follow-up time of 44 months. Evidence of synovitis was apparent in seven cases. It is now our view that if surgical intervention is necessary for this condition, the best surgical option is probably a scapho-trapezio-trapezoid fusion.

The Osteochondral Autograft Transfer System in Hand Surgery: A Comprehensive Evidence-Based Guide for the Hand Surgeon.

INTRODUCTION: The Osteochondral Autologous Transfer System (OATS) is a surgical technique initially developed for knee procedures to address focal articular cartilage defects. Although widely applied in the knee, elbow, shoulder, and ankle, its application in hand surgery is limited but growing. This review aims to compile evidence-based insights into OATS for hand surgery, focusing on indications, technical nuances, outcomes, and donor-site morbidity. Additionally, a demonstrative case is presented showcasing the reconstruction of a capitate using OATS. METHODS: The systematic review followe

[Our experience with repair of the scapholunate ligament using the MITEK bone anchor].

PURPOSE OF THE STUDY: A complete rupture of the scapholunate (SL) interosseal ligament results in palmar flexion of the scaphoid and dorsiflexion of the lunate that leads to disintegration of the carpal circle and the development of scapholunate dissociation with dorsal intercalary segment instability. If the injury is not treated properly or early, the abnormal position of the scaphoid and lunate results in degenerative changes of the wrist shown by X-ray and referred to as scapholunate advanced collapse (SLAC). The aim of this study was to evaluate the results of our method used for reconstr

Arterial Perfusion of the Medial Femoral Trochlea Flap Examined by Micro-Computed Tomography Angiography.

PURPOSE: The medial femoral trochlea (MFT) flap is a source of vascularized articular cartilage that has most commonly been used for reconstruction of proximal pole scaphoid nonunions and lunate osteonecrosis in Kienbock disease. This study aimed to describe the arterial perfusion of the MFT. METHODS: Selective injection of the descending genicular artery (DGA) was performed with radiopaque dye in eight cadaveric lower extremities. Micro-computed tomography was performed on the medial femoral condyle including the MFT segment, with digital analysis quantifying vessel location and internal dia

How does transtrochanteric anterior rotational osteotomy change the dynamic three-dimensional intact ratio in hips with osteonecrosis of the femoral head?

BACKGROUND: The intact ratio (the ratio of the intact area of the femoral head) on a two-dimensional anteroposterior radiograph is associated with the prognosis of hips with osteonecrosis of the femoral head after transtrochanteric anterior rotational osteotomy. However, changes of the three-dimensional intact ratio during dynamic weight-bearing activity and correlation of the three-dimensional intact ratio with clinical scores are still unknown. METHODS: Kinematics of eight hips with osteonecrosis of the femoral head that underwent anterior rotational osteotomy were analyzed using image-matc

Retrospective study of radial dome osteotomy with volar plate fixation versus K-wires in Madelung's deformity: long-term follow-up.

PURPOSE: The emerging role of the locking plate improved the technique also in corrective osteotomies in Madelung's deformity, but there is a lack of analyses between the fixation techniques, as well as little information in functional outcomes and long-term follow-up. The current study compared the outcomes, pitfalls, and advantages of volar plate and screws fixation versus K-wires fixation in a long-term follow-up. METHODS: Twenty-eight children presenting the "distal radius" variant of Madelung's deformity underwent Vickers ligament release and distal radial dome osteotomy between 2009 and

Modified dome osteotomy and anterior locking plate fixation for distal radius variant of Madelung deformity: a retrospective study.

PURPOSE: Dome osteotomy of the distal radius with Vickers ligament release is an established method of treatment for Madelung deformity. Many different surgical procedures have been proposed in literature but techniques, patient inclusion and efficacy evaluations are heterogeneous. MATERIALS AND METHODS: A retrospective review of children affected by 'distal radius' Madelung deformity and treated with a standardized surgical procedure (modified reverse dome osteotomy of the distal radius and volar fixation with a small locking plate) between 2010 and 2018 at a single center was performed. The

The Effect of Dorsally Angulated Distal Radius Deformities on Carpal Kinematics: An In Vitro Biomechanical Study.

PURPOSE: The purpose was to quantify the effect of distal radius dorsal angulation (DA) on carpal kinematics and the relative roles of the radiocarpal and midcarpal joints during wrist motion. METHODS: Six cadaveric specimens (69 ± 17 y) were mounted at 90° elbow flexion in a custom wrist motion simulator. The wrist was guided through planar passive flexion and extension motion trials (∼ 5°/s). A custom modular distal radius implant was used to simulate native alignment and 3 distal radius DA deformities (10°, 20°, 30°). An optical tracking system captured carpal bone motion, from which radio

Wrist osteoarthritis.

Painful wrist osteoarthritis can result in major functional impairment. Most cases are related to posttraumatic sequel, metabolic arthropathies, or inflammatory joint disease, although wrist osteoarthritis occurs as an idiopathic condition in a small minority of cases. Surgery is indicated only when conservative treatment fails. The main objective is to ensure pain relief while restoring strength. Motion-preserving procedures are usually preferred, although residual wrist mobility is not crucial to good function. The vast array of available surgical techniques includes excisional arthroplasty,

An Unusual Variant of a Floating Forearm Injury: A Case Report and Review.

BACKGROUND: "Floating forearm" injuries, perilunate injuries, with an elbow dislocation, are rare and challenging to treat. METHODS: We present the case of an unusual variant of a floating forearm injury, trans-scaphoid perilunate fracture dislocation and elbow dislocation, with a concomitant open both-bone forearm fracture after a fall from a ladder. Treatment involved irrigation and debridement of the forearm wound, open reduction and internal fixation of the forearm and perilunate injury, and closed reduction of the elbow. RESULTS: At final follow-up, he had returned to his previous occup

Differential strain of the axially loaded scapholunate interosseus ligament.

PURPOSE: To directly measure strain changes in the scapholunate ligament via magnetic resonance imaging (MRI) when axially loading the wrist in the neutral and extended positions. METHODS: Six asymptomatic male volunteers without known history of previous wrist injury were enrolled in this MRI-based study. Each subject underwent 3 MRI scans in a 3T scanner: in resting neutral position, in neutral with axial load applied, and in extension with axial load applied. Axial load was applied via extension of an elastic band with known force/elongation curve. We analyzed images and converted them to

[MRI or arthroscopy in the diagnosis of scapholunate ligament tears in fractures of the distal radius?].

In a prospective study, 45 patients with fractures of the distal radius and radiologically suspected tears of the scapholunate interosseous ligament were examined. Magnetic resonance imaging was performed prior to wrist arthroscopy. The latter examination gave the definite diagnosis. MRI was performed in conventional technique (without contrast medium) in 25 cases and after additional intravenous injection of contrast medium in the remaining 20 patients. The images were obtained with a 1.0-T clinical imager using a T(2)-weighted turbo spin echo sequence (slice 3 mm, transversal) and a FLASH 2D

Dynamic scapholunate instability: results of operative treatment with dorsal capsulodesis.

Nineteen patients underwent 20 dorsal capsulodesis for dynamic scapholunate instability. Seventeen patients (18 wrists) were evaluated by a questionnaire and physical examination after a mean postoperative follow-up period of 34 months (range, 12-65). The diagnosis was based on a combination of characteristic symptoms of scapholunate instability and physical findings consisting of dorsal wrist tenderness at the scapholunate interval and a positive scaphoid shift test. Following surgery, a significant decrease was noted in symptoms of pain and clunking. Functional status was improved postoperat

Silicone synovitis. A perspective.

Silicone implant arthroplasty is, arguably, the most effective treatment for the majority of patients with symptomatic arthritis in the hand and wrist. In 1985 the problem of silicone synovitis was first brought to our attention. Since that time there have been numerous reports on this condition leading to a worldwide trend against the use of silicone implants. However, the true incidence and effects of silicone synovitis have not been clearly defined. For this reason, we have undertaken a survey of all patients who have undergone silicone implant arthroplasty in the wrist and hand in our Unit

Automatic analysis of carpal angles using dynamic CT imaging: Reference values in healthy wrists and assessment of scapholunate ligament injuries.

INTRODUCTION: Dynamic CT imaging is a promising modality for evaluating wrist pathologies like scapholunate ligament (SL) injuries. The primary objective of this study is to extract carpal angles from dynamic CT datasets using an automated motion analysis algorithm to provide reference values for healthy wrist motion. Secondly, the feasibility of this automatic method to detect SL ligament pathology was evaluated. METHODS: Dynamic CT scans of healthy wrists and wrists with arthroscopically-confirmed complete SL injuries (Geissler IV) were analysed. Each scan consisted of one static image and

Bilateral trans-radial trans-scaphoid perilunate fracture-dislocation with bilateral dorsal elbow dislocation: a case report.

BACKGROUND: Bilateral trans-radial trans-scaphoid perilunate fracture-dislocation is one of the less common injuries of the wrist, occurring as a consequence of high-energy trauma. Its coincidence with bilateral elbow dislocation is extremely rare. CASE PRESENTATION: The authors present a case of a 24-year old male of Czech origin who attained this unique injury by falling from a height of 4 m onto both upper extremities. Bilateral open reduction of the wrists using both palmar and dorsal approaches and closed reduction of both elbow joints was performed. The arms were immobilized in casts fo
