Organizational monitoring of patient blood management implementation: Results of an Italian national survey.

BACKGROUND AND OBJECTIVES: Patient blood management (PBM) as a standard-of-care approach, sustained by substantial scientific literature, is now widely supported by national and international recommendations and guidelines. Scientific evidence shows that PBM is a strategic tool for improving clinical outcomes, quality of life and patient safety, as well as reducing inappropriate blood component consumption and lowering healthcare costs. MATERIALS AND METHODS: Following up the previous monitoring results (a report on 2018 data) and considering new international policy documents such as World H

Beyond the Diagnosis: A Comprehensive Case Report on Duchenne Muscular Dystrophy.

BACKGROUND: Duchenne muscular dystrophy (DMD) is an X-linked recessive genetic disorder characterized by the absence of dystrophin, leading to progressive muscle degeneration and generalized weakness. It primarily affects males in early childhood and progresses to loss of ambulation and cardiac and respiratory complications. CASE REPORT: This case report describes a 26-year-old male with terminal-stage DMD who was admitted to the surgical intensive care unit (SICU) in a critically ill state with multiple-organ failure. He was ventilator-dependent via tracheostomy, hypotensive, anemic, and req

Perspectives of healthcare professionals regarding support following adverse events in three public hospitals in Gauteng: two sides of the coin.

PURPOSE: The purpose of this study was to explore and describe the perceptions of support among healthcare professionals (HCPs) following adverse event involvement in three public hospitals in Gauteng, South Africa. BACKGROUND: The impact of adverse events on HCPs remains an underestimated health concern that warrants further attention to enable continued care for patients and successful careers after events. Experiences are magnified by unsupportive work environments, and are evident in increased hostility, blaming, fear of punishment, and reputational harm. While these HCPs require support

A comprehensive review of massive transfusion and major hemorrhage protocols: origins, core principles and practical implementation.

Until the beginning of the century, bleeding management was similar in elective surgeries or exsanguination scenarios: clotting tests were used to guide blood product orders and, while awaiting these results, an aggressive resuscitation with crystalloids was recommended. The high mortality rate in severe hemorrhages managed with this strategy endorsed the need for a special resuscitation plan. As a result, modifications were recommended to develop a new clinical approach to these patients, called "Damage Control Resuscitation". This strategy includes four principles: damage control surgery, mi

Community Health Workers as Support for Sickle Cell Care.

Community health workers are increasingly recognized as useful for improving health care and health outcomes for a variety of chronic conditions. Community health workers can provide social support, navigation of health systems and resources, and lay counseling. Social and cultural alignment of community health workers with the population they serve is an important aspect of community health worker intervention. Although community health worker interventions have been shown to improve patient-centered outcomes in underserved communities, these interventions have not been evaluated with sickle

Common bleeding disorders affecting individuals with Hereditary Hemorrhagic Telangiectasia.

PURPOSE: Hereditary Hemorrhagic Telangiectasia (HHT) is an autosomal dominant disorder affecting vasculature in different organ systems; seen at a rate of approximately 1:5000 in North America. Complications, with significant increases in health service utilization, arise from bleeding and shunts, and are particularly problematic in the lung and liver. Although these patients tend to chronically bleed from the GI tract and nasal cavities, a single bleed from arterio-venous malformations in the lungs or brain can have serious health implications and may be fatal. Bleeding due to vascular wall f

Stop holding yourself back.

After working with hundreds of leaders in a wide variety of organizations and in countries all over the globe, the authors found one very clear pattern: When it comes to meeting their leadership potential, many people unintentionally get in their own way. Five barriers in particular tend to keep promising managers from becoming exceptional leaders: People overemphasize personal goals, protect their public image, turn their competitors into two-dimensional enemies, go it alone instead of soliciting support and advice, and wait for permission to lead. Troy, a customer service manager, endangered

Reach, Adoption, and Implementation Strategies of a Telehealth Fall Prevention Program: Perspectives From Francophone Communities Across Canada.

Introduction. A fall may impact a person's physical, emotional, and psychological well-being. Fall prevention programs are being implemented to reduce these negative outcomes. However, linguistic barriers in health services may reduce access to such prevention programs. A telehealth fall prevention program was designed to increase access to such programs in French for Francophone minority communities in Canada. This capacity-building project aimed to support community partners to deliver this telehealth program and document strategies used to reach, adopt, and implement the program within vari

Central Register of Objections for Deceased Donation in Poland 1996 to 2014: The Country With an Opting-Out System.

BACKGROUND: Organ, tissue, and cell procurement from deceased donors for transplantation requires consent and authorization, documented donor's positive acceptance, or lack of objection to donation expressed while alive (consent). It also requires the fulfillment of other legal conditions required by law, such as person's legal abilities to act in this field or to obtain approval for donation (authorization). Consent to and authorization of donation from deceased donors requires regulations at the national level. Poland developed an opting-out policy with the Transplantation Act in 1996. The C

Septic arthritis of the knee: the use and effect of antibiotics prior to diagnostic aspiration.

Septic arthritis of the native knee joint and total knee arthroplasty both cause diagnostic and treatment issues. There is no gold standard test to diagnose a joint infection and the use of joint aspiration is commonly relied on. It is widely accepted by orthopaedic surgeons that antibiotics should be withheld until aspiration has been performed to increase the odds of identifying an organism. Patients often present to other specialties that may not be as familiar with these principles. Our study found that 25 (51%) of the 49 patients treated for septic arthritis of the native or prosthetic kn

Prostatic Abscess in a 45-Year-Old Male: Atypical Presentation and Successful Management With Open Transperineal Drainage.

A prostatic abscess is a rare but serious urological emergency that requires prompt recognition and intervention due to associated risks of urosepsis, multi-organ failure, and death. This case describes a previously healthy 45-year-old male who experienced delayed diagnosis and management of a complex prostatic abscess following multiple encounters with secondary care services. Imaging revealed a prostatic abscess with atypical features, including multiloculation and extension into the ischiorectal fossa and gluteal region. Given the extensive nature of the infection and preoperative signs of

Urban green roofs host intrinsic resistomes shaped by management but not dominated by pathogenic resistance.

BACKGROUND: Urban green roofs are increasingly introduced to enhance urban biodiversity and ecosystem services, yet their role in shaping antimicrobial resistance in cities remains unclear. Using long-read metagenomic sequencing, we characterized antimicrobial resistance genes (ARGs) across an experimental extensive green roof system with plots under four different management regimes specifically designed to test the influence of vegetation and organic amendments, as green waste, which although widely used to improve substrate quality, has been flagged as a potential ARG source. RESULTS: We d
