Sudden rupture of the internal maxillary artery causing pseudoaneurysm (mandibular part) secondary to subcondylar mandible fracture.

The mandible is one of the most frequently fractured bones of the face. Condylar fractures are the most common mandibular fractures in adults. The potential complications of mandibular fractures are infection, paresthesia, malunion, nonunion, asymmetry, and long-term functional and growth disturbances. In this article, we report an uncommon case of life-threatening pseudoaneurysm of the internal maxillary artery secondary to subcondylar fracture. We believe that the subcondylar fracture on the vertical vector and oblique direction can damage vital tissues such as the internal maxillary artery,…

[A case of greater auricular nerve autologous nerve grafting for inferior alveolar nerve injury by mandibular fracture].

The results of peripheral nerve repair have been greatly improved in the last few years following the introduction of micro-surgery and increased application of free autologous nerve transplants. In the field of oral surgery, a rich experience has been made in plastic and reconstructive repair. The inferior alveolar nerve is endangered by a series of mandibular fractures, with fracture lines running along the nerve canal. For plastic repair of the inferior alveolar nerve, we interpose an autologous transplant from the greater auricular nerve.

Restoring premorbid occlusal relationship with 3D-printed occlusal splints in treatment of extensive comminuted mandibular fractures: an exploratory technical report.

BACKGROUND: Extensive comminuted mandible (ECM) fractures with dentition defects pose challenges for occlusal restoration. In selected severe cases with extensive comminution and compromised dentition, conventional MMF may be difficult to apply effectively. This study aimed to evaluate the applicability and outcomes of using three-dimensional (3D) printed occlusal splints for occlusal restoration in ECM fractures. METHODS: This retrospective, non-randomized cohort study with a small sample size was conducted involving 22 patients with ECM fractures. Twelve patients were treated with 3D-printe

Comparative effectiveness of one versus two miniplates in the management of mandibular angle fractures: a systematic review and meta-analysis.

Mandibular angle fractures constitute 20%-30% of mandibular fractures. The optimal fixation method remains debated, with some advocating a single non-compression miniplate for simplicity and others favouring two plates for stability. A PRISMA-compliant systematic review and meta-analysis (PROSPERO: CRD42024559840) was performed across PubMed, Scopus, Web of Science, and Cochrane CENTRAL up to July 2024. Eligible studies compared single versus dual miniplate fixation in adult isolated mandibular angle fractures. Risk ratios (RR) with 95% confidence intervals (CI) were pooled using random-effect…

Biomechanical evaluation of two plating systems for fixation of mandibular condylar process fractures in dogs demonstrates sustainability under bite forces.

OBJECTIVE: To determine the biomechanical strength of 2 different condylar titanium plating systems (trapezoid and lambda condylar plates) in medium- to large-breed dogs for repair of mandibular condylar fractures under physiologic bite forces. METHODS: An experimental study design was utilized in which mandibular condylar process fractures were simulated in medium- to large-breed adult dog cadaver heads. The selected implant was applied to the simulated fracture. The jaws were loaded in a load-to-failure test simulating a bite on a unilateral object while measuring bite force. RESULTS: In 1

Is manual reduction adequate for intraoperative control of occlusion during fixation of mandibular fractures? A systematic review and meta-analysis.

The aim of this systematic review was to find out if manual intraoperative control of occlusion is adequate for the reduction of mandibular fractures in comparison with intermaxillary fixation (IMF). We searched PubMed, Embase, the Cochrane Library and Clinical Trials Registry, and the references of included trials. Our primary outcomes of interest were the reduction of fracture anatomically and radiographically, occlusal disturbances, and the incidence of revision procedures due to poor occlusion or reduction. Our secondary outcomes of interest were operating time and infective complications.…

Computer-Aided Surgical Simulation in Severe Atrophic Mandibular Fractures: A New Method for Guided Reduction and Temporary Stabilization Before Fixation.

In severe atrophic mandibular fractures, the absence of stable occlusion, as well as poor bone volume at the fracture site, constitutes a lack of references during surgery to achieve accurate fracture reduction. Unsuccessful reduction and stabilization can lead to several complications, such as fracture misalignments, malunion, and facial asymmetry. An alternative to avoid the aforementioned issues is fracture reduction based on virtual surgical planning (VSP) technology. However, there is little information regarding VSP protocols for the management of mandible fractures. Herein, a new techni…
