PURPOSE: Originally introduced for mandibular reconstruction more than 40 years ago, the sternoclavicular graft (SCG) has gained widespread popularity for the reconstruction of the ramus-condyle unit (RCU) owing to its anatomic and histological likeness to the normal mandibular condyle. Conventional longitudinal osteotomy design for its harvest has been fraught with considerable complications at the donor site including fracture clavicle and major neurovascular injury. In an attempt to alleviate these ill effects, a new technique for procuring the sternoclavicular graft is presented.

Edentulous patients require an adequate rehabilitation so that the alveolar ridge in the interforaminal region be restored for subsequent implant-supported overdentures. The ultimate goal of distraction is to reconstruct the alveolar ridge to a suitable height and width compatible with Atwood class 2 in an appropriate direction allowing the sagittal interalveolar relation to be normally restored. Methods. A 65-year-old man presented with a grade 4 Mandibular atrophy as per Atwood classification which resulted in unsatisfactory treatment with full dentures. OBJECTIVE: To observe the three-dimensional positional relationship between impacted mandibular third molars (IMTMs) and mandibular canal close contacts using cone beam computed tomography (CBCT).
METHODS: A total of 101 patients with IMTMs were selected who met the diagnostic criteria for 142 teeth (no bone wall imaging area between IMTMs and the mandibular canal, a high-density bone cortical imaging area only, or a ≦1 mm bone imaging area). The parameters of the rotating CBCT anode were set as follows: 110 kV, 40-50 mA; the focal point and exposure field were set as 0.3 mmh and a high-resol

Hemangiopericytoma (HPC) is an uncommon vascular tumor arising from pericytic cells with variable malignant potential. Primary HPCs of the bone are extremely rare; however, involvement of the condylar process has not been reported. We presented a 21-year-old female patient with low-grade malignant HPC in the right mandibular condyle. Clinical examination and imaging findings revealed a well-defined soft mass, encapsulating the mandibular condyle from behind. The lesion and mandibular condyle were removed simultaneously; then, the costochondral graft was used to reconstruct the defect. Histopat

In the extraoral approach for mandibular fractures, facial scars and facial nerve injury are common complications. Herein, we introduce an improved surgical technique to reduce fragments of infected comminuted mandibular fractures with the assistance of computer software. Firstly, the model was reset by 3-dimensional (3D) printing, and then the titanium plate was reconstructed on the model. Finally, open reduction and internal fixation through intraoral approach. This method can effectively control infection, reduce facial scars and avoid the risk of facial nerve injury.

The three-dimensional distribution (3D) of the highest mineralized cortical bone was evaluated in 32 subjects between the ages of 8 and 30 years with asymmetrical mandibles using quantitative computed tomography (QCT). The geometrical distribution of the highest mineralized areas (> 1250 mg/cm3) representative of mandibular cortical bone was determined by 3D reconstruction of the images. The length of the mandible on each side was determined by defining a new linear measurement from the centre of the 3D reconstructed condyle to the midline of the symphysis as identified from a submental view.

This retrospective study aimed to analyze changes in the mandibular angle, based on the ostectomy level of the proximal segment following intraoral vertical ramus osteotomy (IVRO) for mandibular setback in 40 patients with mandibular prognathism, using three-dimensional reconstruction images obtained from cone-beam computed tomography. The ostectomy level of the proximal segment was categorized into two groups based on the distance from the gonion to the ostectomy site: within 5 mm and over 5 mm. A statistically significant increase in the mandibular angle was observed in both hard and soft ti

PURPOSE: Maxillary and mandibular alterations in children affected by anterior synostotic plagiocephaly have, until today, received only scarce attention. The goal of this paper is to focus on the mandibular alterations, persisting after the early correction of anterior plagiocephaly during the first year of life.
METHODS: Maxillary and mandibular metric measurements were performed on high resolution, thin slices CT scans; all images were reconstructed into 3D models which were used to perform the metrical assessments.
RESULTS: Twelve patients were selected; all of them had been treated in e

A mandibular continuity defect can be repaired using either a prosthetic device or autogenous bone. A titanium reconstruction plate can be used with a localized or vascularized flap over the defect of the mandible. Unfortunately, the plate may fail due to plate exposure, screw loosening, fracture, or infection, and will need to be removed. Plate exposure though the skin or mucosa is one of the main reasons for failure. In the present work, the authors introduced a lingually positioned reconstruction plate fabricated via three-dimensional printed bending support. This custom reconstruction plat

Orocutaneous fistulas in the maxillofacial region may be due to tumor resection, osteoradionecrosis, or trauma, and these defects limit the function of the patients and effect aesthetic and also psychological condition. Articulation and nutrition are also affected by these fistulas. Local flaps can be used for the reconstruction of small- and medium-sized defects with ease. Submandibular gland with its rich blood supply from the facial artery is a practical and useful choice for the reconstruction of mandibular region defects.

Homologous bone grafts were used in 135 maxillofacial surgical procedures, including acute midfacial fracture repair (n = 77), elective osteotomies of the facial bones (n = 35), secondary correction of traumatic deformities (n = 6), mandibular reconstruction (n = 10), facial bone augmentation (n = 5), and reconstruction of maxillary tumor defects (n = 2). Postsurgical complications occurred in five of the patients. This article reviews the rationale for using homologous bone grafts, their immune response, how they heal, and the risk of transmission of disease.

Temporomandibular joint (TMJ) reconstruction with alloplastic prosthesis is a commonly performed procedure, which can be used as a treatment for improving pathologic conditions affecting the TMJ. Depending of the severity of the condition, the patient's ability to eat can be impaired, which in turn can affect their quality of life. Several syndromes can affect the patient's facial features, and some of those also affect TMJ's. Use of alloplastic prosthesis in TMJ's affecting syndromes, by means of correcting TMJ conditions and mandibular deformities, is still rather uncommon. Purpose of our st

INTRODUCTION: This case report describes the treatment of an infected mandibular angle fracture 50 days after wisdom tooth removal.
CASE PRESENTATION: A 57-year-old woman with a history of mandibular right wisdom tooth extraction 50 days earlier presented to our clinic with restricted mouth opening, significant pain, paresthesia in her right chin, and fever (temperature, 38.5 °C). Intraoral examination revealed malocclusion and an open purulent laceration of the soft tissue in the 48 region. Orthopantomography and computed tomography revealed a displaced fracture of the right mandibular angle

BACKGROUND: Disarticulation is indicated when pathologic process involves the condyle or when a body/ramus lesion extends into condylar process. The goal of reconstruction is to reinstate joint mechanism for function and also to restore facial symmetry.
AIM: Retrospective evaluation of functional and aesthetic outcomes in patients with disarticulation defects of mandible treated with concomitant alloplastic temporomandibular joint replacement and microvascular free flap reconstruction.
MATERIALS AND METHODS: Nine patients (8 benign pathologies and 1 secondary reconstruction for malignant neo

This retrospective study was designed to assess the efficacy of alveolar cleft reconstruction using bone grafts derived from the posterior region of the mandibular molar and to compare the osteogenic properties between block bone grafts and sandwich bone grafts (a structure consisting of "cortical bone + cancellous bone + cortical bone"). A total of 22 patients were involved and divided into two groups based on the type of bone graft used. The primary outcome measures included the assessment of cleft region volume, filling rate, osteogenesis rate, and resorption rate at six months postoperativ

PURPOSE: Reports on treatment of "infected" mandibular fractures with open reduction and rigid internal fixation (RIF) consist of either isolated cases or mixed series of patients with soft tissue infections or inadequately documented osteomyelitis. The definition of "infected fracture" is often vague and may include both soft tissue or bone sepsis.
MATERIALS AND METHODS: In this retrospective study, seven patients with mandibular fractures and documented osteomyelitis were treated by a protocol that included open reduction and RIF. Technetium-99m methylene diphosphonate (99mTc) and Indium 11

Computer-aided microvascular mandible reconstruction is an increasingly common procedure in oral and maxillofacial surgery. The aim of this retrospective single-centre study was to evaluate the rate and specifics of hardware removal after fibula free flap (FFF) fixation with a patient-specific reconstruction plate. The study included patients who underwent hardware removal between April 2017 and October 2019. Statistical analyses were performed regarding the different indications for plate removal (dental implantation versus complication) and the surgical approach (intraoral versus extraoral).
