Maternal Deaths Following Severe Maternal Morbidity During Childbirth in Canada.

Severe maternal morbidity (SMM) includes potentially life-threatening illnesses during pregnancy, childbirth, or the postpartum period. Our objective was to describe SMM case fatality rates in Canada from 2015/16 to 2021/22, including SMM-specific mortality rates and provincial/territorial variations. We studied all hospital deliveries in Canada (excluding Québec) with data obtained from the Canadian Institute for Health Information. Among 1 917 857 women with a hospital childbirth, 35 432 experienced SMM (18.5 per 1000 deliveries) and of these, 75 died before hospital discharge (2.12 deaths p

Improving Maternal Care in Florida Through Levels of Maternal Care Verification.

BACKGROUND: In response to increasing maternal morbidity and mortality, the American College of Obstetricians and Gynecologists (ACOG) and the Society for Maternal Fetal Medicine (SMFM) developed an Obstetrical Care Consensus on Levels of Maternal Care (LOMC) outlining four levels of care. This standardization reduces preventable morbidity and mortality by establishing hospital capabilities to ensure the provision of risk-appropriate care. LOCAL PROBLEM: In 2020, Florida's Maternal Mortality Review Committee found that 68% of pregnancy-related deaths were preventable, with 22% attributed to s…

Proposals for improving maternal safety (2023 edition): Insights from the analysis of maternal deaths in Japan.

The maternal mortality rate remains approximately 4 per 100 000 deliveries. Between January 2010 and July 2024, 629 maternal deaths were reported, of which 590 were reviewed. The Maternal Safety Proposal summarizes these cases. Deaths from obstetric hemorrhage decreased from 28% in 2010 to 7% in 2019 but rose to approximately 20% in 2022 and dropped to 10% in 2023. In the past 4 years, suicide has surpassed obstetric hemorrhage as a leading cause of death. In 2023, intracranial hemorrhage/infarction became the leading cause, with six cases reported. Cardiopulmonary collapse from amniotic fluid…

[Maternal deaths due to infections in France 2013-2015].

Over the 2013-2015 period, maternal mortality due to infections accounted for 10 % of direct maternal deaths and 13 % of indirect maternal deaths. Among the 21 deaths from infection, and compared to the last triennium, maternal deaths from genital infection doubled with 11 deaths during the 2013-2015 period. This included 6 cases of puerperal toxic shock syndrome, 4 of which due to Streptococcus A, and 5 cases of sepsis caused by intrauterine infection due to Gram-Negative Bacillus. Indirect maternal deaths due to infections from extragenital sources represented 10 deaths in this triennium, in…

[Precarious situation of obstetric practice in Gabon].

In 1993, 2,008 deliveries were recorded at the Provincial Maternity Hospital at Franceville in Gabon. The frequency of cesarean section was 3%. The perinatal mortality of children born by cesaraean section was high, 213 per thousand. The principal indications for cesarean section were the baby being too large to pass through the pelvis, bicicatricial uterus, breech presentation and toxemia during pregnancy. The maternal mortality rate was 200 per 100,000, similar to the rates of most African countries, and 75% of the women that died had undergone cesarean section. The mortality rate for cesare…

Perinatal mortality in the Federation of Bosnia and Herzegovina.

AIM: The aim of the study was to investigate the most frequent causes of perinatal mortality in Bosnia and Herzegovina. METHODS: We analyzed in a retrospective study over a one year period (1999) the following cantons: Sarajevo, Mostar, Tuzla, Bihać, Gorazde, Travnik, Zenica, which represent about one half of the population of Bosnia and Herzegovina. RESULTS: Perinatal mortality in the analyzed regions within Bosnia and Herzegovina was 19.55@1000, which is unacceptable in comparison with developed countries. Early neonatal mortality (9@1000) was lower than late fetal mortality 10.55@1000. Th

Out-of-maternity deliveries in France: A nationwide population-based study.

INTRODUCTION: In France, many maternity hospitals have been closed as a result of hospital restructuring in an effort to reduce costs through economies of scale. These closures have naturally increased the distance between home and the closest maternity ward for women throughout the country. However, studies have shown a positive correlation between this increase in distance and the incidence of unplanned out-of-maternity deliveries (OMD). This study was conducted to estimate the frequency of OMD in France, to identify the main risk factors and to assess their impact on maternal mortality and …

[Trends and causes of maternal deaths from 1990 to 2018].

BACKGROUND: Previous studies have shown a decrease in the maternal mortality (MM) rates in Chile, with a trend towards stability since 2001. However, some of its associated causes such as high blood pressure, obesity, or maternal age, have increased in the last years. AIM: To describe the trend and characteristics of MM in Chile between 1990 and 2018. MATERIAL AND METHODS: MM rates were calculated using death records available at the website of the Department of Health Statistics of the Ministry of Health, using the codes 630 to 679 of the International Classification Diseases (ICD)-9 (630-6…

Ending preventable maternal deaths in Malawi: the stakeholders consensus approach to identify maternal health needs priorities.

BACKGROUND: The Malawian context presents multifactorial challenges that hinder the provision of high-quality maternal health services, leading to high maternal morbidity and mortality rates. Priority setting is a crucial concept that can benefit the healthcare system in Malawi by efficiently distributing limited resources and maximising gains in maternal healthcare. We undertook a national maternal health prioritisation exercise involving key stakeholders to enhance delivery of maternal care in Malawi. METHODS: During group discussions, the Nominal Group Technique was adapted to facilitate c

The role of modeling in evaluation of maternal and child health programs: using the lives saved tool to help answer core evaluation questions.

This paper explains how The Lives Saved Tool (LiST), a computer-based model that estimates the impact of scaling up interventions on stillbirths, maternal, neonatal and child health, can contribute to evaluations of programs being delivered at scale to improve maternal and child health. LiST can be used to estimate the impact of a program in advance, allowing planners to refine, streamline and set appropriate program targets. LiST can also be used to estimate the impact of a program, which is particularly useful given the high costs of measuring changes in population health. Finally, LiST can

Assessing quality of childbirth care provided by skilled health personnel in Exemplar countries.

The world has made progress in increasing the number of births attended by skilled health personnel (SHP), but maternal and neonatal mortality have not declined proportionately. This may indicate that quality of care has not risen at the same rate as SHP coverage. Several 'Exemplar' countries have been previously recognized for outstanding progress in improving maternal and newborn health, and offer a unique opportunity to understand coverage and quality of care provided by SHP. Here we describe quality of care and how it compares with SHP coverage in three Exemplar countries-Nepal, Senegal an

Maternal deaths associated with eclampsia in South Africa: Lessons to learn from the confidential enquiries into maternal deaths, 2005 - 2007.

Eclampsia is the commonest direct cause of maternal death in South Africa. The latest Saving Mothers Report (2005-2007) indicates that there were 622 maternal deaths due to hypertensive disorders of pregnancy. Of these, 334 (55.3%) were due to eclampsia; of the eclamptic deaths, 50 were over the age of 35 years and 83 were under 20 years old. Avoidable factors involved patient related factors (mainly delay in seeking help), administrative factors (mainly delay in transport) and health personnel issues (mainly due to delay in referring patients). The major causes of death were cerebrovascular a
