PROPHYLACTIC VS. SYMPTOMATIC THIRD MOLAR REMOVAL: EFFECTS ON PATIENT POSTOPERATIVE MORBIDITY.

PURPOSE: The present study aimed to assess differences in postoperative morbidity between prophylactic and symptomatic third molar removals, and to assess the effect of age on the recovery of the patient. METHODS: Patients admitted for third molar removal were prospectively followed up four times during treatment in context of the M3BE study. Data were collected through pre-, peri and postoperative surveys (days 3 and 10). Uni- and multivariable logistic regression was used to assess the probability of postoperative symptoms of discomfort on day 3 and day 10 according to several patient- and

Gamma Knife Radiosurgery for Short Unilateral Neuralgiform Headache Attacks with Conjunctival Injection and Tearing (SUNCT) Syndrome: Targeting the Trigeminal Nerve and the Sphenopalatine Ganglion. Case Report and Literature Review.

BACKGROUND: Short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT) is a primary headache syndrome with an unclear pathogenesis, and only in very few cases, SUNCT is secondary to known lesions (secondary SUNCT). Several pharmacological as well as interventional and invasive treatments have been used to treat SUNT cases, with no definitive results. We describe a patient with idiopathic SUNCT syndrome, successfully treated with gamma knife radiosurgery and we report a review of the cases of the literature treated with radiosurgery. CASE REPORT: A 6

Correction of proptosis and zygomaticomaxillary asymmetry using orbital wall decompression and zygoma reduction in craniofacial fibrous dysplasia.

Craniofacial fibrous dysplasia is associated with facial asymmetry, orbital dystopia, and orbital proptosis. Although radical excision of the affected bone with immediate craniofacial reconstruction is usually the best treatment option, complete excision may be impossible in some patients because of involvement of the skull base, including the sphenoid and ethmoidal bones. In adult patients without functional loss or rapid disease progression, a shaving procedure is the optimal alternative for fibrous dysplasia affecting the maxilla and zygoma areas. Lesions in the skull base causing exophthal

Microvascular Decompression for Trigeminal Neuralgia Secondary to Vertebrobasilar Dolichoectasia: Review of the Literature and Illustrative Case.

Classical trigeminal neuralgia (TN) is a chronic pain disorder characterized by severe, unilateral facial pain, often resulting from vasculonervous conflict. A less common cause of TN is vertebrobasilar dolichoectasia (VBD). Microvascular decompression (MVD) is the preferred surgical intervention for TN, but in case of VBD, the surgical procedure is more complex due to the aberrant vascular anatomy. This study aims to review the evidence on MVD for VBD-induced TN, and analyze operative challenges, efficacy, and postoperative outcomes. An illustrative case is also presented. We report on the ca

Characterization of different paresthesias following orthognathic surgery of the mandible.

PURPOSE: Paresthesia is a well known consequence of peripheral nerve injury. However, the neural mechanisms of the 2 recognized types, spontaneous and elicited, are currently unknown. This study aimed to investigate these 2 paresthesias and the possible mechanisms accompanying orthognathic surgery. PATIENTS AND METHODS: Mechanical-touch thresholds and current perception threshold were measured before and 7 days after surgery in 60 chin sites (mental nerve area) of 30 patients who underwent orthognathic surgery. Similar testing was conducted on healthy volunteers (controls). All sites were cla

Subacute Onset of Perineural Rhino-Cerebral Mucormycosis Mimicking Giant Cell Arteritis: A Case Report.

A diabetic woman in her 40s presented with a subacute onset of a left temporal headache with no other symptoms, especially no visual or nasal symptoms. An initial diagnosis of giant cell arteritis was made due to a raised erythrocyte sedimentation rate (ESR) and the presence of jaw claudication; however, the temporal artery biopsy was negative. Corticosteroid therapy was commenced, but subsequent computed tomography (CT) imaging of the face revealed extensive left maxillary sinus disease with bony erosion of the greater palatine and vidian nerve canals. The ear, nose, and throat (ENT) examinat

Transmaxillary Approach for the Resection of Inferior Orbital Venous Varix: Technical Case Instruction.

BACKGROUND AND IMPORTANCE: The optimal surgical approach for managing orbital venous varices, particularly those involving the inferior orbit, remains controversial. A transmaxillary approach offers a less invasive alternative to the transnasal approach and avoids disruption of the nasal cavity and medial maxillary wall. This report describes the details of a technique for ligating a venous varix between the inferior rectus and medial rectus muscles that aims to provide relief of symptoms and prevent recurrence. CLINICAL PRESENTATION: A 77-year-old man presented with progressive left eye pain

Modular organization of the head retraction responses elicited by electrical painful stimulation of the facial skin in humans.

OBJECTIVE: To explore whether the trigeminocervical reflexes (TCRs) show a reflex receptive field organization in the brainstem. METHODS: The facial skin of 16 healthy subjects was electrically stimulated at nine sites reflecting the distribution of the three branches of the trigeminal nerve. The reflex-evoked EMG responses were measured bilaterally from the neck muscles and the head and neck kinematic reactions were detected. RESULTS: TCRs are site dependent. There was a vertical gradient in the magnitude of the reflex responses. EMG and kinematic reflexes were larger when evoked from ophth

Surgical treatment of craniofacial fibrous dysplasia in adults.

Craniofacial fibrous dysplasia (FD) is a rare disorder that may require neurosurgical expertise for definitive management; however, surgical management of FD in adult patients is uncommon. Although other therapies have been shown to slow progression, the only definitive cure for adult craniofacial FD is complete resection with subsequent reconstruction. The authors review the biological, epidemiologic, clinical, genetic, and radiographic characteristics of adult FD, with an emphasis on surgical management of FD. They present a small series of three adult patients with complex FD that highlight

Endoscopic transnasal study of the infratemporal fossa: a new orientation.

INTRODUCTION: The medial portion of the infratemporal fossa (ITF) is not infrequently involved in sinonasal and skull base pathologies. However, endoscopic view of the ITF remains unclear with lack of studies addressing this region from the endoscopic perspective. METHODS: Using an extended endoscopic approach, the pterygopalatine and infratemporal fossae were dissected in 10 sides of five adult cadaver heads. A plane of dissection along the pterygoid base and the infratemporal surface of the greater sphenoid wing was developed. High-quality images were produced by coupling the video camera t

[Experimental studies on tissue distribution of 14C-labelled antimicrobial agent in the otorhinolaryngological field--application of macro-autoradiography].

The distribution of antimicrobial agents in infected target tissues would be essential for their application to treat the sites of infection. Taking accounts of the view point, we had reported the distribution of 3 nalidixic acid analogs in tissues which are related to otorhinolaryngology by macro-autoradiography and radiometry. We investigated here in the distribution of a nalidixic acid analog OPC-7241. This agent is 9-fluro-5-methyl-8-(4-methyl-1-piperaginyl)-6, 7-dihydro-1-oxo-1H, 5H-benzo [ij]quinoligine-2-carboxylic acid supplied by Otsuka Pharmaceutical Co., Ltd. Autoradiography was per

Functional Arterial Anatomy of the Cranial Base.

Two main vascular routes supply the brain, internal carotid arteries (ICAs) and vertebral arteries. The external carotid artery (ECA) mainly supplies the skin, muscle, and skull; thus, neurointerventionalists should be aware of ECA-ICA, ECA-vertebral artery, and extracranial-intracranial anastomoses. These anastomoses are not newly formed but are arteries that are remnants of developmental processes. During development, the ICA has anastomoses with the original ECA at the first and second pharyngeal arches, and carotid-basilar anastomoses exist between the ICA and vertebral artery/basilar arte

[Ectopic tissue of the thyroid gland and the parathyroid glands].

Ectopic thyroid tissue results from developmental defects of the early stages of thyroid embryogenesis, in which the median thyroid anlage descends from the floor of the mouth to its final pre-tracheal position. The most common sites of ectopic thyroid tissue are accordingly in the area of the floor of the mouth and in the course of the thyroglossal duct. Rare localizations are intrathoracic (mediastinal, cardiac, pulmonary) and sub-diaphragmatic (including the adrenals, liver, gall bladder, and gastrointestinal tract). The most important differential diagnosis of ectopic thyroid is metastasis

The "black evil" affecting patients with diabetes: a case of rhino orbito cerebral mucormycosis causing Garcin syndrome.

Mucormycosis is a life-threatening infection affecting patients with diabetes. It is an angioinvasive disease often resistant to treatment with a debilitating course and high mortality. Here, we report a case of a 45 year old woman with type 2 diabetes mellitus who presented to us with history of right-sided ptosis and facial palsy, and subsequently developed loss of vision and palatal palsy. She was in diabetic ketoacidosis. Nervous system examination revealed involvement of right second, third, fourth, sixth, seventh, ninth, and tenth cranial nerves, suggestive of Garcin syndrome. The hard p

Malignant schwannoma of the infratemporal fossa: a case report.

INTRODUCTION: Malignant schwannomas or neurofibrosarcomas are rare nerve tumors of unknown etiology. These neoplasms are highly aggressive with a marked propensity for local recurrence and metastatic spread. Their management continues to be a challenge for pathologists and surgeons. Maxillofacial locations are very exceptional. We report the case of a patient with unusual malignant schwannoma of the infratemporal fossa discovered at a late evolving stage. CASE PRESENTATION: A 56-year-old woman, of Moroccan nationality, presented to our hospital in 2013 with a large right-sided hemifacial swel

Efficacy and safety of intraseptal and periodontal ligament anaesthesia for extraction of upper lateral incisors and lower first premolars in patients with type 2 diabetes mellitus.

INTRODUCTION: The use of local anesthetics with vasoconstrictors in patients with Type 2 diabetes mellitus (DM) requires particular caution because of possible cardiovascular effects and increased susceptibility of diabetic nerve fibers to ischemic injury. Intraseptal anaesthesia (ISA) and periodontal ligament anaesthesia (PLA) may represent effective alternatives to conventional local anaesthesia in diabetic patients. This study aimed to compare the clinical efficacy and safety profiles of ISA and PLA administered with a computer-controlled local anesthetic delivery system (CCLADS) for extrac

Outcome of Temporalis Myofascial Flap in Maxillary Reconstruction.

The maxilla is an important component of the midfacial skeleton from both an aesthetic and functional view point. In addition to cosmetic impairment, surgical procedures in this region have substantial ramifications for speaking, swallowing and mastication, all of which may lead to psychological anguish for the patient. This necessitates rehabilitation with an obturator prosthesis or surgical reconstruction. The temporalis muscle flap has considerable potential for reconstructing various maxillofacial area abnormalities. It appears to be one of the finest solutions among all other regional fla
