Who's hurt most by debt? Life course adversity and the debt gap in midlife health.

BACKGROUND: Does the association between indebtedness and midlife health, that is, the debt gap in health, vary systematically by individuals' propensity for becoming indebted? To answer this question, we investigated potential heterogeneity in the debt-health association and tested competing hypotheses derived from cumulative inequality and relative deprivation. METHODS: With data from the National Longitudinal Survey of Youth 1979 (NLSY79), we measured indebtedness as self-reported negative net worth. Midlife health was assessed using SF-12 mental and physical health scores at age 50. The a

[The role of artificial intelligence in psychiatric diagnosis and classification: A state-of-the-art review in an uncharted domain].

BACKGROUND: The deployment of artificial intelligence (AI) is creating new opportunities for sophisticated classification and diagnosis across mainstream healthcare. Although the technology shows substantial promise, its translation into mental-health practice is hampered by a distinct set of additional challenges. AIM: To elucidate the current state of AI-driven diagnostic methods within psychiatry. METHOD: Reflective essay in which we as experts in psychiatry, AI technology, data innovation, ethics and jurisprudence combine our perspectives. RESULTS: Technological progress is advancing mo

Unbreakable in Crisis: A Systematic Review Exploring Nurse Resilience and Contributing Factors During the COVID-19 Pandemic.

The coronavirus disease 2019 (COVID-19) placed an unprecedented burden on the global healthcare system, severely affecting the physical and mental health of healthcare workers, particularly nurses. Nurses faced immense workloads, increased infection risks, uncertainty, and public scrutiny. Despite these challenges, nurses were lauded for their dedication and resilience in confronting the pandemic. Many experienced mortality, morbidity, and post-COVID sequelae. This review integrates psychological resilience literature from 2020 to 2022, utilizing Whittemore and Knafl's integrative review metho

Gender identity and activity limitations: A national study on transgender and non-binary Canadians.

INTRODUCTION: Activity limitations among transgender and non-binary individuals remain largely unexplored using population-based samples. This study examines the risk of activity limitations across different gender identities in Canada. METHOD: Using data from the 2021 Canadian long-form Census, logistic regressions estimated the adjusted odds of reporting activity limitations (seeing, hearing, mobility/dexterity, mental, cognitive, and other) across gender identities. RESULTS: Non-binary individuals assigned female at birth (AFAB) had the highest predicted probability of reporting any activ

Can you See What We See? African American Parents' Views of the Strengths and Challenges of Children and Youth Living with Adversity.

A premise of positive youth development is that social competencies can develop in adversity and co-exist with problem behaviors. This research tested whether African American youth ages 9-17 who had experienced significant family stressors would form groups that displayed combinations of adversity, problem behavior, and strengths. Parents of a nationally representative sample of African Americans children were interviewed on child difficulties and strengths as part of the CDC's 2019 National Health Interview Survey. About 15% of the national sample of African American youth had experienced vi

Pushing them to the edge: Suicide in immigrant detention centers as a product of organizational failure.

In this paper, we argue that the U.S. immigrant apparatus is a racial project that jeopardizes immigrants' wellbeing through organizational failure (Omi and Winant, 2014; Meyer & Rowman, 1977; Mellahi and Wilkinson, 2004). We utilize Provine and Doty's (2011) work as a foundation to understand how this racial project is systemic and multifaceted in nature. It begins with the negative characterization and criminalization of certain immigrants, mostly Latinx, followed by a poor infrastructure of processing and detention riddled with impediments to their wellbeing, which ultimately pushes detaine

Excess mortality from breast cancer in female breast cancer patients with severe mental illness.

Women with a history of severe mental illness (SMI) have elevated breast cancer mortality. Few studies have compared cancer-specific mortality in women with breast cancer with or without SMI to reveal gaps in breast cancer treatment outcomes. We compared breast-cancer specific mortality in women with or without SMI and investigated effects of stage at presentation, comorbidity, and differences in cancer treatment. Women with their first breast cancer diagnosis in 1990-2013 (n = 80,671) were identified from the Finnish Cancer Registry, their preceding hospital admissions due to SMI (n = 4,837)

Project Kealahou: improving Hawai'i's system of care for at-risk girls and young women through gender-responsive, trauma-informed care.

Project Kealahou (PK) is a six-year, federally-funded program aimed at improving services and outcomes for Hawai'i's female youth who are at risk for running away, truancy, abuse, suicide, arrest and incarceration. PK builds upon two decades of sustained cross-agency efforts among the state's mental health, juvenile justice, education, and child welfare systems to promote system-of-care (SOC) principles of community-based, individualized, culturally and linguistically competent, family driven, youth-guided, and evidence-based services. In addition, PK emphasizes trauma-informed and gender-resp

Predicting ambulatory blood pressure during school: effectiveness of social and nonsocial reactivity tasks in black and white adolescents.

We evaluated a newly developed stress task, the Social Competence Interview, and three nonsocial tasks (video game, mirror drawing, mental arithmetic) for ability to predict ambulatory blood pressure in 237 black and white adolescents. Blood pressure was measured in laboratory, classroom, and transition (between-class) settings. A resting laboratory baseline explained 10-49% of the variance in ambulatory blood pressure levels; the ability of the stress tasks to explain additional variance was assessed in multiple regression analyses. Only the blood pressure response to the interview enhanced p

Involving Independent Mental Capacity Advocates (IMCAs) in Dental Care Decision-Making: A Qualitative Exploration of Dentists' Experiences in England and Wales.

INTRODUCTION: Delivering dental care to individuals who lack decision-making capacity involves complex ethical, legal, and clinical considerations. In England and Wales, Independent Mental Capacity Advocates (IMCAs) are required to support decisions about 'serious medical treatment', a concept given only broad definition in law. While Special Care Dentistry frequently involves patients with cognitive impairments, little is known about how dentists engage with advocates in treatment decision-making. OBJECTIVE: To explore dentists' experiences of appointing and working with advocates in dental

When patients' priorities conflict with those of their medical team; a challenging case of a bleeding patient and his dying pet.

We present an unusual case of an acutely unwell patient with an upper gastrointestinal bleed whose resuscitation efforts were delayed by the discovery of his, similarly, acutely unwell pet on the medical high dependency unit. We highlight the challenges this provided the clinical team and focus on the issues relating to patient safety, consent and multidisciplinary action which may be more relevant to daily clinical practice.

A survey of psychological support provision for people with inflammatory arthritis in secondary care in England.

OBJECTIVES: The consequences of inflammatory arthritis can include depression, anxiety and low mood, reducing patients' quality of life and increasing pressure on the healthcare system. Treatment guidelines recommend psychological support, but data are lacking on the provision available. METHODS: A postal survey concerning psychological support provision was sent to rheumatology units in 143 acute trusts across England. Nurses from 73 rheumatology units (51%) responded. RESULTS: Overall, 73% rated their unit's psychological support provision as 'inadequate' and only 4% rated it as 'good'. Fe

The capacity to give informed consent in a homeless population with developmental disabilities.

This study examined homeless persons with developmental disabilities and their ability to understand the informed consent process for research using a capacity-to-consent screener. Bivariate associations with the inability to pass the screener on the first attempt were noted with three factors: an eighth grade or less in education, chronic homelessness, and mental retardation diagnosis. With multiple regression, the only outcome associated with inability to pass the screener on the first attempt was an eighth grade or less education. This study indicates the need for consideration of developme

Evidence-based psychosocial treatments for ethnic minority youth.

This article reviews research on evidence-based treatments (EBTs) for ethnic minority youth using criteria from Chambless et al. (1998), Chambless et al. (1996), and Chambless and Hollon (1998). Although no well-established treatments were identified, probably efficacious or possibly efficacious treatments were found for ethnic minority youth with anxiety-related problems, attention-deficit/hyperactivity disorder, depression, conduct problems, substance use problems, trauma-related syndromes, and other clinical problems. In addition, all studies met either Nathan and Gorman's (2002) Type 1 or

Clinician perspectives on decision-making capacity after acquired brain injury.

Acquired brain injury frequently alters an individual's ability to make health care decisions based on a clear understanding of the situation and options. This exploratory study investigated the ways health care providers address issues of decisionmaking capacity (DMC) on a daily, functional basis. 33 clinicians providing rehabilitation services to persons with acquired brain injury participated in 1 of 5 semi-structured focus groups. All 33 participants, representing 8 different occupations, agreed that DMC determinations affected their practice every day. Participants underscored a multidime

Ethical issues in dysphagia: when patients refuse assessment or treatment.

The primary goal of intervention for patients with dysphagia is to restore oral feeding. When patients are unable to achieve adequate nutrition, hydration, or safety with oral feeding, then nonoral approaches are often recommended. Although patients' rights to accept or refuse clinical recommendations are widely recognized, when a patient refuses tube feeding or other recommendations, dysphagia specialists are left with a host of practical questions about their role in caring for the patient. We review the criteria for assessing patients' capacity to make informed choices, approaches to decisi

Mental Health Provider Knowledge and Attitudes Toward Diagnosing Anxiety Disorders in the Veterans Health Administration.

Clinical practice guidelines indicate treatments for specific anxiety diagnoses. Misdiagnosing specific anxiety disorders as unspecified anxiety may prevent patients from receiving appropriate care. Provider knowledge and attitudes may influence diagnostic practices. This study evaluated provider knowledge of diagnostic criteria for anxiety disorders and attitudes toward diagnostic processes and the relevance of diagnosis to patients' treatment. This qualitative analysis of interviews included 32 Veterans Health Administration providers in Primary Care Behavioral Health and Specialty Mental He
