Perioperative Management and Anesthetic Challenges of Abdominal Compartment Syndrome: A Case Report.

We present the perioperative management of a patient with abdominal compartment syndrome (ACS) secondary to fecal impaction and bowel perforation. This case highlights the complex challenges associated with ACS in the perioperative setting, particularly the high risk of cardiovascular and respiratory instability. Sudden hemodynamic collapse following induction or surgical incision, as well as complications during decompression, underscore the profound physiologic derangements inherent to this pathology and the critical role of anesthetic planning in patients with this presentation. Key anesthe

Mean Airway Pressure-An Informative but Overlooked Indicator of Mechanical Power.

Mean airway pressure, a monitored variable continuously available on the modern ventilator, is the pressure measured at the airway opening averaged over the time needed to complete the entire respiratory cycle. Mean airway pressure is well recognized to connect three key physiologic processes in mechanical ventilation: physical stretch, cardiovascular dynamics, and pulmonary gas exchange. Although other parameters currently employed in adults to determine "safe" ventilation are undoubtedly valuable for daily practice, all have limitations for continuous monitoring of ventilation hazard. The pu

Approach to the acute abdomen in pregnancy.

Numerous physiologic changes in pregnancy may affect the presentation of abdominal pain in pregnancy. A high index of suspicion must be used when evaluating a pregnant patient with abdominal pain. General anesthesia is considered safe in pregnancy. Intraoperative monitoring and tocolytics should be individualized. Laparoscopic surgery should be performed in the second trimester when possible and appears as safe as laparotomy. If indicated, diagnostic imaging should not be withheld from the pregnant patient. Appendectomy and cholecystectomy appear to be safe in pregnancy. The reported incidence

Management of anesthesia in the foal.

Several unique pharmacologic and physiologic factors must be considered when attempting to anesthetize premature or neonatal foals. Juvenile foals are similar to adults in their physiology and metabolism. Anesthetic drug and protocol selection should reflect the differences between these two age groups. Neonates are best anesthetized using an inhalation technique, whereas older foals can be safely anesthetized with either parenteral or inhalation anesthetic agents. Careful monitoring is absolutely essential when anesthetizing foals. The clinician should plan to routinely administer fluids and

Current status and global trends in self-care among patients with heart failure: challenges and opportunities.

Self-care effectively improves outcomes in adults with chronic conditions such as heart failure (HF). Yet engaging patients in self-care maintenance, self-care monitoring, and self-care management has proven to be difficult for patients and providers. In this review we define self-care as a decision-making process involving the choice of behaviors that maintain physiologic stability and the response to symptoms when they occur. The three core self-care behaviors of maintenance, monitoring, and management are defined and situated within their theoretical context. Available approaches to measure

Monitoring surgery around the cranial nerves.

Intraoperative neurophysiologic monitoring (IONM) of cranial nerve (CN) function is an essential component in multimodality monitoring of surgical procedures where CNs are at risk for injury. In most cases, IONM consists of localizing and mapping CNs and their pathways, and monitoring of CN motor function during surgery. However, CN VIII, which has no motor function, and is at risk for injury in many surgical procedures, can be easily and accurately monitored using brainstem auditory evoked potentials. For motor CNs, the literature is clear that function can be safely and adequately performed

Television-induced electronegative photoparoxysmal response: an extratemporal seizure mimic?

Video-EEG monitoring is an established gold-standard procedure for diagnosis and differentiation of epileptic and non-epileptic seizures. Epilepsy misdiagnosis, to which factors such as EEG artifact misinterpretation contribute to, is common, and can have long-lasting iatrogenic repercussions to the clinical management of affected patients. Among the many types of responses to photic stimulation, artifacts and physiologic and epileptic responses are possible. All of these can interfere with EEG interpretation when provoked by a source of illumination. Photic-induced responses are of increasing

Vascular Imaging of Matrix Metalloproteinase Activity as an Informative Preclinical Biomarker of Drug-induced Vascular Injury.

Lack of biomarkers specific to and either predictive or diagnostic of drug-induced vascular injury (DIVI) continues to be a major obstacle during drug development. Biomarkers derived from physiologic responses to vessel injury, such as inflammation and vascular remodeling, could make good candidates; however, they characteristically lack specificity for vasculature. We evaluated whether vascular remodeling-associated protease activity, as well as changes to vessel permeability resulting from DIVI, could be visualized ex vivo in affected vessels, thereby allowing for visual monitoring of the pa

Impact of Temporal Resolution on Autocorrelative Features of Cerebral Physiology from Invasive and Non-Invasive Sensors in Acute Traumatic Neural Injury: Insights from the CAHR-TBI Cohort.

Therapeutic management during the acute phase of traumatic brain injury (TBI) relies on continuous multimodal cerebral physiologic monitoring to detect and prevent secondary injury. These high-resolution data streams come from various invasive/non-invasive sensor technologies and challenge clinicians, as they are difficult to integrate into management algorithms and prognostic models. Data reduction techniques, like moving average filters, simplify data but may fail to address statistical autocorrelation and could introduce new properties, affecting model utility and interpretation. Normocalcemic parathormone elevation after successful parathyroidectomy: long-term analysis of parathormone variations over 10 years.

BACKGROUND: The long-term significance of normocalcemic parathormone elevation (NPE) after successful parathyroidectomy for sporadic primary hyperparathyroidism remains unclear. METHOD: Of 239 consecutive patients who underwent targeted parathyroidectomy with intraoperative parathormone monitoring, 96 were followed for ≥ 10 years. NPE was defined as a normal serum calcium level and parathormone (PTH) above the normal reference range ≥ 6 months after successful parathyroidectomy. Recurrence was defined as elevated serum calcium and PTH levels ≥ 6 months after parathyroidectomy. Risk factors fo

Prolactin dysregulation in women with subclinical hypothyroidism: effect of levothyroxine replacement therapy.

This study investigated the effect of levothyroxine treatment on serum prolactin (PRL) levels in women with subclinical hypothyroidism. Sixty-six women (mean age, 58.5 +/- 1.3 years) with confirmed subclinical hypothyroidism (mean thyrotropin [TSH], 11.7 +/- 0.8 mIU/L) were randomly assigned to receive levothyroxine or placebo for 48 weeks. Based on blinded TSH monitoring, physiologic levothyroxine replacement (85.5 +/- 4.3 microg/d; TSH, 3.1 +/- 0.3 mIU/L) was ascertained throughout the study. PRL levels were measured before and after administration of thyrotropin-releasing hormone (TRH) at b…

Characterization of long-term outcomes after Toupet fundoplication: symptoms, medication use, and health status.

GOALS AND BACKGROUND: Fundoplication is increasingly used for the treatment of gastroesophageal reflux disease (GERD). Few studies have tracked patient outcomes of the Toupet method for more than 1 year. Further clinical, physiologic, and patient-based outcome measures have not been well characterized for this method. The current study conducts a long-term, comprehensive outcome evaluation in patients receiving Toupet fundoplication. STUDY: Fifty-five patients who had previously undergone fundoplication were examined. In a subset of 24 patients, esophagogastroduodenoscopy was used to assess t

Unlocking body-surface physiological evolution via IR-temperature dual sensing with single chalcogenide fiber.

Improvements to body-surface physiological monitoring ability including real-time, accuracy and integration, are essential to meet the expansive demands for personal healthcare. As part of this, simultaneous monitoring of sweat metabolites and body temperature offers an exciting path to maximizing diagnostic precision and minimizing morbidity rates. Herein, we report a high-performance biomarker-temperature sensor made of a single As3Se5Te2 chalcogenide glass fiber to monitor physiology evolution on body-surface. The sensor integrates efficient thermal resistance and fiber evanescent wave effe

Insight Into the Relationship Between Gastroesophageal Reflux Disease and Asthma.

Gastroesophageal reflux disease (GERD) is a common condition that presents with symptoms of heartburn and regurgitation. Asthma is an equally common medical condition that often coexists with GERD. The clinical scenario of difficult-to-treat asthma in the setting of concomitant GERD leads to the possibility of GERD-induced asthma. However, asthma may also induce GERD, so confusion has developed about the role of GERD in patients with moderate to severe asthma. Acid-suppressive therapy may be initiated in patients with asthma, but controlled studies have recently questioned the role of such the

Epilepsy emergencies: the first seizure and status epilepticus.

Patients experiencing a first seizure need a careful history and examination to confirm that the presenting seizure is truly the initial event and to identify other possible risk factors for recurrence. The decision of whether to start therapy should be made by the clinician and a fully informed patient. Status epilepticus presents in several forms, with generalized convulsive status epilepticus (GCSE) being the most dramatic. Management of GCSE requires life support and monitoring measures as well as timely administration of antiepileptic drugs (AEDs) to terminate the seizure and reduce the r

Management of sickle cell anemia and pregnancy.

There has been significant decrease in maternal morbidity and mortality of sickle cell disease patients during pregnancy due to better understanding of the pathophysiology of the disease and physiologic changes during pregnancy. Prophylactic blood transfusion does not appear to reduce complications in patients with sickle cell anemia. Patients with sickle hemoglobin C disease and with S beta thalassemia+ have fewer complications but still need close monitoring. Blood transfusion therapy should be made available for medical and obstetrical complications to include increasing hypoxemia, progress
