A case of hyperkinetic movement disorder associated with LGI1 antibodies.

Encephalitis associated with leucine-rich glioma inactivated 1 (LGI1) antibodies is often encountered in elderly male patients and may infre … immunotherapy. Here, we present the first pediatric case with LGI1 antibody associated hyperkinetic movement disorders and emphasize the importance of investigating neuronal autoantibodies in patients with isolated and treatment resistant movement disorders.

Morbidities in rapid eye movement sleep behavior disorder.

Idiopathic rapid eye movement (REM) sleep behavior disorder (iRBD, RBD without any obvious comorbid major neurological disease), is strongly associated with numerous comorbid conditions. The most prominent is that with neurodegenerative disorders, especially synuclein-mediated disorders, above all Parkinson disease (PD). Idiopathic RBD is an important risk factor for the development of synucleinopathies. Comorbidity studies suggest that iRBD is associated with a number of other potential pre-motor manifestations of synucleinopathies such as, cognitive and olfactory impairment, reduced autonomi

Risperidone-induced rabbit syndrome: an unusual movement disorder.

Rabbit syndrome is an antipsychotic-induced rhythmic motion of the mouth and lips, resembling the chewing motion of a rabbit. The motion consists of vertical movement; the tongue is not involved. The reported prevalence of rabbit syndrome ranges from 2.3% to 4.4% of patients treated with typical antipsychotic drugs. There have been isolated reports of rabbit syndrome in patients treated with atypical antipsychotics. Rabbit syndrome needs to be closely differentiated from tardive dyskinesia, the tongue-involving movement disorder. Treatment of rabbit syndrome is empirical, reflecting poor under

Movement disorder in ataxia-telangiectasia: treatment with amantadine sulfate.

Ataxia-telangiectasia is a cerebellar neurodegenerative disorder presenting with ataxia, chorea, myoclonus, and bradykinesia. Literature on treatment of movement disorders is scarce. We treated 17 children (aged 11.2 ± 3.9 years) for 8 weeks with the dopaminergic and anti-N-methyl-d-aspartate (NMDA) agent amantadine sulfate 6.3 ± 0.87 mg/kg/d. Ataxia was assessed by using the International Cooperative Ataxia Scale, parkinsonism by the Unified Parkinson Disease Rating Scale, and chorea/myoclonus by the Abnormal Involuntary Movement Scale. Responders were considered those patients who had at lea…

The distinctive movement disorder of ovarian teratoma-associated encephalitis.

The movement disorder observed in four cases of ovarian teratoma associated encephalitis is described. The illness began with neuropsychiatric symptoms and was followed by prolonged unresponsiveness, respiratory failure, and autonomic instability. The movement disorder consisted of semirhythmic repetitive bulbar and limb movements and persisted during prolonged periods of unresponsiveness, diminishing as awareness returned. The characteristics of the movement disorder differed from recognized dyskinesias. It is suggested that interruption of forebrain corticostriatal inputs by anti-N-methyl-D-

Movement and mood disorder in two brothers with Gaucher disease.

Gaucher disease (GD) is a lysosomal storage disorder with a wide spectrum of phenotypic presentations. We report the case histories of two adult brothers with GD who developed both parkinsonism and psychiatric symptoms. Direct sequencing and real-time polymerase chain reaction were used to establish that the patients were homozygous for mutation L444P. While parkinsonism has been described previously in GD, these patients had atypical features, including a complicated mood disorder. The comorbidity of GD and a mood disorder is a new finding, as psychiatric manifestations of GD have been descri

Restless legs syndrome and periodic limb movement disorder.

Restless leg syndrome and PLMD are nocturnal movement disorders associated with significant adverse effects on the health and well-being of patients and their families [66]. Although the pathophysiological basis remains to be fully described, current research points to abnormalities in CNS function and neurotransmitter systems. The accurate diagnosis of RLS and PMD requires a thorough history, physical examination, diagnostic tests, and often, a referral to a sleep disorders specialist. Considering the prevalence of these conditions and their negative impact, nurses should be well-versed in th…

Pharmacologic management of movement disorder after midbrain haemorrhage.

Movement disorders following midbrain haemorrhage are infrequently encountered in rehabilitation, and are uncommonly corrected by pharmacologic means. This report describes a 20 year-old male with a prior history of cocaine abuse who presented with a 4 day history of dysarthria and blurred vision following methamphetamine abuse. Physical examination demonstrated hypertension, left facial hemispasm, bilateral upward gaze paresis and ataxic gait. Magnetic resonance imaging/magnetic resonance angiography (MRI/MRA) showed multifocal parenchymal haematomas in the mesencephalic tegmentum, subcortica…

Profile of patients enrolled in a new movement disorder clinic.

To assess the need for a regional expertise in movement disorders, the numbers of patients, clinic visits, and medication changes for a new movement disorder clinic were recorded. During 3 1/2 years, 355 patients were seen, with 1,329 clinic visits. Idiopathic Parkinson's disease was the most common diagnosis, comprising 36% of the population, followed by dystonia (17%), tremor (12%), parkinsonism (i.e., Parkinson's plus syndromes, drug-induced parkinsonism, etc.) (10%), chorea (10%), Tourette's syndrome (6.5%), and tardive dyskinesia (3.4%). Distribution of follow-up visits was similar, with …

Creutzfeldt-Jakob's disease presenting with ataxia and a movement disorder.

We describe a 62-year-old patient who presented with ataxia and dystonia with mild dementia and was shown at autopsy to have Creutzfeldt-Jakob's disease. Movement disorders may be the presenting feature of Creutzfeldt-Jakob's disease, and this entity should be considered in the differential diagnosis of an otherwise obscure movement disorder even when the typical electroencephalographic pattern is absent.

A possible post-streptococcal movement disorder with chorea and tics.

A 14-year-old girl developed a movement disorder after a streptococcal infection. In the acute phase of the illness she exhibited simple and complex motor tics and chorea, but all abnormal movements ceased over the following eight months, without recurrence. This case raises questions about the relationship between tics, chorea and auto-immune reactivity.

Quantification of tracking eye movements in patients with neurological disorders.

Tracking eye movements were studied in 20 healthy subjects and in 24 patients with vestibular neuronitis, disorders within the frontal cortex, the cerebellum or the brain stem. The tracking tests were performed at six different target velocities between 10-60 degrees s-1 and different parameters were evaluated. The maximum velocity gain of smooth pursuit as normal in the group with vestibular neuronitis but reduced in the groups with disorders within the frontal cortex, cerebellum and the brain stem. The total amplitude of tracking and amplitude of smooth pursuit were normal in the groups with

Repetitive transcranial magnetic stimulation and constraint-induced movement therapy combined in the treatment of post-stroke movement disorders: a narrative review.

Stroke is a significant cardiovascular and cerebrovascular condition and is among the primary causes of prolonged neurological impairment globally. Approximately 55%-75% of stroke survivors will experience some form of long-term sensorimotor impairment. Post-stroke, the upper limb typically exhibits restricted mobility, complicating daily chores for 70% of patients and impairing normal limb utilization. Repetitive Transcranial Magnetic Stimulation (rTMS), a prominent non-invasive neuromodulation technique designed to enhance functional recovery post-stroke, has garnered significant attention i

Translating Genetic Discovery into a Mechanistic Understanding of Pediatric Movement Disorders: Lessons from Genetic Dystonias and Related Disorders.

The era of next-generation sequencing has increased the pace of gene discovery in the field of pediatric movement disorders. Following the identification of novel disease-causing genes, several studies have aimed to link the molecular and clinical aspects of these disorders. This perspective presents the developing stories of several childhood-onset movement disorders, including paroxysmal kinesigenic dyskinesia, myoclonus-dystonia syndrome, and other monogenic dystonias. These stories illustrate how gene discovery helps focus the research efforts of scientists trying to understand the mechani…

Inhibitory dysfunction contributes to some of the motor and non-motor symptoms of movement disorders and psychiatric disorders.

Recently, it has been proposed that similar to goal-directed and habitual action mediated by the fronto-striatal circuits, the fronto-striato-subthalamic-pallidal-thalamo-cortical network may also mediate goal-directed and habitual (automatic) inhibition in both the motor and non-motor domains. Within this framework, some of the clinical manifestations of Parkinson's disease, dystonia, Tourette syndrome and obsessive-compulsive disorder can be considered to represent an imbalance between goal-directed and habitual action and inhibition. It is possible that surgical interventions targeting the

Clinical significance of periodic leg movements during sleep in rapid eye movement sleep behavior disorder.

The aim of the study was to explore the clinical significance of periodic leg movements during sleep (PLMS) in rapid eye movement sleep behavior disorder (RBD) and the pathological relation between these two disorders. Eighty-one consecutive idiopathic RBD (iRBD) patients, classified into two groups-27 patients with PLMS (iRBD-PLMS) and 54 patients without PLMS (iRBD w/o PLMS), and 31 patients with idiopathic PLMS (iPLMS)-were enrolled in this study. Descriptive variables including Epworth Sleepiness Scale (ESS) scores and polysomnography measures were compared among the three patient groups.

Movement Disorders Society Scientific Issues Committee report: SIC Task Force appraisal of clinical diagnostic criteria for Parkinsonian disorders.

As there are no biological markers for the antemortem diagnosis of degenerative parkinsonian disorders, diagnosis currently relies upon the presence and progression of clinical features and confirmation depends on neuropathology. Clinicopathologic studies have shown significant false-positive and false-negative rates for diagnosing these disorders, and misdiagnosis is especially common during the early stages of these diseases. It is important to establish a set of widely accepted diagnostic criteria for these disorders that may be applied and reproduced in a blinded fashion. This review summa
