Although isolated lower motor neuron disease has been reported as a paraneoplastic complication, it has not been previously described, in association with anti-Hu antibody. We report a 51-year-old man in whom weakness heralded the presence of a small-cell cancer of the lung. His neurological disorder was characterized by an unremitting progression of limb, neck, and chest wall weakness and wasting that commenced and remained predominant in the upper limbs. Electrophysiological studies demonstrated widespread denervation and examination of a muscle biopsy specimen showed evidence of acute and c

A 30 months-old boy with Farber disease developed nystagmus 12 months after hematopoietic stem cell transplantation (HSCT). At 40 months, gait ataxia was evident, and brain MRI showed increased size of pericerebellar sulci and 4th ventricle. EMG showed denervation in the tongue and upper limb muscles, consistent with motor neuron disease. HSCT improves the peripheral manifestations of Farber disease, but may not prevent the progressive neurological deterioration.

Pediatric mangling injuries are approached differently in children younger than 12 years of age due to growth potential. Bony fixation and free tissue transfers can be technically challenging but result in superior sensory and motor outcomes compared to adults. We present a challenging case of pediatric upper extremity reconstruction following motor vehicle trauma with brachial plexus injury, vascular compromise, bony injury, and denervated muscles. Single gracilis functional free muscle transfer was performed to restore elbow flexion. Medical Research Council improved from 0 to 4 and he was a

A 45-year-old man presented with bilateral vocal fold immobility (BVFI) following a remote history of assault. He was found to have a comminuted, telescoped, and ossified posterior cricoid fracture on imaging. Electromyography revealed normal cricothyroid and thyroarytenoid muscle function, but moderate chronic denervation of bilateral posterior cricoarytenoid muscles. The patient underwent endoscopic posterior cricoid split with rib graft (EPCS/RG), and he regained moderate vocal abduction and full vocal adduction, resolution of dysphonia, and was decannulated. This report describes the only

An electromyographic study was performed in 50 cases of severe infantile spinal muscular atrophy (type I). 164 muscles and 93 nerves (ulnar and posterior tibialis) were tested. The thresholds of stimulation of the motor nerves were increased. Motor action potential was absent in 30% of the investigated nerves, motor nerve conduction velocity was slow in 44%, H reflex was absent in 86%. Proprioceptive nerve conduction velocity was slow in some cases. Using the long-time (50-100 msec) direct longitudinal technique of muscle stimulation (provoked fibrillation), denervation could be proved in 98%

This work describes a split gastrocnemius musculotendinous surgical transfer procedure that may be used to correct an equinosupinatus deformity. The procedure was used to realign the foot of a patient whose deformity was iatrogenically induced by an improperly placed gluteal injection. The injection caused symptoms consistent with a sciatic nerve lesion, with resulting denervation, multiple leg atrophy, ensuing muscle imbalance, and equinosupinatus deformity. Correction was obtained by passing the medial portion of the gastrocnemius tendon and muscle deep to the lateral portion, around the lat

We have recorded the latency of the anal reflex, using a simple electrophysiological method, in 22 patients with anorectal incontinence and in 33 control subjects without anorectal disease. The mean latency in the control subjects was 8.3 ms (range 5-11.3 ms). In the patients with faecal incontinence the mean latency of the anal reflex was 13.0 ms (range 6.5-22.0 ms). The reflex latency was within the normal range in only three incontinent patients. These findings support our concept, based on histological examination of the external anal sphincter musculature, that a major cause of this syndr

Spine-specific sarcopenia has recently been recognized as a localized degenerative disorder distinct from systemic sarcopenia. Its main symptom is selective degeneration of paraspinal muscles and is closely associated with various spinal disorders and postoperative prognosis. One of the core features of this condition is fatty infiltration (FI), which more accurately reflects muscle quality and functional impairment than muscle cross-sectional area (CSA). FI is closely associated with paraspinal muscle atrophy and is regulated by multiple interrelated mechanisms, including pathological activat

The molecular basis underlying muscle atrophy, as it occurs during disuse or aging, and activity-induced hypertrophy remain poorly understood. A major challenge has been defining the diverse cellular and niche environments within skeletal muscle, which is mostly composed of multinucleated myofibers. Here, we present a single-nucleus and single-cell transcriptomic atlas, coupled with spatial profiling, of mouse limb skeletal muscle under resting conditions and during experimentally induced atrophy or hypertrophy. We identify condition-dependent shifts in muscle-resident cell populations and fib

Nogo (RTN4) belongs to the reticulon (RTN) family of integral membrane proteins. RTN4A (Nogo-A), RTN4B (Nogo-B) and RTN4C (Nogo-C) are isoforms of RTN4. In the gastrocnemius muscle of transgenic mice bearing an SOD1 mutation ("ALS model"), increased Nogo-A mRNA and protein was reported, and similar changes were reported in muscle biopsies of patients with amyotrophic lateral sclerosis (ALS) but not with peripheral neuropathy or primary muscle diseases, leading to the proposal that Nogo-A in skeletal muscle is a new specific molecular marker of ALS. Here we report, based on studies of muscle bi

It is well-established that regular contraction maintains morphological and functional integrity of skeletal muscle, whereas rigorous exercise training can upregulate muscle metabolic and contractile function. However, when muscles stop contraction, such as during immobilization (IM) and denervation, withdrawal of IGF/Akt/mTOR signaling allows FoxO-controlled protein degradation pathways to dominate. Mitochondria play an important role in regulating both protein synthesis and degradation via several redox sensitive signaling pathways such as mitochondrial biogenesis, fusion and fission dynamic

The causal relationship between electrical injury and development of amyotrophic lateral sclerosis (ALS) remains controversial. We describe the case of a 25-year-old man who developed ALS after a severe electrical injury. Cerebral magnetic resonance imaging (MRI) demonstrated hyperintensities involving the corticospinal tract. Functional testing with transcranial magnetic stimulation established that the motor cortex was relatively inexcitable. In addition, there were features of denervation on electromyography and muscle biopsy that supported concomitant lower motor neuron findings and the di

INTRODUCTION: Tarsal tunnel syndrome is a compressive neuropathy of the tibial nerve with multiple causes. This syndrome is difficult to diagnose and can be missed because of its subjective symptomatology.
OBSERVATION: In our patient, suspected post-traumatic tarsal tunnel syndrome was confirmed at MRI. This case highlights muscle signal anomalies caused by their denervation in the tibial nerve territory.
CONCLUSION: MRI can provide supplementary information to the electromyography (EMG) and contribute to positive and etiologic diagnosis of peripheral nerve lesions.

A 12-year-old spayed Golden Retriever with mycosis fungoides was treated by use of a chemotherapy protocol that included vincristine. After 16 weekly vincristine injections, the dog began to have signs of peripheral neuropathy. Results of electromyographic examination were consistent with muscle denervation, and motor nerve conduction velocity was depressed. Histologic examination of a common peroneal nerve biopsy specimen revealed severe nerve fiber degeneration. Clinical response and pathologic evidence of improvement were observed after the drug had been discontinued for 2.5 months.

No surgical procedure has been available to repair cervical nerve root avulsion inside the spinal canal. Results with peripheral neurotization of denervated muscles have been discouraging. The authors have performed bridge-graft coaptation in three patients with C-5 and C-6 nerve root avulsion. The components of the coaptation included the anterior primary rami of C-3 and C-4 as the donor material, the entire upper trunk as the recipient, and the sural nerve graft as the bridge. This procedure resulted in restoration of motor function in the biceps and shoulder-girdle muscles and produced impr

OBJECTIVES: This study was performed to investigate the muscle-nerve-muscle reinnervation technique in the larynx, in which a nerve conduit implanted into an innervated muscle conducts axonal sprouting into a denervated muscle while maintaining function of the donor muscle.
METHODS: In this study, the muscle-nerve-muscle technique was used to direct superior laryngeal nerve axons to reinnervate intrinsic laryngeal muscles by implanting the recurrent laryngeal nerve stump into the cricothyroid muscle in 8 dogs. In 4 of the dogs, the recurrent laryngeal nerve trunk to the adductor muscles was d

The separate contributions of the glossopharyngeal nerve and the pharyngeal branch of the vagus nerve to the innervation of the pharyngeal muscles were studied first in 10 canine cadavers by dissection of the pharyngeal plexus and the pharyngeal muscles. In 10 other dogs, the parent trunks and 1st division of the glossopharyngeal nerve and the pharyngeal branch of the vagus nerve were electrically stimulated. The evoked stimulation potentials were recorded from the stylopharyngeal, hyopharyngeal, thyropharyngeal, and cricopharyngeal muscles. One of the parent trunks was then transected, and th

The maintenance of muscle mass and its ability to function relies on a bioenergetic efficient mitochondrial network. This network is highly impacted by fusion and fission events. We have recently shown that the acute deletion of the fusion protein Opa1 induces muscle atrophy, systemic inflammatory response, precocious epithelial senescence, and premature death that are caused by muscle-dependent secretion of FGF21. However, both fusion and fission machinery are suppressed in aging sarcopenia, cancer cachexia, and chemotherapy-induced muscle wasting. We generated inducible muscle-specific Opa1

It is well established that long durations of bed rest, limb immobilization, or reduced activity in respiratory muscles during mechanical ventilation results in skeletal muscle atrophy in humans and other animals. The idea that mitochondrial damage/dysfunction contributes to disuse muscle atrophy originated over 40 years ago. These early studies were largely descriptive and did not provide unequivocal evidence that mitochondria play a primary role in disuse muscle atrophy. However, recent experiments have provided direct evidence connecting mitochondrial dysfunction to muscle atrophy. Numerous

We describe a kindred with an unusual congenital lower motor neuron disorder with significant but static muscle weakness predominantly affecting the lower limbs. The proband had talipes equinovarus and congenital hip contractures and did not walk until 19 months of age. Lower-extremity predominant, primarily proximal weakness was identified on assessment at three years. Over a 20 year follow-up there has been no clinical progression. The proband has a four-year-old daughter with very similar clinical findings. Electromyography and muscle biopsy suggest reduced numbers of giant normal duration
