A 77-year-old man presented with a 2-year history of left thumbnail dystrophy, purulent discharge and aching pain. There was no history of trauma. On examination there was a linear area of nail dystrophy and dyschromia of the nail bed. The nail plate was thinned proximally. No other digits were affected. There was no lymphadenopathy. BACKGROUND: The Psoriasis Area and Severity Index does not take the severity of nail involvement into account.
OBJECTIVES: To devise a system of scoring for nail psoriasis.
METHODS: A system of scoring for nail psoriasis was devised that takes into account the site of the pathology, i.e. (i) the matrix, (ii) the subungual tissues distal to the lunula, or (iii) the whole nail unit.
RESULTS: The proposed scoring system evaluates several signs of nail psoriasis separately, each on a 1-3 scale: pitting, Beau's lines, subungual hyperkeratosis and onycholysis.

Involvement of the nails in pemphigus vulgaris (PV) is rare and is usually seen when the disease is severe. The most common clinical manifestations are chronic paronychia and onychomadesis. Treating onychomycosis may be a tantalizing problem depending on the age of the patient, the condition's modifications, and the status of the peripheral circulation. The clinician will have to choose among three main possibilities: topical therapy with the new nail lacquers, new oral drugs, and removal of the nail plate. A combination of different modalities often provides the best option.

Nail biopsy is a safe and useful technique for diagnosis and management of many nail conditions. A basic understanding of nail anatomy and biology is a prerequisite for a successful nail biopsy. The patient must be adequately prepared and there needs to be excellent anesthesia and hemostasis. The type of nail biopsy depends largely on the location of the pathology in the nail unit. The techniques of nail biopsy by location in the nail unit and by lesion type are discussed.

Lateral longitudinal nail biopsy is the gold standard of incisional nail biopsies. It can be obtained under local anaesthetic and requires normal biopsy instruments. Preoperative planning with the patient is important in order that they have realistic expectations of the therapeutic outcome and understand how to care for the wound in the postoperative period.

Antiepileptics are known to produce different types of side effects including nail changes. A 20-year-old epileptic man had yellowish discolouration followed by dystrophy and onycholysis of his 15 nails after taking carbamazepine for 3 months which cleared within six months after discontinuation of the drug. This type of change has not been reported earlier with carbamazepine.

PURPOSE: Penetrating ocular trauma resulting from nails can have disastrous visual consequences. Ocular nail gun injuries occur when a nail propelled at high velocity penetrates the globe. The authors report four consecutive patients with ocular injuries resulting from nail guns.
METHODS: Four patients with ocular nail gun injuries are described: one with scleral penetrating injury, one with corneal penetrating injury, and two with perforating (through- and-through) injuries of the globe.
