Mechanical stress and mechanosensitive protein in skin tumors.

The skin is highly susceptible to external physical stimuli, and mechanical stresses contribute to certain skin tumors. Focusing on keloid, basal cell carcinoma, cutaneous squamous cell carcinoma and melanoma, we systematically examined the roles and mechanisms between mechanical stresses, such as tensile stress and matrix stiffness, and mechanosensitive proteins, including Piezo, transient receptor potential (TRP), integrin family, Yes-associated protein (YAP) and transcriptional coactivator with PDZ-binding motif (TAZ), in skin tumor progression. Additionally, we analyze existing research ch

Line-field confocal optical coherence tomography in dermato-oncology: A literature review towards harmonized histopathology-integrated terminology.

Non-invasive diagnostics like line-field confocal optical coherence tomography (LC-OCT) are being implemented in dermato-oncology. However, unification of terminology in LC-OCT is lacking. By reviewing the LC-OCT literature in the field of dermato-oncology, this study aimed to develop a unified terminological glossary integrated with traditional histopathology. A PRISMA-guided literature-search was conducted for English-language publications on LC-OCT of actinic keratosis (AK), keratinocyte carcinoma (KC), and malignant melanoma (MM). Study characteristics and terminology were compiled. To har

Micro-relief characterization of benign and malignant skin lesions by polarization speckle analysis in vivo.

BACKGROUND/PURPOSE: A recent direction in skin disease classification is to develop quantitative diagnostic techniques. Skin relief, colloquially known as roughness, is an important clinical feature. The aim of this study is to demonstrate a novel polarization speckle technique to quantitatively measure roughness on skin lesions in vivo. We then calculate the average roughness of different types of skin lesions to determine the extent to which polarization speckle roughness measurements can be used to identify skin cancer. METHODS: The experimental conditions were set to target the fine relie

Risk of subsequent keratinocyte carcinomas after a first diagnosis in Tasmania, Australia.

BACKGROUND/OBJECTIVE: A history of keratinocyte carcinoma (KC) is a risk factor for further KCs, but population-based studies quantifying the risk are lacking in Australia. We aimed to describe the risk of subsequent KCs after first KCs in the Australian state of Tasmania. METHODS: Tasmanian residents identified in the Tasmanian Cancer Registry with a first histologically confirmed basal cell carcinoma (BCC), squamous cell carcinoma (SCC) or synchronous BCC and SCC (within 3 months) between January 1985 and December 2013 were followed up for at least 5 years for the development of a subsequen

Overview of familial syndromes with increased skin malignancies.

The vast majority of skin cancers can be classified into two main types: melanoma and keratinocyte carcinomas. The most common keratinocyte carcinomas include basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). Multiple familial syndromes have been identified that can increase the risk of developing SCC, BCC, and/or melanoma. The major syndromes include oculocutaneous albinism for SCC, basal cell nevus syndrome for BCC, familial atypical multiple mole-melanoma syndrome, and hereditary breast and ovarian cancer syndrome for melanoma. In addition, familial syndromes that can predispose

Impact of COVID-19 on dermatologic surgery: experience of a Midwestern academic practice.

The novel coronavirus disease 2019 (COVID-19) pandemic has disrupted clinical practice everywhere. The aim of this study was to quantify the specific impact of COVID-19 on skin cancer treatment at an academic dermatologic surgery practice. We conducted a retrospective chart review to compare metrics such as patient visits, histological upgrading, and Mohs stages per tumor between 60-day periods immediately before and after COVID-19 was declared a pandemic. Out of 1138 total encounters, decreases of 58% in total in-person visits and 38% in Mohs surgeries performed were observed following declar

Aggressive progression of a facial super giant basal cell carcinoma.

Super giant basal cell carcinoma (BCC) is a rare oncological entity. A 52-year-old man with a history of a left upper facial lesion for 11 years was transferred to our facility seeking an evaluation. He frequented multiple hospitals where he was treated with antibiotics for this condition. He reported having the Mohs procedure 8 months prior to this hospital visit but he did not follow-up with the other institution. The physical examination revealed an ulcer involving the upper left orbito-fronto-parieto-temporal area with bone exposure. Both CT and MRI of the face and the brain demonstrated d

Familial skin cancer syndromes: Increased risk of nonmelanotic skin cancers and extracutaneous tumors.

Nonmelanoma skin cancers (NMSCs) represent the most common malignancies worldwide, with reported incidence rising each year. Both cutaneous squamous cell carcinoma (SCC) and basal cell carcinoma (BCC), as well as other NMSCs, represent complex diseases with a combination of environmental and genetic risk factors. In general, hereditary cancer syndromes that increase the risk of NMSC fall under several broad categories: those associated with immunodeficiencies, those that affect skin pigmentation, and those that perturb key molecular pathways involved in the pathogenesis of NMSCs. Many of the s

Imiquimod - Its role in the treatment of cutaneous malignancies.

Imiquimod is a synthetic imidazoquinolone amine, which has potent immune response modifier activity, when topically used. This characteristic property of imiquimod has led to its use in a number of applications in dermatology, particularly in cutaneous malignancies, where it has been found to be effective and safe. Currently, additional mechanisms for its activity in actinic keratosis, basal cell carcinoma, and invasive squamous cell carcinoma have been elucidated. Its usage for cutaneous metastasis in breast cancer has been a further addition to its therapeutic armamentarium recently.

TP53 mutations in human skin cancers.

The p53 gene (TP53) is mutated in numerous human cancers. We have used it as a molecular target to characterize the induction of mutations in human skin cancers. About 50% of all skin cancers in normal individuals exhibit p53 mutations. This frequency rises to 90% in skin cancers of patients with the DNA-repair deficiency known as xeroderma pigmentosum (XP). These mutations are characterized by a specific signature, attributed to the ultraviolet uvB part of the solar spectrum. In this review, we will describe different p53 mutation spectra, in relation to the various histopathological types of

Epidemiology of nonmelanoma skin cancer.

In summary, the cause of NMSC is multifactorial and complex. The interaction among environmental and lifestyle factors along with host factors determines the development and progression of NMSC. Despite the relatively benign nature of NMSC, rising incidence rates in an aging population will result in increased medical costs, morbidity, and mortality. Frequent exposure to UVR, fair skin, and reduced immunity, possibly related to aging, are the major risk factors. Because UVR exposure is responsible for most of the NMSC cases, the key to reducing the number of skin cancers is through prevention,

Functional thrombomodulin expression on epithelial skin tumours as a differentiation marker for suprabasal keratinocytes.

In normal human skin,, immunoreactive thrombomodulin (TM) is expressed in a strict differentiation related pattern, solely in suprabasal spinous layer keratinocytes. To evaluate the potential application of TM as a differentiation marker for keratinocyte-derived skin tumours, we have studied immunohistopathological, biochemical and functional TM activities in various skin tumours. Immunoreactive, full sized and enzymatically active TM was expressed in keratinocyte-derived skin tumours (squamous cell carcinoma, seborrhoeic keratosis and partly Bowen's disease), as well as normal epidermal kerat

Surgical management of advanced scalp cancer.

A series of 53 advanced scalp cancer patients treated with surgery is presented. A review of these patients concludes: 1) local tumor growth--not distant metastasis--is the primary problem; 2) management is based on the tendency of a tumor to spread laterally early in its development with a deep invasion occurring later on; 3) when deep invasion occurs, radical excision and reconstruction is indicated to control the disease; 4) recurrence following adequate local therapy requires a change in treatment modality.

Principles of surgical management of midfacial carcinoma.

A series of large, midfacial, squamous cell and basal cell carcinomas treated by the Departments of Otolaryngology and Dermatology (Cleveland Clinic Foundation) are reported. Most cases required total/partial resection of the external nose, upper lip, hard palate, nasal septum, and adjacent midfacial soft tissues. Emphasis is placed on the principles of preoperative and intraoperative management as it relates to intraoral prosthetic design and total margin control of the surgical specimen. The immediate and delayed reconstructive philosophy and techniques employed here are discussed in detail.

Electrical potential difference between basal cell carcinoma, benign inflammatory lesions and normal tissue.

Measurement of the resting electrical potential difference on the surface of the skin has been made on patients with suspected basal cell carcinoma. It was demonstrated that in fifty-five lesions from forty-nine patients, the mean electrical potential difference from thirty-six basal cell carcinomas was found to be +II mV, from nineteen benign inflammatory lesions it was found to be +2 mV, and from normal skin, contra-lateral to the lesions in both groups, it was found to be -3 mV. All the measurements are relative to a reference electrode placed under the tongue. The difference between the me

Tumor of Follicular Infundibulum Is Molecularly Distinct From Basal Cell Carcinoma.

Tumor of follicular infundibulum (TFI) is a rare cutaneous lesion of unclear histogenesis. Various benign and malignant cutaneous neoplasms have been reported in association with TFI, including most commonly basal cell carcinoma (BCC). Indeed, some authors have suggested that TFI may represent a variant of BCC. Because BCC are well established to be driven by alterations in the Hedgehog signaling pathway, most often secondary to ultraviolet (UV)-induced mutagenesis, we undertook next-generation DNA sequencing of a cohort of TFI to determine their molecular relatedness to BCC. Four TFI cases we
