Echocardiography in neurological disorders.

Numerous neurological disorders are associated with myocardial disease or involve the cardiovascular system. Echocardiography thus proves quite helpful in the evaluation and management of patients with problems such as hydrocephalus, tuberous sclerosis, Friedreich ataxia, mitochondrial encephalo-myopathies, Werdnig-Hoffmann disease, convulsive disorders, syncope, central nervous system infections, etc.

The genetics of neurologic syndromes.

Diagnosis and counseling represent the two primary functions of the clinical geneticist. To establish a diagnosis, the geneticist must be familiar with the entire gamut of genetic and nongenetic disorders. Even when the diagnosis is made by laboratory tests or physical examination, the pervasiveness of genetic/etiologic heterogeneity demands that the geneticist be skilled in the extraction of a thorough family history and in the subtleties of pedigree interpretation. Multiple specialty referrals may be solicited. Counseling is an involved process that includes a thorough discussion of the natu

Neurologic complications of immunizations.

Although there does appear to be at least a temporal relationship between pertussis immunization and serious acute neurologic illness, data to suggest that children with stable preexisting neurologic disease or positive family history of neurologic disease are at increased risk for complications of pertussis immunizations are inconclusive. Furthermore, there are no firm statistical data concerning the incidence of pertussis vaccine-related encephalopathy. Rather, the literature on pertussis vaccine complications is replete with anecdotal reports and retrospective studies with a number of quest

Acute neurologic infections.

This article discusses acute bacterial, viral, toxin-mediated, and parasitic neurologic infections, emphasizing those infections that are potentially treatable, are rapidly fatal if untreated, or pose a significant risk of person-to-person transmission.

Neuropeptides in neurological tumours.

Concentrations of seven neuropeptides have been determined in 69 human neurological tumours. The majority of tumours were intrinsic to the central nervous system, being astrocytomas. In general, within the the better differentiated tumours (Grade I/II astrocytomas) higher concentrations of five neuropeptides (neuropeptide Y, somatostatin, substance P, vasoactive intestinal peptide and cholecystokinin) were measured in comparison to the poorly differentiated tumours. Of the metastatic tumours, five were derived from oat cell carcinoma of the bronchus. Very high concentrations of bombesin were i

Meningoceles and neurological involvement.

The term meningocele is used in this communication to include all lesions presenting as swellings associated with spina bifida, when neural tissue is not exposed on the surface. 144 meningoceles were operated on during an 18-year period. The stalk of the meningocele was connected with the neural tissue in 113 of them. Two distinct types of lesions have been noted in these patients and have been designated "meningocele with tethered cord" (52) and "meningocele with tethered roots" (56). In both conditions about half the number of cases presented clinically before the age of 3 months, when parap

Neurological aspects of insulinomas.

Neurological involvement occurred in every one of a series of 30 patients with an insulinoma. The episodic nature of the hypoglycaemia caused symptoms and signs to fluctuate and often led to delay in diagnosis (mean length of history was 3 years). The commonest feature at first presentation was confusion (20 instances), but as the illness evolved, coma (16 instances) and convulsions (8 instances) became more frequent. Objective weakness was found in 7 patients, with 3 examples of hemiparesis and 2 each of paraparesis and monoparesis; in all, the weakness resolved over a period of 1 hr to 3 day

Neurological complications of vasculitis.

The vasculitides are a group of disorders that include the polyarteritis nodosa group of systemic necrotizing vasculitides, hypersensitivity vasculitis, Wegener's granulomatosis, lymphomatoid granulomatosis, giant cell arteritis, Behçet's disease, and isolated angiitis of the central nervous system. Classification is based on clinical, angiographic, and histological features. The frequency and distribution of neurological involvement vary with the underlying disorder and may provide the initial symptoms. Polyarteritis nodosa and Wegener's granulomatosis may affect both the central and peripher

Neurological involvement in legionellosis.

A study of the clinical, electrophysiological and biochemical features of 16 patients with legionellosis has been performed. Evidence of central and peripheral nervous system involvement has been found in the majority of patients. This is characterised by confusion out of keeping with the degree of toxic or metabolic upset, signs of anterior midline cerebellar dysfunction, grossly elevated creatinine kinase of skeletal muscle origin, and a subclinical peripheral neuropathy.

Neurologic perspectives of otosclerosis.

Of 500 patients with roentgenographically verified otosclerosis, 230 had vestibular manifestations. Because of its frequency in general practice, otosclerosis has a major impact on the differential diagnosis of vertigo and related symptoms. The vestibular syndrome encompasses the full symptomatic spectrum common to diseases of the vestibular system, from episodic violent rotational vertigo to a sense of almost continuous imbalance, and cannot usually be distinguished from idiopathic endolymphatic hydrops. Otosclerosis is easy to diagnose when vestibular symptoms occur in conjunction with condu

Neurologic manifestations of homocystinuria.

Recognition of the disease homocystinuria by the neurologist may be of importance as is illustrated in the dramatic case history of a 20 year old woman. Confusion with Marfan's syndrome frequently occurs. The clinical and pathologic anatomical features are described. A review of the literature is given and the use of contraceptives is discussed.

Copper and neurological function.

The role of copper in maintaining normal neurological function has been examined in animals copper-deficient by dietary means, and in the genetic disorders of copper homeostasis -- Menkes' kinky-hair disease in humans and the mottled (Mo) mutants in the mouse. With the exception of the disorder in Mo mice, reduced myelination is a constant feature of these copper diseases but there is otherwise a lack of conformity in the structural defects produced in different species. Dietary copper-deficient animals show a reduction in noradrenaline and dopamine concentrations, together with a depressed ty
