Postoperative respiratory depression following excision of carotid body tumours.

This report is of two adult patients who were scheduled for excision of carotid body tumours. One of the patients had bilateral tumours, while the second had a unilateral tumour. In the first, anaesthesia was maintained by nitrous oxide:oxygen and halothane 1-2%. Postoperatively, the intramuscular injection of meperidine 75 mg resulted in apnoea, cyanosis, and loss of consciousness. In the second, anaesthesia was maintained by nitrous oxide:oxygen, supplemented by fentanyl 2 micrograms.kg-1 and incremental doses of vecuronium. Following complete reversal of neuromuscular block, the patient bec

Investigation of human mitochondrial myopathies by phosphorus magnetic resonance spectroscopy.

Abnormal mitochondria are an increasingly recognized cause of neuromuscular disease. We have used phosphorus magnetic resonance spectroscopy to monitor noninvasively the metabolism of high-energy phosphates and the intracellular pH of human skeletal muscle in vivo in 12 patients with mitochondrial myopathy. At rest, an abnormality could be demonstrated in 11 of 12 patients. Ten patients had evidence of a reduced muscle energy state with at least one of the following abnormalities: low phosphorylation potential, low phosphocreatine concentration, high adenosine diphosphate concentration, or hig

Migration of Hymenolepis diminuta (Cestoda) and changes in 5-HT (serotonin) levels in the rat host following parenteral and oral 5-HT administration.

Changes in the distribution of the tapeworm Hymenolepis diminuta in the rat small intestine were studied for up to 3 h following intraperitoneal, intramuscular, subcutaneous, and oral administration of 5-HT. Changes in 5-HT levels in the arterial circulation, intestinal lumen, and in worm tissue were also monitored. In all cases following 5-HT administration there was an anteriad migration by H. diminuta. The extent of the migration was dose dependent, the higher the dose the more marked the migration response. Normal 5-HT levels in the parasitized gut are higher than those in uninfected anima

Blood Flow Restriction in Athletic Populations-Part 1: Safety Considerations, and Methodological Frameworks.

Background: Blood flow restriction (BFR) training induces morphological and neuromuscular adaptations using low-intensity exercise (20-40% 1RM), offering a reduced mechanical load alternative to traditional high-load resistance training. Safe and effective implementation, however, requires a clear understanding of physiological mechanisms, contraindications, and pressure determination methodologies. In this three-part series, we provide a comprehensive review of BFR for athletic populations and provide strength and conditioning coaches with a structured framework for screening, safety, and met

The TOFr of 0.75 to 0.85 is the optimal timing for IONM during thyroid surgery: a prospective observational cohort study.

BACKGROUD: Recurrent laryngeal nerve (RLN) injury is one of the serious complications of thyroid tumour surgery, surgical treatment of thyroid cancer requires careful consideration of the RLN and its impact on glottis function. There has been no unified standard for precise neuromuscular block monitoring to guide the monitoring of RLN in thyroid surgery. This study aimed to investigate the correlation between Train-of-four stabilization ratio (TOFr) and neural signal values of intraoperative neurophysiological monitoring (INOM) during thyroid operation, and further to determine the optimal tim

Qnox index for quantification of intraoperative nociception and analgesia: a prospective single-centre validation study.

BACKGROUND: The Qnox index is a novel monitor to quantify intraoperative nociception based on an electroencephalographic algorithm. We evaluated the ability of the Qnox index to discriminate noxious from non-noxious stimuli, respond to stimuli, and discriminate different levels of analgesia in patients under propofol anaesthesia with neuromuscular block. METHODS: Qnox was compared with heart rate and mean arterial pressure with five designated stimuli: tetanic stimulations without (tetanic 1) and with sufentanil (tetanic 2), skin incision, tracheal intubation, and a non-noxious period. The re

Multimodal Surveillance Model for Enterovirus D68 Respiratory Disease and Acute Flaccid Myelitis among Children in Colorado, USA, 2022.

Surveillance for emerging pathogens is critical for developing early warning systems to guide preparedness efforts for future outbreaks of associated disease. To better define the epidemiology and burden of associated respiratory disease and acute flaccid myelitis (AFM), as well as to provide actionable data for public health interventions, we developed a multimodal surveillance program in Colorado, USA, for enterovirus D68 (EV-D68). Timely local, state, and national public health outreach was possible because prospective syndromic surveillance for AFM and asthma-like respiratory illness, pros

Changes of extremity and laryngeal muscle electromyographic amplitudes after intravenous administration of vecuronium.

OBJECTIVES: To compare electromyographic (EMG) amplitude changes in extremity and laryngeal muscles after vecuronium administration and study the effects of topical lidocaine on laryngeal EMG in a swine model. STUDY DESIGN: Prospective animal study. METHODS: Electrically evoked EMG activities were recorded from the vocalis muscles (directly and via an EMG endotracheal tube) and from the flexor digitorum muscles before and after intravenous vecuronium administration. EMG amplitudes were followed for 30 minutes after vecuronium injection together with monitoring of limb twitches to train-of-fo

Somatosensory-evoked potential monitoring during instrumented scoliosis corrective procedures: validity revisited.

BACKGROUND: Intraoperative monitoring (IOM) using somatosensory-evoked potentials (SSEPs) plays an important role in reducing iatrogenic neurologic deficits during corrective pediatric idiopathic procedures for scoliosis. However, for unknown reasons, recent reports have cited that the sensitivity of SSEPs to detect neurologic deficits has decreased, in some to be less than 50%. This current trend, which is coincident with the addition of transcranial motor-evoked potentials, is surprising given that SSEPs are robust, reproducible responses that were previously shown to have sensitivity and sp

Comparison of Different Neostigmine Doses for Reversal of Cisatracurium-Induced Neuromuscular Block in Children Under Total Intravenous Anesthesia: A Randomized Controlled Trial.

BACKGROUND: Neostigmine is widely used to reverse nondepolarizing neuromuscular blockade in children, but the optimal dose under total intravenous anesthesia is uncertain. AIMS: The primary aim was to compare the time to full neuromuscular recovery (TOF ratio of 1.0) following administration of neostigmine at doses of 0, 10, 20, and 30 μg/kg in children at a TOF count of 3. Secondary objectives were full reversal within 10 min and adverse events. METHODS: This prospective, randomized, double-blind, parallel-group, superiority trial enrolled 120 children (2-10 years; ASA I-II) undergoing tons

The electromyographic single twitch stimulation for monitoring the effect of rocuronium on vocal cord opening - a randomised controlled trial.

BACKGROUND: The European Society of Anaesthesiology and Intensive Care recommends the use of neuromuscular blocking drugs for tracheal intubation, but the monitoring is difficult, because the parameters are mostly relative values (e.g. Train-of-four ratio), which show no or only weak correlations to the vocal cord aperture. We investigated the predictive effect of the quantitative single twitch (0.3 ms duration supramaximal stimulus, frequency 0.1 Hz) to estimate vocal cord aperture (primary endpoint). Secondarily, we focused on rocuronium dose-related differences between single twitch amplitu

Effect of a multicomponent nutritional supplement on functional outcomes for Duchenne muscular dystrophy: A randomized controlled trial.

BACKGROUND & AIMS: Duchenne muscular dystrophy (DMD) is an X-linked neuromuscular condition causing progressive muscle weakness and premature death. Whilst effective treatments such as gene therapy are developed, families often seek complementary therapies such as nutrition supplements to help their son maintain function; however, there is limited evidence supporting the use of nutritional supplements in DMD. This study aimed to compare the effect of a Standard nutritional supplement with an Enhanced nutritional supplement combining three nutriceuticals on functional outcomes in ambulatory boy

Hand or foot train-of-four tests and surgical site muscle relaxation assessed with multiple motor evoked potentials: A prospective observational study.

BACKGROUND: Intra-operative muscle relaxation is often required in orthopaedic surgery and the hand train-of-four (TOF) test is usually used for its quantification. However, even though full muscle relaxation is claimed by anaesthesiologists based on a TOF count of zero, surgeons observe residual muscle activity. OBJECTIVE: The aim of the study was to assess if hand or foot TOF adequately represents intra-operative muscle relaxation compared with multiple motor evoked potentials. DESIGN: Prospective observational study. SETTING: A single-centre study performed between February 2016 and Dece

Seromas Following Pediatric Spinal Deformity Surgery: Is Operative Management Necessary?

BACKGROUND: Seromas are known complications after pediatric spinal deformity surgery. Although many surgeons perform an early debridement to prevent deep surgical site infections (SSIs), a less invasive approach to seroma management has not been studied. We hypothesized that a conservative approach to seroma management would be safe and yield equivalent outcomes. METHODS: We performed a retrospective review of patients who developed a postoperative seroma with or without nonpurulent drainage. Inclusion criteria were patients below 21 years who underwent primary posterior spinal fusion from 19

Anisotropic composite material phantom to improve skeletal muscle characterization using magnetic resonance elastography.

The presence and progression of neuromuscular pathology, including spasticity, Duchenne's muscular dystrophy and hyperthyroidism, has been correlated with changes in the intrinsic mechanical properties of skeletal muscle tissue. Tools for noninvasively measuring and monitoring these properties, such as Magnetic Resonance Elastography (MRE), could benefit basic research into understanding neuromuscular pathologies, as well as translational research to develop therapies, by providing a means of assessing and tracking their efficacy. Dynamic elastography methods for noninvasive measurement of tis

An implementation protocol for noninvasive ventilation prescription: the physiotherapist's role in an Italian hospital.

BACKGROUND: For patients with chronic respiratory failure or sleep breathing disorders, adaptation and training are important prerequisites for successful home noninvasive ventilation (NIV) and CPAP. In Europe, management of NIV/CPAP is sometimes performed by physiotherapists (PTs). However, their role within the NIV/CPAP management team is not well defined. OBJECTIVE: To describe the implementation of a standardized protocol to NIV adaptation for patients with chronic respiratory disease. Secondary outcomes were to evaluate the PT's role and time-consumption. METHODS: We enrolled 201 patien

Timing Matters: Assessing Fatigue During and After Resistance Training.

BACKGROUND: Monitoring neuromuscular fatigue during resistance training informs load prescription and recovery management. Two common approaches, namely within-training assessment (immediately after each set) and post-training assessment (after session completion), differ in their methodological demands, yet their acute responses and within-session reliability remain insufficiently characterized. HYPOTHESIS: Fatigue levels would be influenced by training protocol, value type (maximum vs average), and set number or timepoint, with corresponding differences in within-session reliability. STUDY
