Amputation stump lengthening with the Ilizarov technique. A case report.

It is often difficult to create a functional stump from a traumatic amputation, especially in a child. A case is described of a traumatic, high, below-knee amputation in a five-year-old child. The resulting stump was too short to allow a conventional below-knee prosthesis, decreasing gait efficiency. The Ilizarov technique was used to increase tibial length. Bony lengthening was very successful, but several problems were encountered with the soft-tissue reconstruction. This technique of stump reconstruction holds promise only if the soft-tissue problems are anticipated. The suggestions for fut

[Diagnosis of deficiency in cofactor of phenylalanine hydroxylase: a metabolic emergency].

We report on two cases of children suffering from biopterin synthetase deficiency. Both were treated with the same treatment schedule with biopterin and neurotransmitters: 6-hydroxytryptophan and dihydrophenylalanine (DOPA). The only difference between the two cases is the time of diagnosis and therefore of treatment. The child who was treated early has a normal neurologic development. The other one has been treated since he was 7 months old and is mentally deficient (DQ = 0.60). This older child also suffers from dystonia probably secondary to Levodopa treatment. The authors emphasize the unc

[Spatial distribution of light intensity with a phototherapy halogen lamp].

The spatial energy distribution under a phototherapy unit fitted with a special halide lamp (ex. Hereaus, "Photo-Therapie 800") is particularly uneven, with a pronounced decrease in energy towards the edges. With a correct positioning of the child in the centre of the luminous cone however, the energy of the effective light is still distinctly higher than that received from a phototherapy unit fitted with 6 blue fluorescent tubes (Philips 20W/52 BAM blue). With the addition of lateral aluminium reflectors, not only is the effective energy intensity increased, but also the spatial energy distri

A case of spontaneous cure of spondylolisthesis in adolescence.

The writers present a case of spontaneous cure of spondylolytic spondylolisthesis in a child aged twelve, which resulted in an almost perfect result within three years. This is an extremely rare occurrence, only five cases having previously been reported.

A case of glutaric acidemia type II (severe multiple acyl-CoA dehydrogenation disorder) with subsequent prenatal exclusion in a sibling.

A case of severe multiple acyl-CoA dehydrogenation disorder is described. This is the second such case reported to have had an elevated maternal serum alpha-fetoprotein and normal amniotic alpha-fetoprotein. The child's 3-day extrauterine life was characterized by intractable acidosis, respiratory distress, and ventricular fibrillation. The characteristic biochemical, morphologic, and microscopic findings of this condition are reviewed. A subsequent pregnancy was evaluated using chorionic villus sampling and analysis of cultured trophoblasts. The trophoblasts were biochemically normal, and a n

Use of an elbow restraint to improve finger-feeding skills in a child with Rett syndrome.

A single-subject study with an ABAB design was conducted to examine the effectiveness of an elbow restraint splint in improving the functional band use of a child with Rett syndrome. The intervention consisted of the child's wearing an elbow restraint on her nondominant (left) arm during snack time. Her ability to finger-feed cereal from a bowl was evaluated across four phases. An analysis of the data revealed only modest improvements in the amount of cereal consumed and in the time required to complete the eating task. Variability in the subject's performance made visual interpretation of the

Disentangling the Effects of Exposure to Maternal Substance Misuse and Physical Abuse and Neglect on Child Behavioral Problems.

There is inconsistent evidence on the association between prenatal substance exposure and child behavioral problems. Children affected by maternal substance misuse are vulnerable to physical abuse and neglect. Few studies have examined factors contributing to internalizing and externalizing problems in these children. This study, with a focus on children with prenatal substance exposure, investigated whether family characteristics (the duration of maternal substance misuse postpartum or the existence of a sibling) moderated the association between adverse exposure (physical abuse or neglect) a

Behavioral outcomes of picky eating in childhood: a prospective study in the general population.

BACKGROUND: Picky eaters in the general population form a heterogeneous group. It is important to differentiate between children with transient picky eating (PE) and persistent PE behavior when adverse outcomes are studied. We analyzed four PE trajectories to determine the associations with child mental health prospectively. METHODS: From a population-based cohort, 3,748 participants were assessed for PE at 1.5, 3, and 6 years of age using maternal reports. Four trajectories were defined: persistent (PE at all ages); remitting (PE before 6 years only); late-onset (PE at 6 years only); and nev

Reporting involvement activities with children and young people in paediatric research: a framework analysis.

BACKGROUND: The active involvement of patients and the public in the design and delivery of health research has been increasingly encouraged, if not enforced. Knowledge of how this is realised in practice, especially where children and young people (CYP) are concerned, is limited, partly due to the low level of reporting of patient and public involvement (PPI) in general. The aim of this work was to assess how researchers funded by the National Institute for Health and Care Research (NIHR) report the involvement of CYP in the design and conduct of child health research to better understand the

Short-term longitudinal associations among parent-child relationship quality, internalizing and externalizing problems, and belief in a just world: a three-wave RI-CLPM.

Parent-child relationship quality is associated with adolescent internalizing and externalizing problems, but conventional cross-lagged models do not separate stable between-person differences from within-person fluctuations. This three-wave study examined short-term reciprocal associations among parent-child relationship quality, belief in a just world (BJW), and internalizing and externalizing problems, as well as indirect effects through BJW and differences by gender. Participants were 591 students from one middle school in Henan Province, China (Mage = 13.30 years, SD = 0.94; 51.8% boys).

Association of Maternal Prepregnancy BMI With Offspring Cardiometabolic Outcomes in Childhood.

OBJECTIVE: This study aimed to evaluate associations of maternal prepregnancy BMI with offspring BMI and blood pressure in childhood, specifically for infants born preterm. METHODS: In this observational cohort study of children in the Environmental Influences on Child Health Outcomes (ECHO) Cohort, we utilized four levels of maternal prepregnancy BMI and child BMI. Children were categorized as being born extremely, very, or moderately preterm; late preterm; or term. RESULTS: In total, 13,810 children from 44 ECHO cohorts were included in these analyses. After adjusting for maternal educatio

Influence of parental anxiety and beliefs about medicines on feeding and exercise in children living with asthma.

This study's primary objective was to establish differences in beliefs about medicines, levels of asthma-related anxiety and diet and exercise behaviours between parents of children with well controlled and poorly controlled asthma. Secondary objectives were to explore how asthma control might shape relationships between parental cognitions and parenting practices concerning paediatric asthma. Parents of children with asthma aged 10-16 years (N = 310) completed standardised questionnaires measuring beliefs about medicines, parental asthma-related anxiety, parenting attitudes towards child acti

A brief compassion focused therapy intervention can help self-critical parents and their children: A randomised controlled trial.

BACKGROUND: Parents can be highly self-critical of their own parenting, which can negatively impact parenting style and child outcomes. AIMS: The aim of this randomised controlled trial (RCT) was to examine the efficacy of a brief 2-hour Compassion Focused Therapy intervention (CFT) for parents to determine if it can reduce self-criticism, improve parenting and improve child social, emotional and behavioural outcomes. MATERIALS & METHODS: In total, 102 parents (87 mothers) were randomised to either a CFT intervention (n = 48) or waitlist control group (n = 54). Participants were measured at

Methods and Software to Analyze Gene-Environment Interactions under a Case-Mother-Control-Mother Design with Partially Missing Child Genotype.

INTRODUCTION: The case-mother-control-mother design allows to study fetal and maternal genetic factors together with environmental exposures on early life outcomes. Mendelian constraints and conditional independence between child genotype and environmental factors enabled semiparametric likelihood methods to estimate logistic models with greater efficiency than standard logistic regression. Difficulties in child genotype collection require methods handling missing child genotype. METHODS: We review a stratified retrospective likelihood and two semiparametric likelihood approaches: a prospecti

Preventing child maltreatment: Key conclusions from a systematic literature review of prevention programs for practitioners.

Child maltreatment (CM) is a worldwide social problem and there is a large consensus that its prevention is of crucial importance. The current literature review highlights CM prevention studies that target practitioners, with the aim of assessing the knowledge in this area, informing future efforts and benefiting the international task of mitigating CM. Specifically, the study presents key conclusions from prevention programs evaluated in peer-reviewed journals from the last decade selected using the PRISMA systematic literature review guidelines. Out of the 26 manuscripts that discussed preve

B3GALNT2-Related Dystroglycanopathy: Expansion of the Phenotype with Novel Mutation Associated with Muscle-Eye-Brain Disease, Walker-Warburg Syndrome, Epileptic Encephalopathy-West Syndrome, and Sensorineural Hearing Loss.

Mutations in B3GALNT2, encoding a glycosyltransferase enzyme involved in α-dystroglycan glycosylation, have been recently associated with dystroglycanopathy, a well-recognized subtype of congenital muscular dystrophy (CMD). Only a few cases have been reported with B3GALNT2-related dystroglycanopathy with variable severity ranging from mild CMD to severe muscle-eye-brain disease. Here, we describe a child with a novel homozygous nonsense mutation in B3GALNT2. The affected child has severe neurological disease since birth, including muscle disease manifested as hypotonia, muscle weakness, and wa

Mediators in the randomized trial of Child- and Family-Focused Cognitive-Behavioral Therapy for pediatric bipolar disorder.

Mediation analyses can identify mechanisms of change in Cognitive-Behavioral Therapy (CBT). However, few studies have analyzed mediators of CBT for youth internalizing disorders; only one trial evaluated treatment mechanisms for youth with mixed mood diagnoses. This study evaluated mediators in the randomized trial of Child- and Family-Focused CBT (CFF-CBT) versus Treatment As Usual (TAU) for pediatric bipolar disorder (PBD), adjunctive to pharmacotherapy. Sixty-nine children ages 7-13 with PBD were randomly assigned to CFF-CBT or TAU. Primary outcomes (child mood, functioning) and candidate m

Inadequate child supervision: The role of alcohol outlet density, parent drinking behaviors, and social support.

Supervisory neglect, or the failure of a caregiver to appropriately supervise a child, is one of the predominant types of neglectful behaviors, with alcohol use being considered a key antecedent to inadequate supervision of children. The current study builds on previous work by examining the role of parental drinking and alcohol outlet densities while controlling for caregiver and child characteristics. Data were obtained from 3,023 participants via a telephone survey from 50 cities throughout California. The telephone survey included items on neglectful parenting practices, drinking behaviors

Evaluating clinically significant change in mother and child functioning: comparison of traditional and enhanced behavioral parent training.

The Strategies to Enhance Positive Parenting (STEPP) program, an enhanced behavioral parent training (BPT) intervention, was developed to improve engagement in and outcomes following treatment for single-mother families of school-age youth with attention-deficit/hyperactivity disorder (ADHD). A previous randomized clinical trial of the STEPP program demonstrated that the intervention resulted in statistically significant improvements at the group-level in child oppositional behavior, various areas of child impairment, parental stress, and parenting behavior, relative to a wait-list control con

[Child health check-ups as an instrument in child welfare. First results of compulsory check-up systems in German federal states].

Routine well-child visits, implemented as a means of secondary prevention and covered by health insurance, lead to early identification of disorders and abnormalities in child development."Guiding principles for children" (by the G-BA) have determined the content of the eleven examinations, ranging from U1 immediately after birth to J1 in adolescence; eight of them take place within the first four years of age. Since cases of child maltreatment, neglect, or abuse became public in 2007, almost all German federal states have established mandatory examination and notification processes in the new
