Transoral open reduction with rigid internal fixation for subcondylar fractures of the mandible using a small angulated screwdriver system: is endoscopic assistance necessary?

PURPOSE: Although endoscopically assisted transoral open reduction and internal fixation of condylar mandible fractures is currently a popular technique, the need for it and its benefits remains unclear. This prospective study evaluated the efficacy and safety of open reduction and rigid internal fixation of subcondylar fractures of the mandible using a new small angulated screwdriver system without endoscopic assistance. PATIENTS AND METHODS: Fifteen patients with 15 linear subcondylar fractures were treated using this intraoral approach from June 2007 through March 2010 at the Division of O

Comparison between radial head replacement and open reduction and internal fixation in clinical treatment of unstable, multi-fragmented radial head fractures.

The objective of this study was to compare replacement of the radial head by metal prostheses with open reduction and internal fixation (ORIF) for the treatment of unstable, multi-fragmented radial head fractures. A prospective randomised controlled trial was employed to investigate 45 patients with unstable, multi-fragmented fractures of the radial head, from January 2004 to June 2007. The patients were randomised to two groups: the ORIF group and the radial head replacement group. Over the next two years, follow-up assessments recorded Broberg and Morrey scores and postoperative complication

Salter-Harris Type III and IV medial malleolar fractures: growth arrest: is it a fate? A retrospective study of 48 cases with open reduction.

Salter-Harris type III and IV medial malleolar fractures (MacFarland fracture) is a joint fracture of the ankle in children. The fracture line passes through the medial part of the lower epiphyseal disk of the tibia. Prognosis is dominated by later risk of misalignment and osteoarthritis. The aim of this study was to evaluate the functional and radiological outcome of these fractures. We retrospectively analyzed the cases of 48 children with MacFarland fractures (31 boys and 17 girls), mean age at the time of trauma 11 years 6 months (range, 8-15 years). The fractures were classed into two gro

Wound-healing risk factors after open reduction and internal fixation of calcaneal fractures: does correction of Bohler's angle alter outcomes?

The study reviewed in this article evaluated a group of patients who underwent surgical therapy for calcaneal fractures at a Level I trauma center. One group of patients was treated after outpatient referral to the center, whereas the other group was admitted to, and underwent surgery at, the center. This study attempted to determine which patient risk factors or injury characteristics might lead to an increased rate of wound-healing complications. Bohler's angle is a classic radiographic method of determining the severity of calcaneal injury in this group of patients. The question posed by th

Predictive value of a preoperative computed tomography-derived perifracture soft-tissue envelope index for fracture-related infection after open reduction and internal fixation of tibial plateau fractures.

BACKGROUND: Fracture-related infection (FRI) is a major complication after open reduction and internal fixation (ORIF) of tibial plateau fractures. This study evaluated whether a preoperative computed tomography (CT)-derived soft-tissue envelope index (STEI) was associated with 12-month FRI and whether it provided incremental value for preoperative risk stratification. METHODS: This retrospective single-center cohort study included 347 adults with unilateral closed tibial plateau fractures who underwent ORIF between January 2020 and December 2024. The STEI was defined as the mean ratio of sof

General Anesthesia vs Monitored Anesthesia Care With Local Anesthetic Infiltration (MAC-VL) for Ankle Fracture Open Reduction and Internal Fixation: Impact on Recovery Metrics and Safety.

BACKGROUND: Monitored anesthesia care (MAC) combined with regional anesthetic techniques is commonly used for open reduction and internal fixation (ORIF) of unstable ankle fractures. When compared to general anesthesia (GA), MAC with regional anesthesia has been associated with reduced systemic adverse effects and shorter recovery times, although regional anesthesia carries additional costs and potential complications of its own. A new technique involving the infiltration of high-volume, low-concentration (VL) local anesthetic was therefore developed. When used in conjunction with MAC, we refe

Open Reduction of Lateral Femoral Condyle Impaction Fracture Using Autologous Bone Graft in the Setting of Anterior Cruciate Ligament Reconstruction With Bone-Patellar Tendon-Bone Autograft.

Lateral femoral condyle (LFC) impaction fractures are an increasingly recognized pathology occurring concomitantly with anterior cruciate ligament (ACL) rupture. These lesions are important to identify and treat because of the impact they have on knee instability, meniscal degeneration, a faster rate of progression to osteoarthritis, and increased chance of ACL graft rerupture. The technique presented is intended for use in large and deep LFC impaction fractures in conjunction with ACL reconstruction with bone-patellar tendon-bone autograft. It uses an open anterolateral arthrotomy and a subch

Clinical outcomes of closed reduction vs. small-incision-assisted open reduction with intramedullary nailing in complex comminuted femoral shaft fractures (AO/OTA 32-C): a retrospective cohort study.

BACKGROUND: Intramedullary nailing (IMN) is the preferred treatment owing to its minimally invasive nature, high healing rates, and reduced stress shielding. However, the optimal reduction method for complex comminuted fractures (AO/OTA 32-C) has been controversial. Closed reduction preserves blood supply but requires extensive fluoroscopy and technical expertise. Small-incision-assisted open reduction enhances visualisation and facilitates reduction but entails slightly increased soft tissue exposure. METHODS: This retrospective cohort study analysed 70 patients with AO/OTA 32-C femoral shaf

Conversion Total Knee Arthroplasty After Tibial Plateau Fracture Open Reduction and Internal Fixation: A Propensity-Matched Analysis of 90-Day and 1- and 3-Year Outcomes.

BACKGROUND: Conversion total knee arthroplasty (TKA) following tibial plateau fracture (TPF) open reduction and internal fixation (ORIF) is surgically complex. This study utilized a national-level database to compare 90-day and 1- and 3-year outcomes between TKA following TPF ORIF and primary TKA. METHODS: A deidentified database was queried (2003 to 2023) for patients of ≥ 18 years undergoing conversion TKA within 10 years of TPF ORIF (n = 615). A primary TKA control group (n = 224,331) was identified for one-to-one propensity score matching, yielding 613 patients per group. The primary outc

Open Reduction and Internal Fixation and Intraoperative Exploration of the Superior Retinacular Arterial System in Young Adults Garden III Femoral Neck Fracture: A 10 Case Report.

At present, there is no uniform standard for the treatment of femoral neck fractures (FNFs) in young adults, and the current strategy is inclined to the native hip joint preservation. Femoral neck fractures in young adult are mostly caused by high-energy violence, and the blood supply of femoral head is severely damaged, which is prone to secondary femoral head avascular necrosis. In this article, we report 10 cases of young adult Garden III FNF treated with open reduction and internal fixation (ORIF) and microsurgical exploration of the superior retinacular vessels from 2015 to 2018, to more

Posterior wall acetabular fracture in a 13-year-old boy treated by open reduction and mini-plate internal fixation: long-term follow-up of 17 years.

BACKGROUND: Acetabular fractures are uncommon in children and adolescents, mainly because of predominant cartilaginous component and strong surrounding ligaments. Although acetabular fractures at this age can lead to significant disability, there is no consensus regarding management, which continues to be controversial. Particularly, long-term outcome after operative management has not been evaluated. CASE PRESENTATION: We report a case of a 13-year-old boy skeletally immature who presented with an isolated acetabular fracture involving the posterior wall secondary to a traumatic hip dislocat
