PURPOSE OF REVIEW: Visual loss from optic neuropathy and ophthalmoplegia involving multiple cranial nerves are the hallmarks of an orbital apex syndrome. Historically, the terms superior orbital fissure, orbital apex, and cavernous sinus have been used to define the anatomic locations of a disease process. However, the diagnostic evaluation and management is similar for each of these entities. The authors reviewed the literature on the diagnosis and evaluation of disorders involving the orbital apex.
RECENT FINDINGS: High-resolution MRI is the preferred modality for evaluating most lesions in…

PURPOSE: To report 2 cases of severe necrotising orbital cellulitis which illustrate the need for aggressive surgical management to prevent blindness.
METHODS: The case records of 2 patients with necrotising orbital cellulitis were reviewed.
RESULTS: Both patients had orbital cellulitis associated with sinusitis. Each case was characterised by the rapid development of severe systemic toxicity, extensive soft tissue necrosis and abscess formation. One patient developed panophthalmitis and the eye had to be eviscerated. The other patient underwent repeated surgical drainage of multiple orbital…

PURPOSE: To describe two cases of orbital abscess in neonates and to review the literature of orbital cellulitis in neonates.
DESIGN: Two interventional case reports.
METHODS: Photographs, orbital computed tomography scans, and full pediatric examination were obtained in two cases of orbital abscess in neonates.
RESULTS: Acute ethmoiditis with orbital abscess formation was found in both infants. In one of them, Staphylococcus aureus was identified as the source of infection.
CONCLUSIONS: The clinical findings of our cases concur with the literature (eight cases) indicating that orbital abs…

Orbital foreign bodies are found in a variety of instances according to the foreign body, its volume, its nature and its location. Clinical cases are variable. In an emergency, the suspicion of an orbital foreign body in case of an orbital wound is studied as well as the surgical indications for extraction of this foreign body. Later, a misdiagnosed orbital foreign body can lead to infectious complications such as cellulitis, orbital abscess, fistula, or cerebral abscess with possible mortality. The different cases are presented with their surgical indications.

PURPOSE: To study the demographic factors, clinical diagnosis, results of investigation, modalities of treatment and their outcome in ocular and extraocular cysticercosis.
METHOD: A total of 20 patients were recruited for the study. Ultrasonography and computed tomography were done for all the patients. Serial ultrasound was obtained in patients receiving medical treatment. Therapy was individualized according to the location of the cyst.
RESULTS: The commonest clinical presentation was proptosis (9 of 20) with restriction of ocular movements, followed by subconjunctival cyst, subretinal cys

The literature on MRI of orbital schwannomas is limited. The appearances in three patients with an orbital schwannoma were reviewed. A superior orbitotomy through a subfrontal craniotomy revealed a schwannoma in all cases. MRI characteristics of very low signal on T1-weighted images and homogeneous postcontrast enhancement may be helpful for differentiating schwannomas from other intraconal masses.

Because of the confined nature of the orbital space, many disease processes such as cancers, infections, or inflammations present with overlapping clinical manifestations. Also, with the multitude of muscular, neurovascular, sensory, and glandular structures located close to each other in this space, precise anatomic localization of various biologic processes can be difficult. Nevertheless, the aim of this review is to highlight recent publications about unusual orbital infections. The review begins with anatomic and etiologic classification of orbital infectious diseases. It then progresses a…

Idiopathic orbital myositis is a subtype of nonspecific orbital inflammation primarily involving the extraocular muscles. It occurs most frequently in young to middle-aged adults with a 2 to 1 female predominance. The cardinal clinical feature is orbital pain exacerbated by eye movement. Other common findings include diplopia, proptosis (which is generally minimal), conjunctival injection and chemosis, and periorbital edema. Thyroid eye disease is commonly confused with orbital myositis, but the latter is characterized by a more acute onset, more severe pain, and a rapid response to systemic c…

An Abyssinian cat presented with epistaxis, melaena and retinal vascular changes. Plasma protein levels were markedly elevated and a monoclonal gammopathy was present on serum protein electrophoresis. Based on these laboratory findings, hyperviscosity syndrome was assumed to be responsible for the clinical signs. The cat responded initially to fluid therapy, but relapsed and then developed unilateral exophthalmia. The animal was euthanased and an extramedullary plasmacytoma of the right orbit was diagnosed at postmortem examination.

BACKGROUND: The occurrence of proptosis at birth is unusual and may be associated with a variety of tumours and structural abnormalities. Congenital orbital teratoma is a rare cause of congenital proptosis.
METHODS: A case report is presented of a female infant with gross right proptosis.
RESULTS: Computed tomography demonstrated a characteristic multicystic structure with no intracranial involvement. Histological examination found tissues derived from all three germ cell layers, consistent with a congenita orbital teratoma. The tumour was successfully removed preserving the globe and vision…

What causes Graves' ophthalmopathy is still a mystery, but the disease process results from a complex interplay of genetic and environmental factors. Genes such as those encoding for human leukocyte antigens, cytokines or putative target antigens may determine a patient's susceptibility to the disease and the disease severity, but environmental factors may determine its course. During the last 5 years, significant progress has been made towards a more in-depth understanding of the initiating events of the orbital immune process which occur in the context of autoimmune thyroid disease. Once est…

Many pathologic processes may lead to vertical orbital dystopia. We reviewed 47 consecutive cases seen over a 13-year period. Twenty-nine patients underwent eye leveling procedures to improve cosmesis, 2 of these by camouflage procedures and 27 by orbital translocation. Ten patients had 16 secondary operations. There was one death, serious complications occurred in 3 patients, and nuisance complications occurred in 20 others. Seven patients developed diplopia postoperatively, and in 6 patients it was troublesome. In these, it resolved fully in 2 patients, improved to be of no consequence in 2,…

Orbital sonography with color-flow Doppler imaging is a relatively new technology with significant application in the pediatric patient. This review stresses the primary indications for pediatric ophthalmic ultrasound and also discusses those instances where the use of ultrasound supplements other imaging studies.

A review was made of the clinical data of 28 patients with metastatic orbital disease diagnosed in our clinic between 1972 and 1993. Fifteen were men and 13 were women. The average age at diagnosis was 58.7 years. The right orbit was involved in 12 and the left orbit in 14 cases. Two cases had bilateral orbital involvement. Breast carcinoma was the most frequent tumor (8 of 28, 28.6%) followed by neuroblastoma (7 of 28, 25.0%), lung carcinoma (6 of 28, 21.4%), prostate carcinoma (3 of 28, 10.7%), gastrointestinal carcinoma (2 of 28, 7.1%), renal cell carcinoma and thyroid carcinoma (1 of 28, 3…

We examined 9 consecutive cases of unilateral orbital myositis (7 women and 2 men; age range 15-46 years) presenting to Casualty. Only 3 were correctly diagnosed on the first visit. Eight patients exhibited globe retraction in the acute stages and, after treatment with systemic steroids, all made a full recovery. None of the patients had associated systemic disease and all remain well over a 6-12 month follow-up period. Orbital myositis may be a more common condition than previously thought; it can present with a variety of clinical signs, and may be difficult to diagnose in the early stages. …
