Necrotizing odontogenic fasciitis of head and neck extending to anterior mediastinum in elderly patients: innovative treatment with a review of the literature.

BACKGROUND: Necrotizing fasciitis (NF) of odontogenic origin affecting the head and neck region is a rare but serious clinical condition, which, if diagnosed late, can lead to a fatal outcome. The early diagnosis of necrotizing fasciitis can be difficult. Delay in diagnosis leads to increase in the area of necrosis with a resulting increase in cosmetic deformity and life-threatening complication. In this study, we present two cases of elderly patients with aggressive NF affecting the neck and anterior mediastinum, which were of odontogenic origin. METHODS: In the two patients selected necroti

Successful catheter ablation targeting triggering premature ventricular contractions and continuous propagating activity: Suppression of ventricular fibrillation storms in ischemic cardiomyopathy.

UNLABELLED: Ventricular fibrillation (VF) storm is a life-threatening condition that is particularly challenging to manage when resistant to conventional therapies such as antiarrhythmic drugs, deep sedation, overdrive pacing, and hemodynamic support. We report a case of a 55-year-old male with ischemic heart disease and recurrent VF storm unresponsive to initial treatments, requiring percutaneous cardiopulmonary support to stabilize hemodynamics. A left ventricular inferoseptal Purkinje-related premature ventricular contraction (PVC) was identified as the VF trigger, with continuous propagati

A Novel Anatomic Classification System and Management Protocol for Odontogenic Cervicofacial Necrotizing Fasciitis.

OBJECTIVE: Cervicofacial necrotizing fasciitis (CNF) is a rare but life-threatening condition, most often secondary to odontogenic infections. The aim of this study was to propose a new, anatomically based classification system for CNF of odontogenic origin and to outline a management protocol. METHODS: This retrospective single-center study included all patients diagnosed with CNF of odontogenic origin between June 2017 and May 2024. Sociodemographic data, anatomic extent of disease, treatment modalities, intensive care unit (ICU) requirements, grafting, and physiotherapy were recorded and a

Search for an aetiological virus candidate in chronic lymphocytic leukaemia by extensive transcriptome analysis.

As an approach to determining the aetiology of chronic lymphocytic leukaemia (CLL), we searched for a virus expressed in human CLL B-cells by combining high-throughput sequencing and digital subtraction. Pooled B-cell mRNA transcriptomes from five CLL patients and five healthy donors were sequenced with 454 Life Sciences technology. Human reads were excluded by BLAST (Basic Local Alignment Search Tool) and BLAT (BLAST-like alignment tool) searches. Remaining reads were screened with BLAST against viral databases. Purified B-cells from two CLL patients, with and without stimulation by phorbol-e

Prognostic factors in pathologic fractures secondary to metastatic tumors.

OBJECTIVE: Pathological fractures caused by metastases sharply decrease the quality of life and increase mortality rates for patients with malignant neoplasias. Orthopedic advances in osteosynthesis and endoprosthesis have been beneficial in the prevention and treatment of such fractures. The objective of our study was to determine which prognostic factors for pathologic fractures treated in our Service were significant. METHOD: This was a retrospective study enrolling 112 patients treated for pathologic fractures secondary to metastatic tumors between April 1994 and December 2004 in our Serv

The changing incidence and sites of colorectal cancer in the Israeli Arab population and their clinical implications.

Israeli Arabs have been at low risk for colorectal cancer (CRC) and had mainly proximal cancer, but increasing CRC is now noted. We examined this trend and CRC site and compared them to the total Jewish population and to the low-risk Jews of Asian-African origin. Israel Cancer Registry CRC data, 1982-2002, for Arabs and Jews was computed by gender, age and site: rectal cancer included recto-sigmoid junction; "right-sided" CRC included the proximal colon up to and also the splenic flexure. During 1982-2002, Arab CRC trends increased significantly in both sexes due to left-sided CRC (women, p =

Microscopic colitis: Struggling with an invisible, disabling disease.

BACKGROUND AND AIMS: Microscopic colitis causes chronic or recurrent nonbloody, watery diarrhoea, which is associated with urgency, faecal incontinence and abdominal pain. The patient's health-related quality of life is often impaired. In microscopic colitis, health-related quality of life has been studied using questionnaires originally constructed and validated for patients with inflammatory bowel disease. The aim of this study was to explore the impact of microscopic colitis on everyday life. METHODS AND RESULTS: Inductive, qualitative, semi-structured interviews were performed with 15 per

Cultural adaptation and validation of the Sidamic version of the World Health Organization Quality-of-Life-Bref Scale measuring the quality of life of women with severe preeclampsia in southern Ethiopia, 2020.

BACKGROUND: Women with severe preeclampsia often present with more health complaints compared to those with uncomplicated pregnancies. Estimating the quality of life of women affected with severe preeclampsia could provide direction for further interventions. However, the current measurement of the quality of life has not been culturally adapted and validated for this population. This study aimed to translate, culturally adapt, and test the reliability and validity of the World Health Organization Quality-of-Life-Bref Scale (WHOQOL-BREF) in southern Ethiopia among women with severe preeclampsi

The effects of lead administration during development on lithium-induced polydipsia and dopaminergic function.

Previous studies have demonstrated that postnatal (days 2-29 of life) administration of lead (200 mg/kg/day by gavage) to Long-Evans rats caused permanent increases in lithium-induced polydipsia (LIP). These lead-induced increases in LIP were apparently not of renal origin, did not occur in animals treated with lead after day 30, and persisted for at least 6 months. The present studies have narrowed the dose-time window for lead-induced increases in LIP. The first study showed that continuous administration of lead (200 mg/kg/day, p.o.) in the form of lead acetate during days 2-9 of life cause

Life course models for upper aero-digestive tract cancer.

Upper aero-digestive tract (UADT) cancers are collectively cancers of various human body sites, such as the oral cavity, pharynx, oesophagus and larynx. Worldwide, they are the fourth most frequent cancer type and the fourth most common cause of mortality from cancer. Many studies have shown that several chronic diseases, such as cancer, which occur more commonly in later adulthood, are influenced by social and psychological circumstances during birth, childhood, adolescence and early adult life. It is suggested that the build up of problematic circumstances throughout life is the cause of dis

Clinical management of hereditary gingival fibromatosis: Case report with 13 years follow-up.

BACKGROUND: Hereditary gingival fibromatosis (HGF) is one of the categories of non-plaque-induced gingival diseases of genetic origin. Current studies show high genetic heterogeneity and suggest that not all forms of HGF are the same and that more than one biological mechanism may result in gingival growth. This report presents a case of syndromic HGF with generalized and complex clinical manifestations associated with other conditions such as body hypertrichosis and hearing deficit. METHODS: This is a case report of a male patient with HGF, detailing the orthodontic and periodontal managemen

Prenatal diagnosis of cardiac rhabdomyoma associated with tuberous sclerosis: report of 3 cases.

Cardiac rhabdomyoma is the most common cardiac tumor in fetal life, accounting for 60-86% of primary fetal cardiac tumors. It is primarily benign, originating form myocardial muscles and consisting of immature myocytes. About 50-60% of these tumors are associated with tuberous sclerosis. In this report, we present the clinical course and discuss the importance of prenatal diagnosis of cardiac tumors and their follow-up after birth.

Habit reversal training for the itch-scratch cycle associated with pruritic skin conditions.

Habit reversal therapy was originally developed by Azrin and Nunn in 1977 as a multicomponent treatment for nervous tics and habits. Their premise for clinical intervention was that an old habit can be broken by replacing it with a new, more desirable habit. Dermatology nurses trained in habit reversal therapy can enhance system management and quality of life for patients. Recommendations for the evaluation of habit reversal in patients with eczema and prurigo nodularis are made.

Genetic variation in immunoregulatory pathways and atopic phenotypes in infancy.

BACKGROUND: Asthma is a chronic respiratory disease that often originates in early childhood. Although candidate gene studies have identified many potential asthma susceptibility genes in adult populations, few have studied associations with immune phenotypes in the first year that might be early clinical markers of asthma. OBJECTIVE: The aim of this study was to assess the contribution of genetic variation to cytokine response profiles and atopic phenotypes in the first year of life in the Childhood Origin of Asthma cohort. METHODS: Two hundred seven European American children participating

The importance of the life cycle perspective for safety assessment in the European Commission's Safe and Sustainable by Design Framework.

The European Commission Safe and Sustainable by Design (SSbD) framework represents a paradigm shift in innovation by integrating safety and sustainability considerations from the earliest stages of development, guided by a life cycle perspective. A key driver of this shift is the extension of safety beyond siloed considerations towards a comprehensive life cycle approach. However, this extension of safety to the entire life cycle remains insufficiently understood and is often underestimated or even neglected by practitioners, largely due to a lack of understanding of its practical implications

A rare cause of massive haemoptysis in an adolescent.

Haemoptysis is defined as the expectoration of blood or blood-tinged sputum originating from the lower respiratory tract. Massive haemoptysis, although rare, can often cause life-threatening asphyxia, respiratory failure, shock and, rarely, mortality. Pulmonary tuberculosis (PTB), although common, rarely presents with massive haemoptysis. This case report highlights an adolescent male who presented with life-threatening haemoptysis owing to PTB which was managed by successful pulmonary vascular embolisation.Abbreviations: AV: arteriovenous; BAE: bronchial artery embolisation; BPF: bronchopulmo

Melatonin and Kidney Health: From Fetal Stage to Later Life.

Melatonin, an endogenous hormone mainly released at night by the pineal gland, has multifaceted biofunctions. Emerging evidence points to melatonin having a crucial role in kidney health and disease. As the prevalence of chronic kidney disease (CKD) is still rising, a superior strategy to advance global kidney health is needed to not just treat CKD, but prevent it early on. Adult kidney disease can have its origins in early life. This review aims to evaluate the recent literature regarding melatonin's effect on kidney development, its clinical uses in the early stage of life, animal models doc
