Nocardial osteomyelitis: a case report and review of the literature.

The occurrence of human nocardiosis is increasing in both immunocompetent as well as immunosuppressed patients. A 63-yr-old male construction worker presented with complaints of left hip pain and rapidly enlarging masses on his left neck and chest. He had been treated for a pneumonia of unknown etiology 3 mo prior to admission. A debridement of the left femur was performed and the curetted material was positive for Nocardia asteroides. Osteomyelitis due to the hematogenous spread of Nocardia asteroides is rare, but will undoubtedly be seen with increasing frequency. The literature, as well as …

[Salmonella osteomyelitis (Salmonella infection with manifestation in the tibia].

A report is given of a special form of osteomyelitis the infection of the bone marrow caused by salmonellae. At the same time the development of infection and the attendant disease, which lead to this infection are dealt with. By means of an almost classical medical case the form of the disease is discussed.

Pyogenic spinal osteomyelitis: a review of 61 cases.

The presentation and outcome of 61 cases of nontuberculous spinal osteomyelitis were reviewed. Although the commonest presentation was subacute, with back pain predominating, 10% had septicaemia and 7% paraperesis or paraplegia. Most infections were caudal to the fourth thoracic vertebra. One third were associated with preceding urinary, respiratory, dental or abdominal sepsis. Staphylococcus aureus was the infecting organism in 85% of isolates. Delay in diagnosis was frequent. There were four deaths, and seven individuals remained severely disabled: outcome was otherwise satisfactory. An algo…

Blastomycotic osteomyelitis of the mandible: successful treatment with ketoconazole.

Blastomycotic osteomyelitis of the mandible in a 40-year-old man was treated with ketoconazole for 26 months. Clinical, roentgenographic, and scintigraphic improvement was observed, and 13 months after cessation of treatment he remains free of disease. This case and three other previously reported cases indicate that ketoconazole is effective in the treatment of blastomycotic osteomyelitis.

Vascularized muscular and musculocutaneous flaps for management of osteomyelitis.

Thirty-three cases of recalcitrant osteomyelitis treated by free tissue transfer are reviewed. The immediate success rate was 76 per cent; however, on follow-up for at least 1 year, there was a 12 per cent rate of recurrent drainage. The authors conclude that this is a good operation for this problem but that it is not a panacea because if followed long enough, a significant number of patients will develop recurrent drainage.

Vapor-permeable membrane therapy for ulcers of osteomyelitis.

Two patients with chronic ulcerations secondary to arterial insufficiency of the distal extremities were found to have concomitant osteomyelitis immediately beneath the ulcers. An adjunctive would care regimen is described in which local excision or debridement of the involved bone with subsequent systemic antibiotic therapy was the initial treatment. The resultant cutaneous defect was then covered with a vapor-permeable membrane and allowed to close by secondary intention. Rapid and complete healing occurred in both cases in spite of the compromised arterial supply to both areas. This techniq…

Beta-lactam therapy of osteomyelitis and septic arthritis.

The goal of therapy in osteomyelitis is to eradicate established bone infection and to prevent progression into the chronic form. Beta-lactams generally lack organ toxicity or serious side effects when used in high doses for the prolonged intervals necessary to eradicate osteomyelitis. Also, the spectrum of many beta-lactams covers the usual etiologic agents of bone and joint infections. Penetration of beta-lactams into joint fluid is variable but adequate. The significance of measurable "bone levels" is unclear. Staphylococcus aureus in some series accounts for up to 80% of acute haematogenou…

Osteomyelitis in dogs and cats caused by anaerobic bacteria.

Localised osteomyelitis was diagnosed in 2 dogs and 2 cats. The disease was caused by fight wounds in 3 cases. Radiographic examination demonstrated a circumscribed zone of cortical bone lysis, sequestra and periosteal new bone. Each case was treated surgically by sequestrectomy and debridement. Infection was due mainly to anaerobic bacteria. The pathogenic bacteria isolated from the lesions of dogs were Actinomyces viscosus, Fusobacterium nucleatum and Bacteroides spp, and from the lesions in cats were Clostridium villosum , Peptostreptococcus anaerobius, Wolinella recta and Bacteroides gingi…

Hematogenous group B streptococcal osteomyelitis in an adult.

We have reported a case of group B streptococcal vertebral osteomyelitis in a 55-year-old alcoholic man. This may be the first reported case of group B streptococcal osteomyelitis in an adult that was not secondary to surgery, ischemic peripheral vascular disease, or a contiguous infection.

[Osteomyelitis of the turbinates and its significance in chronic sinusitis].

We described the treatment of hyperplasia of the turbinate by sub-mucous resection of the inferior turbinate in 1981. The conchal bones removed were examined, and a histological study is now presented. In most cases we found a nonbacterial inflammation of the bone marrow of the inferior turbinate. The bearing of this and other changes in the bone marrow on the increase in size of the bone causing reduced ventilation of the nose and the paranasal sinuses, leading to sinusitis are discussed.

Chronic haematogenous osteomyelitis; studies on an experimental model.

The progression of acute haematogenous osteomyelitis into a chronic infection was investigated in a group of ten dogs in which the infection was produced by injection of micropaque barium mixed with Staphylococcus aureus or Salmonella Group C-2 into the tibial nutrient artery. Antibiotics were not used. Twenty four months later the infected limb of the surviving animals exhibited clinical, histological, radiological and microbiological changes which closely resembled those found in chronic haematogenous osteomyelitis (CHO) in humans. We observed spontaneous fractures, skin fistulae, bone seque…

Primary osteomyelitis of the sternum due to Salmonella hirschfeldii.

The case of a 71 year-old man with primary osteomyelitis of the sternum caused by Salmonella hirschfeldii is reported. The diagnosis was confirmed by culture, the bony defect was curetted to remove all dead bone and the patient was successfully treated with 6 weeks of high-dose ampicillin (2 g q6h). The treatment is particularly important because inadequate treatment may result in death and overtreatment may cause deformity and instability of the chest wall. Primary osteomyelitis of the sternum is a rare entity and this is apparently the first such case to be reported in the English literature…
