Propofol and arrhythmias: two sides of the coin.

The hypnotic agent propofol is effective for the induction and maintenance of anesthesia. However, recent studies have shown that propofol administration is related to arrhythmias. Propofol displays both pro- and anti-arrhythmic effects in a concentration-dependent manner. Data indicate that propofol can convert supraventricular tachycardia and ventricular tachycardia and may inhibit the conduction system of the heart. The mechanism of the cardiac effects remains poorly defined and may involve ion channels, the autonomic nervous system and cardiac gap junctions. Specifically, sodium, calcium a

Is seborrhea a sign of autonomic impairment in Parkinson's disease?

An increase of sebum excretion rate (SER) is frequently observed in patients suffering from Parkinson's disease (PD). Some authors attribute it to the hyperactivity of the parasympathetic system, while others consider the possible action of androgens or of MSH-hormone. The aim of our study was to verify and quantify SER in 70 parkinsonian patients and compare it with SER in 60 normal subjects. We found higher values of SER in male subjects, both in normal and in parkinsonian patients. The highest rate of excretion was observed in parkinsonian males, in agreement with the possible main role of

Autonomic changes during wake-sleep transition: a heart rate variability based approach.

Autonomic function during sleep and wakefulness has been extensively investigated, however information concerning autonomic changes during the wake to sleep transition is scarce. The objective of the present study was to non-invasively characterize autonomic function and additional physiologic changes during sleep onset in normal and abnormal sleep. The estimation of autonomic function was based on time-frequency analysis of the RR interval series, using the power components in the very-low-frequency range (0.005-0.04 Hz), low-frequency (0.04-0.15 Hz), and high-frequency range (0.15-0.5 Hz). Rapid eye movement sleep parasympathetic activity predicts wake hyperarousal symptoms following a traumatic event.

Heart rate variability (HRV) can be used to assess changes in output of the parasympathetic nervous system (PNS). Considering that patients with post-traumatic stress disorder (PTSD) often experience disturbances in sleep, arousal, and autonomic functioning, we sought to explore the association of PNS activity during sleep with hyperarousal symptoms of PTSD. Because a broad literature supports the importance of rapid eye movement (REM) sleep in PTSD, REM-sleep features were specifically examined as predictors of PTSD symptom severity. A total of 90 participants, primarily civilian and female, …

Two weeks of remote ischaemic preconditioning alters sympathovagal balance in healthy humans.

NEW FINDINGS: What is the central question of this study? Delayed cardiovascular responses occur following a single bout of remote ischaemic preconditioning (RIPC). Is heart rate variability (HRV), a surrogate marker of cardiac vagal control, able to detect a delayed effect after a single bout of RIPC? Do repeated bouts of RIPC further alter HRV? What is the main finding and its importance? Indices of HRV indicated a shift in sympathovagal balance toward greater parasympathetic activity following 2 weeks of RIPC but not after a single bout of RIPC. Thus, repeated bouts of RIPC were necessary t

Exploring fetal response to acidosis in ewes: Choosing an adequate experimental model.

INTRODUCTION: Knowledge of fetal physiology during labor has been largely generated from animal models. Our team recently developed a new index to assess parasympathetic activity using different experimental protocols to obtain acidosis. The objective of the present study was to discuss the different protocols and to review other models proposed in the literature. MATERIAL AND METHODS: Pregnant ewes underwent a surgical procedure at the 123±2 days gestational age (term=145 days). Three experimental protocols were used: protocol A consisted of 25%, 50% and 75% umbilical cord occlusion (UCO) fo

Muscarinic receptor agonists and antagonists: effects on ocular function.

Muscarinic agonists act mainly via muscarinic M₃ cholinoceptors to cause contraction of the iris sphincter, ciliary muscle and trabecular meshwork as well as increase outflow facility of aqueous humour. In the iris dilator, the effect of muscarinic agonists is species dependent but is predominantly relaxation via muscarinic M₃ receptors. In the conjunctiva, muscarinic agonists stimulate goblet cell secretion which contributes to the protective tear film. Muscarinic M₂ and M₃ receptors appear mainly involved. In the lens muscarinic agonists act via muscarinic M₁ receptors to produce depolarizat

Cardiac manifestations of Anderson-Fabry disease in children and adolescents.

AIM: Fabry disease (Fabry) is a rare X-linked disorder caused by a deficiency of the lysosomal enzyme alpha-galactosidase A. The progressive accumulation of the major substrate, globotriaosylceramide, leads to renal dysfunction and hypertrophic cardiomyopathy, which are reported to become apparent in the third decade. This study was performed to determine if signs of cardiac manifestations of Fabry are seen in younger Fabry patients. METHODS: Twenty children and adolescents of <or=18 years of age with confirmed Fabry underwent a standard 12-lead electrocardiogram (ECG), a 2-h Holter ECG, bloo

Intestinal neuronal dysplasia (type B) causing fatal small bowel ischaemia in an adult: a case report.

Intestinal neuronal dysplasia (IND) is a well-documented but controversial entity defined as a congenital malformation of gastrointestinal innervation caused by dysplastic embryonal development of the enteric nervous system. Mainly described as a colonic disorder mimicking Hirschsprung's disease in the paediatric population, involvement of the ileum is rare. We report a case of fatal IND of the small intestine in a 58-year-old man presenting with a recent onset of features of small intestinal obstruction. IND has been described as a cause of colonic pseudo-obstruction and chronic constipation

Responsiveness to autologous sweat and serum in cholinergic urticaria classifies its clinical subtypes.

BACKGROUND: It has been reported that patients with cholinergic urticaria have a type 1 allergy to autologous sweat; however, the pathogenesis of that disorder has not been fully elucidated. OBJECTIVE: We investigated the responsiveness to autologous sweat and serum in patients with cholinergic urticaria in relation to their clinical characteristics. We further classified the clinical subtypes that are clearly characterized by responsiveness to in vivo and in vitro tests as well as their clinical features. METHODS: Intradermal tests with autologous sweat and serum were performed in 18 patien

Constitutive secretion of immunoglobulin A and other proteins into lumina of unstimulated submandibular glands in anaesthetised rats.

Salivary fluid secretion is dependent upon reflex stimuli mediated by autonomic nerves. In order to determine if immunoglobulin A (IgA) and salivary proteins are secreted in the absence of nerve stimulation, small volumes (< 2 microl) of saliva were consecutively collected from the submandibular duct of anaesthetised rats following rest pauses in order to sample the protein contents of the ductal system. Within the first 5 microl of such saliva collected by parasympathetic nerve stimulation, IgA and other salivary proteins reached peak concentrations that were over 20-fold greater than levels

The rationale behind cholinergic drug treatment for dementia related to cerebrovascular disease.

Common to all subtypes of dementia, including Alzheimer's disease (AD), and those associated cerebrovascular disease (CVD), Lewy body pathology and Parkinson's disease, is degeneration of cholinergic neurotransmission. The cholinergic hypothesis of AD is based on evidence of reduced cholinergic markers and decreased numbers of cholinergic neurons and nicotinic acetylcholine receptors (nAChR) in the hippocampus and cortex of the brain-both areas associated with memory, learning and executive function impairments characteristic of cognitive decline in AD. There is growing evidence for the involv

Neurochemical studies of Alzheimer's disease.

Neurochemical studies of post-mortem human brain have made a major contribution to understanding the neuronal basis of neurodegenerative disease and formed the basis of rational therapies for such disorders. The application of this approach to the neurochemical pathology of Alzheimer's disease was pioneered by David Bowen. By combining assessment of post-mortem tissue (where the disease has usually run its full course) with tissue obtained ante-mortem (where the disease course is incomplete), it has been possible to (1) establish which neurones are lost in the disease, (2) determine which neur

Parasympathetic control of cycle length dependence of endocardial ventricular repolarisation in the intact feline heart during steady state conditions.

OBJECTIVE: Parasympathetic modulation of the rate dependence of ventricular repolarisation in vivo during steady state conditions was investigated by analysing the effects of removing vagal activity in three groups of anaesthetised cats. METHODS: Bilateral vagotomy was performed while different levels of background sympathetic tone were present in the control periods of each group: in group 1 (n = 6), cardiac sympathetic nerves were intact; in group 2 (n = 7), bilateral stellate ganglionectomy was performed; and in group 3 (n = 7), beta adrenergic blockade (propranolol, 0.5 mg.kg-1) was perfo

[Pathomorphology of feline dysautonomia (Key-Gaskell syndrome). Histologic, electron microscopic and immunohistologic findings in 4 cats].

The histopathologic, immunohistochemical and electronmicroscopic findings of four dysautonomic cats are presented. The main histopathologic changes were seen in the nervous system, e.g. in sympathetic and parasympathetic ganglia that were similarly affected. Acute cases showed a degeneration of most of the neurons, in one subacute case less neurons and degeneration of some of the remaining neurons were present. Degenerating neurons were swollen or shrunken and had pycnotic, eccentric nuclei. The Nissl substance was lost, the cytoplasma stained eosinophilic and was often vacuolated. Degeneratin

Age-related impairment in complex maze learning in rats: relationship to neophobia and cholinergic antagonism.

Scopolamine was utilized to assess cholinergic muscarinic blockade on the performance of young (3 months) and aged (23 months) male F-344 rats in a 14-unit T-maze task. Prior to training, a portion of each age group received a gustatory neophobia test (percent consumption of a novel sucrose solution) to assess involvement of norepinephrine systems implicated in age-related impairments of rats in other memory tasks. All rats were pretrained in one-way active avoidance (1.0 mA) on 3 consecutive days. Rats meeting criterion (8/10 avoidances on last day) began maze training the next day consisting

[Hemodynamic changes caused by laparotomy during aorto-iliac surgery].

A haemodynamic study was carried out on 53 patients undergoing elective surgery of the abdominal aorta in order to assess the haemodynamic changes consequent to abdominal manipulations prior to aortic clamping. Surgery was carried out under general anaesthesia and ventilation was controlled. The following parameters were monitored: mean arterial pressure (Pa), cardiac index (CI), systolic index (SI), systemic vascular resistances (Rsa), pulmonary vascular resistances (Rpa), heart rate (fC). Cutaneous circulation was monitored using a photoplethysmographic transducer. The following haemodynamic

On the localization and mediation of the centrally induced hyperactive urinary bladder response to L-dopa in the rat.

The hyperactive bladder response observed after central adrenergic stimulation with L-dopa, following enzymatic blockade of its peripheral metabolism, was studied in the rat. The bladder response was recorded by a cystometric procedure. The hyperactive bladder action thus produced was noted after transection of the CNS at the level of the superior colliculi but not when the transection was made caudal to the inferior colliculi. No effect was seen after transection of the cauda equina at L6, i.e. parasympathetic decentralisation of the bladder. It is concluded that the hyperactive bladder react
