A comparative study on the efficacy of kirschner wire tension band combined with anchor cross-suture internal fixation versus partial patellectomy in the treatment of comminuted inferior pole patellar fractures.

OBJECTIVE: The treatment of comminuted inferior pole patellar fractures has long posed a challenge for orthopedic surgeons. This study aims to compare the biomechanical stability and clinical efficacy of Kirschner wire tension band combined with anchor cross-suture fixation versus traditional partial patellectomy in the treatment of comminuted inferior pole patellar fractures. METHODS: A retrospective analysis was conducted on 14 patients who underwent Kirschner wire tension band combined with anchor cross-suture fixation (Group A) in our department of orthopedics from September 2020 to April

[Advances on the treatment of the first-time patellar dislocation in children and adolescents].

The first patellar dislocation in children and adolescents is a common clinical sports injury. The controversy in the treatment strategies poses a significant challenge to long-term functional prognosis of patients. Studies have shown that demographic factors (young age, female gender), skeletal maturity, and anatomical abnormalities (such as trochlear dysplasia (TD), increased distance between tibial tubercle and trochlear groove (TT-TG) were closely related to patellar dislocation. Although conservative treatment is the preferred option, it has a high recurrence rate and is prone to leaving

[Monostotic patellar Paget's disease].

Paget's disease or osteitis deformans is a bone metabolic disease characterized by an increased bone resorption followed by an increased but irregular bone formation. This results in a weakened, deformed bone and an increase bone mass with collagen fibers forming an irregular and pathologic mosaic instead of the parallel symmetry that characterizes healthy bone. It is considered as a non-neoplastic disorder that mimics a bone tumor. Its origin is still uncertain; however, it has been postulated that genetic or environmental factors are involved in its etiology. It rarely occurs in young patien

[Multiple pathology in a Merowingian individual of a graveyard near Insheim (Rhineland-Pfalz)].

The following case report describes in detail a 57 years old (+/- 5 years) male individual from a Franconian graveyard in Insheim, Province of Rhineland-Palatinate, dating to between the 6th and 7th century A.D. The individual displays a number of unusual pathologies. The atlas shows a complete aplasia of the posterior arch, probably resulting in a torticollis. The changed static induced a massive spondylar-arthrotic degeneration of the cervical spine on the right-lateral portion. On the fronto-parietal section two fractures are visible which were caused by a sharp and violent force. The area

Stifle lameness in the horse: a survey of 86 referred cases.

The clinical and radiological characteristics of stifle lameness in 86 horses are described. The majority of these cases had been lame for some weeks before referral. The most frequent diagnoses made were osteochondrosis dissecans (13 per cent) and subchondral bone cyst (38 per cent). Both of these conditions were seen principally in Thoroughbreds at or before the onset of training. The other bone lesions encountered were osteoarthritis (3 per cent), fractures (4 per cent) and "epiphysitis" (1 per cent). The commonest soft tissue condition was an atypical or partial upward fixation of the pate

Patient satisfaction after elective implant removal of the lower extremity - a retrospective cohort study.

PURPOSE: The topic of elective implant removal (IR) in healed fractures of the lower extremity remains controversial, particularly when unspecific symptoms of discomfort, which cannot be quantified, are the primary indication. This study aims to assess indications and outcomes of elective IR of the lower extremity, focusing on unspecific symptoms of discomfort and patient satisfaction postoperatively. MATERIALS AND METHODS: The retrospective cohort study was conducted at a single level I academic trauma center. We included patients who underwent elective IR for healed fractures of the ankle,

Injuries and conditions of the extensor mechanism of the pediatric knee.

The extensor mechanism of the knee is essential to ambulation and is subject to a number of traumatic, congenital, and inflammatory processes. In the pediatric population, the spectrum of pathologic conditions affecting the extensor mechanism is specific to skeletally immature patients. In addition, certain congenital and developmental disorders may further predispose the knee extensor mechanism to injury. The pathologic processes can be subdivided into categories: conditions of the attachments and insertions of the quadriceps and patellar tendons, conditions of the patella, conditions of the

Cement or press-fit? Impact of patellar fixation technique on outcomes after primary TKA: a systematic review.

INTRODUCTION: The optimal fixation technique for patellar components in primary total knee arthroplasty (TKA); cemented versus press-fit, remains controversial. While early press-fit designs were associated with complications, second-generation implants with improved osseointegration features have renewed interest in cementless fixation. METHODS: This systematic review included studies comparing cemented and press-fit patellar fixation in primary TKA. A comprehensive literature search identified 202 studies, of which eight met predefined inclusion criteria. Data were extracted on survivorship

Osteonecrosis: A disabling disease not to be ignored in asthma and atopic conditions.

Osteonecrosis, also referred to as avascular necrosis, is a disease characterized by necrosis or death of a bone secondary to impairment in blood supply. The condition affects the epiphyseal ends of the bones such as the femur and the humerus, but it can also involve the metacarpal and metatarsal bones, the patella, the knee, the vertebrae, and the jaw. A plethora of inflammatory, autoimmune, hematological, thrombotic, and vascular diseases can lead to osteonecrosis. Corticosteroids are intimately linked to the development of osteonecrosis. The frequent use of systemic corticosteroids in patie

Management of the First Patellar Dislocation: A Narrative Review.

First patellar dislocation is a common injury of the knee, involving often adolescents and the active population. The consequences of the first episode can be various and potentially disabling. Among these, acute patellar dislocation can often result in recurrent patellar instability. Recurrent patellar instability is certainly multifactorial but depends primarily on the injury of the medial patellofemoral ligament (MPFL), the major soft-tissue stabilizer of the patella. Some classifications are extremely useful in establishing the diagnosis and therapy in patellofemoral disease, in particular

The surgical treatment of arthrofibrosis of the knee.

Sixty-one lysis of adhesion procedures were performed for arthrofibrosis of the knee between 1981 and 1990. In 43 cases (37 patients) the etiology was prior knee ligament surgery. Ten cases had sustained fractures about the knee and eight had miscellaneous etiologies. Six patients required a second lysis of adhesion procedure. A retrospective analysis of the 43 ligament patients was undertaken to evaluate the results of surgical treatment. All 43 cases were available for followup at an average of 3.6 years. Surgical indications included flexion or extension deficits of > or = 10 degrees or whe

[The "3-in-1" block: myth or reality?].

The technique described by Winnie in 1973 is supposed to provide a regional block of the femoral, femoral cutaneous, and obturator nerves by a single injection within the femoral nerve sheath. This study aimed to assess the diffusion spaces for the local anaesthetic solution used in this technique. The anatomical study included the dissection of 2 adult and 1 foetal cadavers. It was associated with a radiographic study in adult volunteers. About 20 to 60 ml of an isotonic radiographic contrast (iopamidol 150) were injected into the femoral nerve sheath located with the help of a nerve stimulat

Medial Opening-Wedge High Tibial Osteotomy.

BACKGROUND: Mechanical axis deviation (MAD) disrupts the natural force distribution within the knee joint. MAD malalignment imparts atypical tibial plateau loading forces, which can cause painful knee cartilage degeneration, abnormal gait, and reduced activity and mobility1. A medial opening-wedge high tibial osteotomy corrects varus MAD for deformities centered in the proximal tibia, reducing medial knee compartment stress, which often improves pain, function, and self-image2. DESCRIPTION: Surgical planning starts with a history to understand the patient symptoms, potential etiologies, and g

Influence of applied stress on energy dissipation and crack growth in articular cartilage.

Mechanical stress-induced damage to the articular cartilage can result in fracture initiation or significant tissue degeneration leading to osteoarthritis (OA) compromising joint mobility. Despite the clinical significance, a comprehensive understanding of the crack progression in cartilage damage remains elusive due to complex mechanical responses and variable initial crack geometry. This study investigated the impact of physiologically relevant stress levels, ranging from light-weight to high stress activities, on crack propagation in cartilage under compressive cyclic loading. Through empir
